Cílem prospektivní randomizované srovnávací studie, sponzorované IGA MZ, je zkoumat možnost ovlivnění produkce aktivních forem O2 při operačním traumatu podáním Indomethacinu jako inhibitoru cyklooxygenázy. Předběžně byl vyhodnocen soubor 30 nemocných s koxartrózou nebo gonarf rózou, přijatých do nemocnice k náhradě kyčelniho nebo kolenního kloubu. Po randomizaci byli nemocni rozděleni do dvou skupin. První skupině byl předoperačně a 24 hodin po operaci podán Indomethacin (Indo, n = 18), druhá skupina byla kontrolní (Kont, n = 12). U obou skupin byly sledovány parametry umožňující hodnotit antioxidačni rezervy organismu, produkci volných radikálů (VR), kvalitu pooperačni analgezie, krvácivost a diurézu. Data byla vyhodnocena neparametrickým Mann-Whitney testem. Autorům se nepodařilo prokázat statisticky významné ovlivněni produkce volných radikálů podáním Indomethacinu. Indomethacin zlepšuje kvalitu pooperačni aruilgezie a snižuje spotřebu ostatních analgetik. Prodlužuje krvácivost v bezprostředním pooperačním obdobi ale nezvyšuje celkovou krevní ztrátu, snižuje glomerulámi filtraci a diurézu, ale nevede pravděpodobně ke zhoršení normálních renálních funkcí. Vzhledem k malému počtu sledováni je platnost uvedených závěrů nespolehlivá.
The aim of the prospective randomized comparative study was to assess the possibility to affect the production of free forms of oxygen during operative trauma with indomethacine as cyclooxygenase inhibitor. In the preliminary report we present the first thirty patients with coxarthrosis or gonarthrosis admitted for hip or knee replacement. The patients were randomized in two groups. Patients in the first group were preoperatively and 24 hours after surgery given indomethacine (Indo, n = 18), patients in the other group were controls (Kont, n = 12). In both groups we followed the parameters enabling to evaluate antioxidatory reserve of the organism, free radicals production, quality of postoperative analgesia, coagulation parameters and diuresis. Data were analyzed using non-parametric Mann-Whitney test. The study failed to demonstrate statistically significant effect of indomethacine on free radicals production. Indomethacine improves the quality of postoperative analgesia arui decreases the consumption of analgesics. Indomethacine prolongs bleeding time in the immediate postoperative period hut does not increase overall blood loss; decreases glomerular filtration rate and diuresis but probably does not induce deterioration of normal renal functions. According to the small sample size of patient groups, fhe validity of presented results is limited.
