Celkem 60 pacientů s kritickou ischémií dolní končetiny bez možnosti další revaskularizace bude randomizováno do tříramenné studie. V prvním rameni budou pacienti léčeni standardně prostaglandinem E. Ve druhém rameni bude probíhat eskalační studie zaměřená na optimální dávku G-CSF a simvastatinu k mobilizaci endotelových prekurzorů. Ve třetím rameni bude pacientům odebrána kostní krev a po zpracování aplikována do postižené končetiny. Primárním cílem bude účinnost a bezpečnost experimentálních metod terapie. Sekundárním cílem bude zjištění rozdílů mezi diabetiky a nediabetiky a stanovení optimálního poměru jednotlivých buněčných populací podílejících se na terapeutickém efektu. Sledován bude ústup bolestí, zhojení trofických efektů a doba do amputace končetiny. Součástí projektu bude studie kvality života a ekonomická kalkulace.; Sixty patients with critical peripheral artery disease of the leg without further revascularization possibilities will be randomized into the three arm study. In the first arm, standard treatment with prostaglandin for four weeks will be given. In the second arm, an escalation study to determine the optimal dose of G-CSF and simvastatin for endothelial precursors mobilization will be performed. In the third arm, the processed autologous marrow blood will be applied into the ischemic leg artery. Primaryendpoint will be safety and effectiveness of the experimental treatments. Secondary endpoints will be finding the difference between diabetics and nondiabetics and to determine the role of different cellular progenitors on the treatment effect. Evaluatedwill be the dissapearance of leg pain at rest, healing of ischemic defects and time to amputation. Quality-of-life study and economical calculation will be performed.
