AIM: Patients with inflammatory bowel disease (IBD) are prone to cardiovascular disorders, although there is little research to support this assertion, and other data are controversial in children. We aimed to determine the extent of premature atherosclerosis in Crohn's disease (CD) by measuring reactive hyperaemia index (RHI) as a functional marker of endothelial dysfunction (ED). METHODS: Twenty-one patients with CD and twelve healthy matched subjects were enrolled in the study. Diagnosis was based on the standard clinical, endoscopic and histological criteria, including the Paediatric Crohn's disease Activity Index. ED was assessed using the plethysmographic RHI, combined with specific biochemical markers of ED. RESULT: RHI values were significantly lower in the patients with CD than the controls (p < 0.05). E-selectin (p < 0.05), asymmetric dimethylarginine (p < 0.01) and high-sensitive CRP (p < 0.05), but not vascular cells adhesive molecule-1 values, were significantly increased in the CD subjects compared with the control group. CONCLUSION: Significantly decreased RHI and elevated plasma levels of specific biochemical parameters seems to be related to systemic inflammation and ED in children with CD. Our results support the hypothesis regarding RHI and ED in paediatric CD. This combined method assessment might be a useful tool for detection of ED and stratification of cardiovascular risk in patients with CD.

BACKGROUND AND AIM: To study relationships between endothelial dysfunction (ED) and coronary atherosclerosis derived from intravascular ultrasound (IVUS) and virtual histology (VH). METHODS: Endothelial dysfunction was examined by EndoPAT system (Itamar Medical) in 56 patients who underwent IVUS and VH (Volcano corp.). Reactive hyperaemia index (RHI) < 2 was used for definition of ED. IVUS sequences were divided into 5 mm-long non-overlapping and adjacent vessel segments. Plaque phenotype was determined for each frame and 5 mm vessel segment was labeled according to highest frame score (from 0 for "no lesion" to 5 for "thin cap fibroatheroma; TCFA"). RESULTS: IVUS-VH data were collected from 41 patients suitable for three-dimensional analysis. Patients with ED exhibited larger plaque burden than those without ED (0.46 ± 0.08 vs. 0.39 ± 0.07, p = 0.014), smaller lumen area (8.59 ± 2.19 vs. 11.90 ± 3.50, p = 0.016), higher plaque risk score (2.82 ± 1.18 vs. 1.84 ± 0.90, p = 0.012), and higher number of TCFA frames (0.36 ± 0.22 vs. 0.22 ± 0.16, p = 0.038). Relative amounts of fibrous tissue correlated positively with RHI (p = 0.034, r = 0.33). The numbers of fibroatheromas and calcified plaques correlated with RHI inversely (r = -0.34, p = 0.031 and r = -0.32, p = 0.044, respectively). CONCLUSIONS: Endothelial dysfunction correlates with severity and phenotype of coronary lesions and can contribute to non-invasive detection of individuals with higher risk of cardiovascular events.

Idiopatické střevní záněty jsou spojené s vyšším rizikem kardiovaskulárních onemocnění v dospělosti, literární údaje, zahrnující dětskou populaci, jsou však dosud sporadické. Cílem naší prospektivní studie byla detekce endoteliální dysfunkce (ED) pomocí reaktivního hyperemického indexu (RHI) a biochemických parametrů u rizikové skupiny dětí s Crohnovou chorobou (CD). Do výzkumné studie bylo zařazeno celkem 21 pacientů s CD a 12 zdravých jedinců kontrolní skupiny. Diagnóza CD byla založena na klinických, endoskopických a histologických kritériích, zahrnujících pediatrický index aktivity Crohnovy choroby (PCDAI). ED byla hodnocena pomocí pletyzmografického měření reaktivního hyperemického indexu (RHI) a vybraných biochemických parametrů. Nalezli jsme signifikantně nižší hodnotu RHI u pacientů s CD oproti kontrolnímu souboru (p <0,05). Současně byly v souboru pacientů s CD nalezeny signifikantně zvýšené hodnoty E-selektinu (p <0,05), asymetrického dimetylargininu (p <0,01) a vysoce senzitivního CRP (p <0,05). Závěry naší studie ukazují, že CD může být spojena s ED a tedy vyšším rizikem rozvoje předčasné aterosklerózy. Zároveň ukazují na vhodnost využití kombinace měření RHI a biochemických markerů pro detekci ED a následné hodnocení kardiovaskulárního rizika pacientů s CD v dlouhodobém časovém horizontu.

ED was assessed using the combination of RHI and specific biochemical markers. We found significantly lower RHI values in CD patients in comparison to the healthy controls (p<0,05). E-selectin (p<0.05), asymmetric dimethylarginine (p<0.01) and high-sensitive CRP (p<0.05) were significantly increased in CD patients compared to the control group. Our results demonstrate that Crohn´s disease might be associated with ED and therefore with a higher risk of development of premature atherosclerosis. We also demonstrated the suitability of using the combination of RHI and specific biochemical markers in assessing ED and stratifying of cardiovascular risk and long term follow-up in children with CD.

OBJECTIVES: Our aim was to assess endothelial function in physically healthy patients with schizophrenia and related psychoses and to compare the results with healthy controls. Endothelial dysfunction was shown to predict future cardiovascular events in general population so we assumed to find a higher prevalence of endothelial dysfunction in patients with psychosis, as their cardiovascular morbidity is markedly higher than in general population, and to confirm the referred correlation with the traditional cardiovascular (CV) risk factors. DESIGN: We assessed the traditional CV risk factors and endothelial function using non-invasive peripheral arterial tonometry (EndoPAT2000) in 50 stabilized and medicated schizophrenic patients (aged between 18 and 50 years) without any history of cardiovascular diseases and compared the results with 50 age-matched healthy controls. SETTING: Psychiatric Clinic, University Hospital, Hradec Kralove and 2nd Department of Internal Medicine, General University Hospital, Prague, Czech Republic RESULTS: There was no difference in relative hyperaemia index as an endothelial function measure between patients and controls (2.19±0.68 vs. 1.98±0.57, p=NS) and there were also no correlations between reactive hyperaemia index and the traditional CV risk factors, illness duration or psychotic symptoms. On the other hand, the two study groups differed significantly in almost all traditional CV risk factors.
