The objective of this study was to compare semen quality (sperm density, progressive motility and spermia) between long-term childhood cancer survivors and a control group of males. The second objective was to correlate the semen analysis of the survivors with cancer treatment and endocrine status. The semen quality of 143 survivors (median age, 23.6 years) was compared to 200 men (median age, 27.9 years) who had not been diagnosed with cancer. The cancer-related risk factors and gonadotrophin levels were compared. Overall, 65% of the survivors had abnormal semen analysis compared to 26.5% of the controls (p < 0.0001). Survivors with nonaspermia had lower sperm density than the controls (p < 0.001). Other observed correlations were not significant. Survivors who were treated with alkylating agents were more likely to have abnormal semen analysis (p < 0.008). Follicle-stimulating hormone and luteinising hormone levels were significantly elevated (p < 0.0001) in survivors with abnormal semen analysis. The semen quality parameters, except for low sperm density, did not differ in survivors with nonaspermia compared to the controls. The risk factors included treatment with alkylating agents. Elevated gonadotrophin levels correlated with abnormal semen analysis. All cancer survivors should be made aware of the possibility of suffering from cancer treatment-related infertility.

V příspěvku je prezentován případ sedmiletého chlapce vyšetřovaného v akutní pediatrické ambulanci pro protrahované febrilie a asymetrickou krční lymfadenopatii. Pro alteraci celkového stavu, vysoké zánětlivé parametry, bilaterální konjunktivitidu a meningismus byl odeslán k hospitalizaci s podezřením na Kawasakiho nemoc (KD). Za hospitalizace bylo zjištěno selhávání levé komory srdeční, pro progresi stavu vyžadoval pacient oběhovou podporu a UPV. Stav progredoval v horizontu několika dní. Imunologem byla stanovena diagnóza PIMS-TS (Pediatric inflammatory multisystem syndrome temporally associated with SARS-CoV-2). Anamnesticky pacient neprodělal onemocnění Covid-19, údaj o pozitivním nebo negativním testování rodiny nemáme. U pacienta však byly prokázány protilátky proti novému koronaviru.
A case report of a 7 years old boy with long term fever and asymetric cervical lymphadenopathy examined in acute paediatric outpatient clinic is presented in this paper. He was admitted to hospital for altered general status, increase of laboratory inflammatory parameters, bilateral conjunctitvitis and postitive meningeal syndrom as suspected Kawasaki disease (KD). Left ventricle insufficiency presented during hospital stay and circulatory support and artificial lung ventilation was needed for the deterioration of patient´s general condition. Progression of patient´s status occurred within a few days. The diagnose of PIMS-TS (Pediatric inflammatory multisystem syndrome temporally associated with SARS-CoV-2) was stated by immunological consultant. There were no information of Covid-19 in patient´s case history and we have also no information about Covid-19 tests in patient´s family, nor negative or positive, but novel coronavirus antibodies were proven in our patient.
