Alpers' syndrome is an early-onset neurodegenerative disorder usually caused by biallelic pathogenic variants in the gene encoding the catalytic subunit of polymerase-gamma (POLG), which is essential for mitochondrial DNA (mtDNA) replication. The disease is progressive, incurable, and inevitably it leads to death from drug-resistant status epilepticus. The neurological features of Alpers' syndrome are intractable epilepsy and developmental regression, with no effective treatment; the underlying mechanisms are still elusive, partially due to lack of good experimental models. Here, we generated the patient derived induced pluripotent stem cells (iPSCs) from one Alpers' patient carrying the compound heterozygous mutations of A467T (c.1399G>A) and P589L (c.1766C>T), and further differentiated them into cortical organoids and neural stem cells (NSCs) for mechanistic studies of neural dysfunction in Alpers' syndrome. Patient cortical organoids exhibited a phenotype that faithfully replicated the molecular changes found in patient postmortem brain tissue, as evidenced by cortical neuronal loss and depletion of mtDNA and complex I (CI). Patient NSCs showed mitochondrial dysfunction leading to ROS overproduction and downregulation of the NADH pathway. More importantly, the NAD+ precursor nicotinamide riboside (NR) significantly ameliorated mitochondrial defects in patient brain organoids. Our findings demonstrate that the iPSC model and brain organoids are good in vitro models of Alpers' disease; this first-in-its-kind stem cell platform for Alpers' syndrome enables therapeutic exploration and has identified NR as a viable drug candidate for Alpers' disease and, potentially, other mitochondrial diseases with similar causes.

PURPOSE: Zellweger spectrum disorders (ZSDs) are known as autosomal recessive disorders caused by defective peroxisome biogenesis due to bi-allelic pathogenic variants in any of at least 13 different PEX genes. Here, we report 2 unrelated patients who present with an autosomal dominant ZSD. METHODS: We performed biochemical and genetic studies in blood and skin fibroblasts of the patients and demonstrated the pathogenicity of the identified PEX14 variants by functional cell studies. RESULTS: We identified 2 different single heterozygous de novo variants in the PEX14 genes of 2 patients diagnosed with ZSD. Both variants cause messenger RNA mis-splicing, leading to stable expression of similar C-terminally truncated PEX14 proteins. Functional studies indicated that the truncated PEX14 proteins lost their function in peroxisomal matrix protein import and cause increased degradation of peroxisomes, ie, pexophagy, thus exerting a dominant-negative effect on peroxisome functioning. Inhibition of pexophagy by different autophagy inhibitors or genetic knockdown of the peroxisomal autophagy receptor NBR1 resulted in restoration of peroxisomal functions in the patients' fibroblasts. CONCLUSION: Our finding of an autosomal dominant ZSD expands the genetic repertoire of ZSDs. Our study underscores that single heterozygous variants should not be ignored as possible genetic cause of diseases with an established autosomal recessive mode of inheritance.

Tyrosinemie typ 1 (TYR 1) je vzácné autosomálně recesivní onemocnění z okruhu dědičných poruch metabolismu aminokyselin. Podkladem jsou bialelické mutace v genu FAH, vedoucí k poruše aktivity fumarylacetoacetáthydrolázy, klíčového enzymu v metabolismu tyrosinu. U dětí s TYR 1 dochází alternativní metabolickou cestou k nadprodukci toxických metabolitů, především sukcinylacetonu a fumarylacetoacetátu, které poškozují hepatocyty a tubulární buňky ledvin. Klinicky se TYR 1 projevuje akutní formou jaterního a renálního selhávání v kojeneckém věku nebo chronicky rozvojem jaterní fibrózy, cirhózy, křivice a hepatocelulárního karcinomu v kojeneckém nebo batolecím věku, časté jsou i porfyrické krize. Biochemická diagnostika je založena na průkazu zvýšených hladin sukcinylacetonu v krvi i moči, eventuálně doprovázených elevací tyrosinu a stoupající hladinou alfa-fetoproteinu. Diagnostiku je třeba potvrdit na molekulární úrovni. V léčbě se používá nitisinon, který blokuje produkci toxických metabolitů, v kombinaci s nízkobílkovinnou dietou suplementovanou směsí esenciálních aminokyselin bez tyrosinu a fenylalaninu. Pokud konzervativní terapie nezabrání progresi jaterního selhávání nebo rozvoji hepatocelulárního karcinomu, je indikována transplantace jater. V našem sdělení předkládáme klinický průběh onemocnění u 10měsíčního chlapce, u kterého se onemocnění projevilo hepatopatií, melénou a hepatocelulárním karcinomem. Kromě nitisinonu a diety byla nutná chemoembolizace přívodné jaterní arterie v oblasti nejrychleji rostoucího karcinomu, která umožnila získat čas na přípravu k transplantaci jater. Po úspěšné transplantaci byla metabolická léčba ukončena, chlapec prospívá a je v dobrém klinickém stavu.
Tyrosinemia type 1 is a rare metabolic disease with autosomal recessive inheritance caused by biallelic mutations in the FAH gene, leading to impaired activity of fumarylacetoacetate hydrolase, a key enzyme in tyrosine metabolism. In children with TYR 1, an activation of alternative metabolic pathway leads to an overproduction of toxic metabolites, mainly succinylacetone and fumarylacetoacetate, which damage hepatocytes and renal tubular cells. Clinically, TYR 1 presents with acute progression to liver and renal failure at an early age or chronically with the development of liver fibrosis, cirrhosis, rickets and hepatocellular carcinoma, manifestations of porhyria crisis are also common. Biochemical diagnosis is based on evidence of increased levels of succinylacetone in the blood and urine, accompanied by an elevation of tyrosine and rising level of alpha-fetoprotein. The diagnosis must be confirmed at the molecular level. The treatment is based on an administration of nitisinone, which blocks the production of toxic metabolites in combination with a low-protein diet supplemented with a mixture of essential amino acids without tyrosine and phenylalanine. If conservative therapy does not prevent the progression of liver failure or the development of hepatocellular carcinoma, liver transplantation is indicated. We present the clinical course of the disease in a 10-month-old boy with TYR1, manifesting with hepatopathy, melena, and hepatocellular carcinoma. In addition to nitisinone and diet, chemoembolization of the supplying hepatic artery in the area of the fastest-growing carcinoma was necessary, which enabled us to gain time to prepare liver transplantation. After a successful transplant, the boy is in a good clinical condition.

Úvod: Dědičné poruchy metabolismu (DPM) sacharidů představují heterogenní skupinu více než 250 různých onemocnění způsobených porušenou syntézou, transportem či odbouráváním galaktózy, fruktózy, glukózy, disacharidů, glykogenu, glykosaminoglykanů a glykoproteinů/glykolipidů. Jednotlivé DPM sacharidů jsou sice vzácné, ale celkový výskyt v populaci je > 1 : 5000. Diagnostika není součástí laboratorního novorozeneckého screeningu a závisí na klinickém podezření, biochemickém a hematologickém vyšetření a indikaci selektivního metabolického screeningu. Materiál a metody: Práce shrnuje klinické, diagnostické a terapeutické aspekty nejčastějších DPM sacharidů u > 360 pacientů diagnostikovaných na našem pracovišti. Výsledky: Klinické projevy u dětí s DPM sacharidů jsou heterogenní a u řady poruch se mohou překrývat. První příznaky u DPM galaktózy a fruktózy začínají akutními projevy jaterního selhávání s postižením tubulárních funkcí ledvin a Fanconiho syndromem. Většina jaterních glykogenóz (GSD) začíná hepatomegalií, poruchou růstu, atakami hypoglykemií po 2,5–3hodinovém lačnění, hepatopatií, dyslipidemií a laktátovou acidózou, ale i neutropenií (GSD Ib) nebo jaterním selháním (GSD IV). Svalové glykogenózy se manifestují hypotonií a kardiomyopatií (GSD II) a svalovou slabostí a myalgiemi s atakami rhabdomyolýz (GSD V). Fenotyp jaterních a svalových GSD se překrývá s poruchou fosfoglukomutázy 1. Porucha transportu glukózy a galaktózy (GLUT2) spojuje fenotyp GSD s nefropatií při Fanconiho syndromu. Poruchy metabolismu sacharidů v komplexních molekulách způsobují mukopolysacharidózy (MPS) a dědičné poruchy glykosylace (CDG). Klinicky charakteristické pro skupinu MPS a CDG jsou kraniofaciální dysmorfie, encefalopatie, hepato/splenomegalie, porucha růstu, kostní deformity, postižení myokardu a srdečních chlopní, hernie, recidivující otitidy a chronická rýma. Závěr: Včasná diagnóza je nezbytná pro úspěšnou léčbu. Zahrnuje dietní opatření, mezi která patří bezlaktózová a nízkogalaktózová dieta (galaktosemie), nízkofruktózová dieta (intolerance fruktózy), antihypoglykemický režim s přídavkem nevařených škrobů (jaterní formy GSD), zvýšený příjem bílkovin (GSD III) nebo ketogenní dieta (GLUT1). Některé poruchy glykosylace (CDG) lze léčit pomocí manózy nebo galaktózy. V léčbě dětí s MPS se používá enzymová substituční terapie (ERT) a transplantace hematopoietickými kmenovými buňkami.
Introduction: Inherited metabolic disorders (IMD) of carbohydrates represent a heterogeneous group of >250 different diseases caused by impaired synthesis, transport or degradation of galactose, fructose, glucose, disaccharides, glycogen, glycosaminoglycans and glycoproteins/glycolipids. Individual IMD of carbohydrates are rare, but the overall incidence in the population is >1:5 000. Their diagnosis, except galactosemia in some countries is not part of laboratory neonatal screening of IMD and depends on clinical suspicion, biochemical and haematological analyses, and indication of selective metabolic screening. Material and methods: We summarize our experiences with the clinical, diagnostic, and therapeutic aspects of the most common IMD of carbohydrates in >360 patients diagnosed at our institution. Results: Clinical manifestations in children with IMD of carbohydrates are heterogeneous and may overlap with several diseases. The first symptoms of IMD of galactose and fructose begin with acute manifestations of liver failure with impaired renal tubular functions and Fanconi syndrome. Most liver glycogenoses (GSD) begin with hepatomegaly, growth failure, attacks of hypoglycaemia after 2.5-3 hours of fasting, hepatopathy, dyslipidaemia and lactic acidosis, but also neutropenia (GSD Ib) or liver failure (GSD IV). Muscle glycogenoses are presented by hypotonia and cardiomyopathy (GSD II) and muscle weakness and myalgia with attacks of rhabdomyolysis (GSD V). Hepatic and muscle GSD phenotype overlaps with phosphoglucomutase 1 deficiency. Glucose-galactose transport (GLUT2) disorder links GSD phenotype to nephropathy with Fanconi syndrome. IMD of carbohydrates in complex molecules cause mucopolysaccharidoses (MPS) and congenital disorders of glycosylation (CDG). Clinically characteristic of the MPS and CDG group are craniofacial dysmorphy, encephalopathy, hepato/splenomegaly, growth disorder, bone deformities, involvement of the myocardium and heart valves, hernia, recurrent otitis, and chronic rhinitis. Conclusion: Early diagnosis is essential for successful treatment. Dietary intervention includes a lactose-free and low-galactose diet (galactosemia), a low-fructose diet (fructose intolerance), an anti-hypoglycaemic regimen with the addition of uncooked starches (liver GSDs), increased protein intake (GSD III), or a ketogenic diet (GLUT1). Some congenital disorders of glycosylation (CDG) can be treated with mannose or galactose. Enzyme replacement therapy (ERT) and hematopoietic stem cell transplantation are used in the treatment of children with MPS.

Aromatic L-amino acid decarboxylase (AADC) deficiency is a rare autosomal recessive genetic disorder affecting the biosynthesis of dopamine, a precursor of both norepinephrine and epinephrine, and serotonin. Diagnosis is based on the analysis of CSF or plasma metabolites, AADC activity in plasma and genetic testing for variants in the DDC gene. The exact prevalence of AADC deficiency, the number of patients, and the variant and genotype prevalence are not known. Here, we present the DDC variant (n = 143) and genotype (n = 151) prevalence of 348 patients with AADC deficiency, 121 of whom were previously not reported. In addition, we report 26 new DDC variants, classify them according to the ACMG/AMP/ACGS recommendations for pathogenicity and score them based on the predicted structural effect. The splice variant c.714+4A>T, with a founder effect in Taiwan and China, was the most common variant (allele frequency = 32.4%), and c.[714+4A>T];[714+4A>T] was the most common genotype (genotype frequency = 21.3%). Approximately 90% of genotypes had variants classified as pathogenic or likely pathogenic, while 7% had one VUS allele and 3% had two VUS alleles. Only one benign variant was reported. Homozygous and compound heterozygous genotypes were interpreted in terms of AADC protein and categorized as: i) devoid of full-length AADC, ii) bearing one type of AADC homodimeric variant or iii) producing an AADC protein population composed of two homodimeric and one heterodimeric variant. Based on structural features, a score was attributed for all homodimers, and a tentative prediction was advanced for the heterodimer. Almost all AADC protein variants were pathogenic or likely pathogenic.
Cíl studie: Zhodnocení výsledků transplantace kmenových buněk krvetvorby (HSCT, hematopoietic stem cell transplantation) u dětí s dědičnými metabolickými poruchami (DMP) a maligní infantilní osteopetrózou (MIOP) v České republice od začátku transplantačního programu v roce 1989 do současnosti. Metody: V období 1/1993 až 12/2021 bylo ve FN v Motole provedeno celkem 31 alogenních HSCT u 22 pacientů s DMP a 5 pacientů s MIOP. Dárcem kmenových buněk byl HLA identický sourozenec (MSD, matched sibling donor), nepříbuzný dárce (MUD, matched unrelated donor) nebo haploidentický rodinný dárce (MMFD, mismatched family donor). Zdrojem kmenových buněk byla kostní dřeň (KD), periferní kmenové buňky (PBSC, peripheral blood stem cells) nebo pupečníková krev (UCB, umbilical cord blood). Předtransplantační přípravný režim byl použit v souladu s doporučením IEWP-EBMT (Evropská pracovní skupina pro transplantace vrozených onemocnění). Výsledky: 1. alogenní HSCT podstoupilo 22 pacientů s DMP a 5 pacientů s MIOP. Medián věku v době HSCT byl 19,5 měsíce. Primární přihojení štěpu po 1. transplantaci bylo dokumentováno u 24 z 27 pacientů (89 %). U 3 pacientů byla z důvodu nepřihojení či odhojení štěpu nutná retransplantace. Akutní reakce štěpu proti hostiteli (GVHD) byla dokumentována u 11 pacientů (41 %), těžkou formu gr. III vyvinul pouze 1 pacient, chronickou GVHD 1 pacient. Zemřelo celkem 6 pacientů, 3 na komplikace spojené s transplantací, 2 na progresi základního onemocnění, 1 pacient po úspěšné transplantaci na komplikace neurochirurgického výkonu. Celkové přežití je 78 %, 21 z 27 pacientů žije s mediánem doby sledování 169 měsíců po transplantaci (12–347 měsíců), 20 pacientů se zlepšením/stabilizací projevů onemocnění. Závěry: HSCT je v současné době standardní léčebnou metodou u vybraných DMP a MIOP. Naděje na úspěch transplantace vysoce převyšuje její rizika, zásadní je časně stanovená diagnóza a indikace k HSCT. Léčba pacientů vyžaduje komplexní přístup a spolupráci lékařů transplantačního a metabolického centra a dalších specialistů.
Objective: Evaluation of the results of hematopoietic stem cell transplantation (HSCT) in children with inborn errors of metabolism (IEM) and malignant infantile osteopetrosis (MIOP) in the Czech Republic from the beginning of the transplant programme in 1989 to the present. Methods: In the period 1/1993 to 12/2021 a total of 31 allogeneic HSCTs were performed at the University Hospital Motol in 22 patients with IEM and 5 patients with MIOP. Patients were transplanted from HLA identical sibling (MSD), matched unrelated donor (MUD) or mismatched family donor (MMFD). The source of stem cells was bone marrow (BM), peripheral blood stem cells (PBSC) or umbilical cord blood (UCB). The conditioning regimen was used in accordance with the IEWP-EBMT (Inborn Errors Working party – European Bone Marrow Transplant). Results: 22 patients with IEM and 5 patients with MIOP underwent the first allogeneic HSCT at the median age of 19,5 months. Engraftment after the first transplant was documented in 24 of 27 patients (89 %). Three patients underwent retransplantation for primary or secondary graft failure. Acute graft-versus-host disease (GVHD) was documented in 11 patients (41 %), severe form of aGVHD gr. III in 1 patient and chronic GVHD in 1 patient, respectively. A total of 6 patients died, 3 patients due to transplant-related mortality, 2 patients from disease progression, respectively and 1 patient died after a successful transplant due to complications of neurosurgery. The overall survival is 78 %, 21 of 27 patients are alive with a median follow-up of 169 months after transplantation (12-347 months), 20 of them with improvement or stabilization of the disease. Conclusions: HSCT is currently the standard therapy for selected IEM and MIOP. This procedure has become much safer during recent decades, timely diagnosis and indication for HSCT are essential. The treatment requires multidisciplinary management and continuous collaboration with other specialists.
Huntington ́s disease (HD) is a progressive neurodegenerative disease with onset in adulthood that leads to a complete disability and death in approximately 20 years after onset of symptoms. HD is caused by an expansion of a CAG triplet in the gene for huntingtin. Although the disease causes most damage to striatal neurons, other parts of the nervous system and many peripheral tissues are also markedly affected. Besides huntingtin malfunction, mitochondrial impairment has been previously described as an important player in HD. This study focuses on mitochondrial structure and function in cultivated skin fibroblasts from 10 HD patients to demonstrate mitochondrial impairment in extra-neuronal tissue. Mitochondrial structure, mitochondrial fission, and cristae organization were significantly disrupted and signs of elevated apoptosis were found. In accordance with structural changes, we also found indicators of functional alteration of mitochondria. Mitochondrial disturbances presented in fibroblasts from HD patients confirm that the energy metabolism damage in HD is not localized only to the central nervous system, but also may play role in the pathogenesis of HD in peripheral tissues. Skin fibroblasts can thus serve as a suitable cellular model to make insight into HD pathobiochemical processes and for the identification of possible targets for new therapies.
In this study, we report on a novel heteroplasmic pathogenic variant in mitochondrial DNA (mtDNA). The studied patient had myoclonus, epilepsy, muscle weakness, and hearing impairment and harbored a heteroplasmic m.8315A>C variant in the MTTK gene with a mutation load ranging from 71% to >96% in tested tissues. In muscle mitochondria, markedly decreased activities of respiratory chain complex I + III and complex IV were observed together with mildly reduced amounts of complex I and complex V (with the detection of V*- and free F1-subcomplexes) and a diminished level of complex IV holoenzyme. This pattern was previously seen in other MTTK pathogenic variants. The novel variant was not present in internal and publicly available control databases. Our report further expands the spectrum of MTTK variants associated with mitochondrial encephalopathies in adults.
