Cíl: Zjistit, zda je nová metoda úspěšná při blokádě obou ilioinguinálních nervů pro zmírnění pooperační bolesti po císařském řezu. Materiály a metody: Mezi lednem 2022 a lednem 2023 bylo do této studie zařazeno 300 pacientek na Porodnicko-gynekologickém oddělení Lékařské fakulty Univerzity Al-Azhar. 150 z těchto pacientů dostalo infiltraci bupivakainu na obě strany blízko přední páteře kyčelní a 150 dostalo normální injekci fyziologického roztoku do stejných míst. Výsledky: Studie porovnávala obě skupiny a objevila významné rozdíly v načasování žádostí o analgetika, intervalu před první ambulancí pacienta, délce pobytu v nemocnici, skóre pooperační bolesti a incidenci pooperační nevolnosti a zvracení, přičemž skupina A na tom byla lépe. Závěr: Po císařském řezu je účinným způsobem, jak zmírnit pooperační nepohodlí a použití analgetik, lokální anestetikum injekce „bupivakainu” k bilaterální blokádě ilioinguinálních nervů.
Aim: To determine whether the novel method is successful in blocking both ilioinguinal nerves to lessen postoperative pain following caesarean surgery. Materials and methods: Between January 2022 and January 2023, 300 patients were enrolled in this study at the Obstetrics and Gynaecology Departments of the Faculty of Medicine at Al-Azhar University. About 150 of these patients received bupivacaine infiltration on both sides close to the anterior superior iliac spine, and 150 received a normal saline injection at the same locations. Results: The study compared the two groups and discovered significant differences in the timing of analgesic requests, interval before the patient’s first ambulation, length of hospital stay, postoperative pain score, and incidence of postoperative nausea and vomiting, with group A performing better. Conclusion: After a caesarean section, the local anaesthetic “bupivacaine” injection used to block the ilioinguinal nerves bilaterally is an efficient way to lessen postoperative discomfort and analgesic use.
AIMS: The aim of the study was to analyse the prognostic and predictive value of LC3A positive' 'Stone Like Structures'' (SLSs) in a large cohort of patients with non-small cell lung carcinoma (NSCLC) and to check its relationship with tumor infiltrating lymphocytes (TILs) and PD-L1 expression. METHODS: Tissue microarrays from 1015 patients diagnosed at the Institute of Pathology and Molecular Pathology, University Hospital Zurich, Switzerland, were stained for LC3A, PD-L1, CD3 and CD68 using automated tissue stainer Ventana Benchmark Ultra (Roche). TILs were assessed in matched haematoxylin and eosin stained slides. RESULTS: LC3A positive SLSs, were significantly associated with worse overall (OS) and disease-free survival (DFS) outcomes in patients with lung adenocarcinoma (LADC) (HR = 2.4, 95 %CI(.994-1.008, p = 0.029) and HR = 3.9, 95 %CI (1.002-1.014), p = 0.002 respectively), whilst it was associated with better OS and DFS in patients with lung squamous cell carcinoma (LUSC), with marginal significance (HR = .99, 95 %CI(.975-1.011),p = 0.042 and HR = .99, 95 %CI (.975-1.008), p = 0.026). Multivariate analysis showed that LC3A SLSs are independent poor prognostic factor only in patients with LADC. In addition, LC3A SLSs, were negatively associated with CD68 count in LADC, whilst there was a positive correlation in LSCC. CONCLUSIONS: LC3A SLSs are differentially associated with the survival outcomes and CD68 count in LADC and LSCC. Further studies are justified for the understanding the underlying biological mechanisms of this phenomenon.

The Hodgkin/Reed-Sternberg cells of classical Hodgkin lymphoma (HL) are characterised by the aberrant activation of multiple signalling pathways. Here we show that a subset of HL displays altered expression of sphingosine-1-phosphate (S1P) receptors (S1PR)s. S1P activates phosphatidylinositide 3-kinase (PI3-K) in these cells that is mediated by the increased expression of S1PR1 and the decreased expression of S1PR2. We also showed that genes regulated by the PI3-K signalling pathway in HL cell lines significantly overlap with the transcriptional programme of primary HRS cells. Genes upregulated by the PI3-K pathway included the basic leucine zipper transcription factor, ATF-like 3 (BATF3), which is normally associated with the development of dendritic cells. Immunohistochemistry confirmed that BATF3 was expressed in HRS cells of most HL cases. In contrast, in normal lymphoid tissues, BATF3 expression was confined to a small fraction of CD30-positive immunoblasts. Knockdown of BATF3 in HL cell lines revealed that BATF3 contributed to the transcriptional programme of primary HRS cells, including the upregulation of S1PR1. Our data suggest that disruption of this potentially oncogenic feedforward S1P signalling loop could provide novel therapeutic opportunities for patients with HL.

To test the role of the miR17-92 (miR) cluster in dental bones, we evaluated the incisor tooth phenotype by micro-CT in 5- and 12-week-old conditional knockout (CKO) mice deficient in the miR17-92 cluster in type-I collagen-expressing cells and bone strength by finite element analysis. The incisor teeth of CKO mice showed a 23-30 % reduction in tissue volume and bone volume. Accordingly, the stiffness and failure load of incisor teeth assessed by finite element analysis showed an 18-40 % decrease in CKO compared to wild-type mice. A positive correlation between bone parameters and strength data suggests that the decreased mechanical properties of incisor teeth are due to decreased tissue volume and bone volume. Subsequently, we found that the width of alveolar bone was reduced by 25 % with a 16 % increase in periodontal ligament space, suggesting that the CKO mice are more susceptible to tooth movement. Since alveolar bone is populated primarily by osteoblast lineage cells, it is likely that the reduction in periosteal expansion of alveolar bone in the lower jaw of CKO mice results from decreased periosteal bone formation. Overall, our phenotype analysis demonstrates that the miR17-92 cluster is essential for development and maintenance of tooth strength by regulating its tooth size.
