Úvod: Požití korozivní látky představuje potencionálně závažný stav s možným ohrožením života a rozvojem časných i pozdních komplikací. Příznaky a diagnostika: Příznaky u poleptání mohou být různorodé, nespecifické a nevedou ke stanovení diagnózy, nemáme-li údaj o požití nebezpečné látky. Nejprve je nutné zjistit informace o požité látce. Dalším krokem by mělo být vyšetření otorynolaryngologem. Zcela zásadní pro další léčebný postup je provedení flexibilní endoskopie horní části zažívacího traktu. Cílem endoskopického vyšetření je vyhodnotit rozsah změn v jícnu a žaludku. Podle endoskopického nálezu je pak vedena léčba postiženého pacienta. V případě podezření na perforaci musí endoskopii předcházet provedení CT hrudníku a břicha s aplikací kontrastní látky per os. Léčba: V případě přítomnosti závažných respiračních příznaků, edému hrtanu či jeho nekrózy je nutné zajištění dýchacích cest. V léčbě poleptání se uplatňují inhibitory protonové pumpy, antibiotika by měla být použita při perforaci jícnu, ve třetím stadiu poleptání, při aplikaci steroidů a postižení plic. Kortikoidy nejsou jako prevence striktur indikovány, jediná indikace k podání kortikoidů jsou těžké edematózní změny na dýchacích cestách. Způsob výživy nemocného se odvíjí podle endoskopického nálezu. Pacientům s poleptáním I. a II.A stupně podáváme iniciálně tekutou dietu na 48 hod. U pacientů s postižením II.B a III. stupně není možné aplikovat výživu dietou. Je nutné zavést nazoenterální sondu a podávat enterální výživu sondou. Při vzniku perforace GIT je indikováno akutní operační řešení. Mezi pozdní komplikace se řadí především vznik striktur. Závěr: Poleptání jícnu a žaludku se řadí mezi méně častá postižení. Svojí závažností však může pacienta ohrozit na životě či vyvolanými změnami ovlivnit život na dlouhá léta.
Introduction: Ingestion of a corrosive substance is an infrequent but serious condition with a possible threat to life. Symptoms and diagnosis: Clinical symptoms of esophageal burns are non-specific and are determined by the degree and extent of injury Subjective symptoms do not lead to a diagnosis without information of corrosive substance ingestion. It is necessary to define the substance and find out information of its local and systemic toxicity. The next step should be an ENT examination to descibe local extent of corrosive injury in the oral cavity, pharynx and larynx and exclude severe injury and oedema of larynx and urgency of tracheostomy or intubation. A flexible endoscopy of the upper digestive tract is essential for further treatment. The aim of the endoscopic examination is to evaluate the extent of changes in the oesophagus and stomach. If GIT perforation is suspected, CT scan of the thorax and abdomen must precede the endoscopy. Treatment: In the presence of obstructive respiratory symptoms, laryngeal edema or severe burns of the airways is necessary to secure the airways as the first step. Wide-spread antibiotics should be used in the case of perforation of the oesophagus, in the third stage corrosions and in lung involvement. Corticoids are not indicated as a prevention of strictures, the only indication is to reduce oedema of the airways. Patient’s nutrition depends on degree and extent of the injury. Patients with I and II.A degree burns are nourished orally, a liquid diet for 48 hours is recommended. In patients with II.B and III. degree oral intake is excluded and nasoenteral tube is inserted for tube feeding. In case of GIT perforation, an acute surgical procedure is indicated. Stenosis of the oesophagus are frequent and severe long-term consequences of severe corrosions and long-term care and repeated interventions to restore of possibility of oral feeding could be indicated. Conclusion: Oesophageal and stomach burns are less frequent injuries. However, due to its severity, it can endanger patient’s life or its quality for long years. Multidisciplinary approach is essential in corrosive injuries management.

Chemické poleptania sú zrak ohrozujúce poranenia, vyžadujúce okamžitú liečbu. Hlavným cieľom počiatočnej terapie je intenzívny a výdatný výplach spojovkového vaku, za účelom neutralizácie a eliminácie pôsobiacej chemikálie. Ďalšie terapeutické postupy, či už konzervatívne, alebo chirurgické, závisia od klinického nálezu. Cieľ: V kazuistike popisujeme priebeh a liečbu pacientky s poleptaním oboch očí zásaditou látkou. Kazuistika: V práci uvádzame kazuistiku 35-ročnej ženy, s chemickým poleptaním oboch očí alkalickým činidlom. Do troch týždňov nastala úplná reepitelizácia oboch rohoviek. Vzhľadom k tomu sme predpokladali uspokojivú prognózu hojenia, avšak na ľavom oku došlo s odstupom času k lýze rohovky. Stav sa podarilo zvládnuť, ale následne došlo na ľavom oku zrejme v súvislosti s infekciou COVID-19 k závažnej keratouveitíde s hypopyonom a lýzou rohovky, čo v konečnom dôsledku viedlo k eviscerácii postihnutého oka. Výsledky a záver: V kazuistike popisujeme management ľahkého poleptania na pravom oku a komplikovaného stredne ťažkého poleptania ľavého oka zásaditou chemikáliou. Obe oči boli do troch týždňov kompletne prehojené, no nález na ľavom oku sa skomplikoval, čo dospelo v konečnom dôsledku až k eviscerácii postihnutého oka. Ochorenie COVID-19 mohlo prispieť k zhoršeniu nálezu na rohovke alebo mohlo ísť iba o koincidenciu dvoch ochorení a keratouveitída mala závažný priebeh z dôvodu predchádzajúceho ťažkého priebehu hojenia a mnohých komplikácií po chemickom poleptaní oka.
Chemical burns are sight-threatening injuries that require immediate management. The main goal of the initial treatment is prompt and copious irrigation to neutralize and eliminate the chemical, followed by various therapeutic options (conservative and surgical) according to the individual patient’s postinjury findings. Purpose: In this case report, we describe the course and treatment of a patient with chemical alkali burns of both eyes. This study reports the outcomes of an ocular alkali burn patient who became infected with COVID-19 and where severe keratouveitis with corneal graft melting and hypopyon occurred, ultimately resulting in evisceration of the eye. Observations: We report the case of a 35-year-old woman, after an alkali burn of both eyes. Complete re-epithelialization of both corneas occurred within three weeks. Due to this observation, we assumed a satisfactory healing prognosis. However, corneal lysis occurred in the left eye over time. The condition was managed, but subsequently severe keratouveitis with hypopyon and corneal lysis occurred in the left eye, apparently in connection with the COVID-19 infection, which ultimately led to the evisceration of the affected eye. Conclusions and Importance: In the case report, we describe the management of a mild chemical alkali burn of the right eye and a complicated moderate burn of the left eye with an alkaline chemical. Both eyes were completely healed within three weeks. However, the findings on the left eye became complicated, which ultimately led to the evisceration of the affected eye. The disease of COVID-19 could have contributed to the deterioration of the findings on the cornea, or it could have just been a coincidence of two diseases, with the keratouveitis having a serious course due to the previous difficult course of healing and many complications after the chemical burn.

Poleptání kůže, ať kyselinou nebo zásadou, patří ke stále častějším pracovním úrazům. Látek, které způsobují poruchu integrity kožního krytu, je nepřeberné množství. Epoxidová pryskyřice, jejíž používání v řadě průmyslových odvětví je dlouhodobě na vzestupu, se i dnes řadí svým výskytem mezi raritní původce kožních postižení. Nízký epidemiologický výskyt je však v kontrastu s možnou mírou postižení. I zde se ukazuje, že jedno z největších úskalí postižení kůže chemikáliemi je jejich dlouhodobá absorpce měkkými tkáněmi. Pokud se úraz a následná léčba navíc zkomplikují nespecifickou hypersenzitivní reakcí při vrozené predispozicí jedince, může se zprvu banální ambulantní rána změnit v dlouhodobě se nehojící defekt vedoucí až k ústavní péči spojené se systémovou léčbou.
Chemical burns represent a frequent cause of an occupational injury. Numerous chemical substances, such as epoxy resin as we reported on in this article, could potentially cause skin damage or, moreover, systemic toxicity. There is an increasing trend in use of epoxy resin in different kinds of industry. Nevertheless, a minimum of occupational diseases are effected by epoxy resin, however, this could be in contrast with the possible severity of the disease. Chemical substances penetrate the human skin and are absorbed by soft tissues for the long time resulting into a major damage of the skin. In this publication we present a case of a chemical burn caused by the epoxy resin and complicated by a severe nonspecific hypersensitive reaction requiring a hospital care and systemic treatment.

Corneal alkali burns are potentially blinding injuries. Alkali induces oxidative stress in corneas followed by excessive corneal inflammation, neovascularization, and untransparent scar formation. Molecular hydrogen (H2), a potent reactive oxygen species (ROS) scavenger, suppresses oxidative stress and enables corneal healing when applied on the corneal surface. The purpose of this study was to examine whether the H2 pretreatment of healthy corneas evokes a protective effect against corneal alkali-induced oxidative stress. Rabbit eyes were pretreated with a H2 solution or buffer solution, by drops onto the ocular surface, and the corneas were then burned with 0.25 M NaOH. The results obtained with immunohistochemistry and pachymetry showed that in the corneas of H2-pretreated eyes, slight oxidative stress appeared followed by an increased expression of antioxidant enzymes. When these corneas were postburned with alkali, the alkali-induced oxidative stress was suppressed. This was in contrast to postburned buffer-pretreated corneas, where the oxidative stress was strong. These corneas healed with scar formation and neovascularization, whereas corneas of H2-pretreated eyes healed with restoration of transparency in the majority of cases. Corneal neovascularization was strongly suppressed. Our results suggest that the corneal alkali-induced oxidative stress was reduced via the increased antioxidant capacity of corneal cells against reactive oxygen species (ROS). It is further suggested that the ability of H2 to induce the increase in antioxidant cell capacity is important for eye protection against various diseases or external influences associated with ROS production.

Postkorozivní striktury horního aerodigestivního traktu jsou vážnou komplikací poleptání ohrožující zdraví pacientů. Při vzniku rozsáhlých striktur nebo nekrózy je obvykle třeba poškozenou část zažívacího traktu zresekovat a obnovit kontinuitu polykacích cest pomocí autologního materiálu. Jestliže je strikturami současně postižen hypofarynx a jícen, je následná léčba obzvláště složitá, protože rekonstrukce této oblasti interferuje s polykáním i dýcháním. Při nutnosti náhrady je technicky nejjednodušší provést totální laryngektomii a resekci postižených částí polykacích cest s následnou rekonstrukcí. Morbidita spojená s totální laryngektomií je pro pacienty, kteří nemají korozivní postižení hrtanu, obvykle neakceptovatelná. Proto se vyvinula technika kolofaryngoplastiky. 35letý pacient požil v suicidálním pokusu kyselinu solnou. Akutně byla provedena ezofagektomie s krční ezofagostomií a totální gastrektomie. Pacient byl indikován k rekonstrukci polykacích cest v druhé době. Vzhledem k plně zachovalému hrtanu byla provedena rekonstrukce hypofaryngu a jícnu technikou kolofaryngoplastiky bez resekce hrtanu. Po intenznivní rehabilitaci hlasu a polykání byla provedena úspěšná dekanylace a obnoven plný perorální příjem.
Postcorrosive strictures of the upper aerodigestive tract are serious complications of caustic injury. In case of development of extensive strictures or necrosis, it is usually necessary to perform a resection of the damaged part of the digestive tract with a restoration of the continuity of the swallowing pathways by autologous material. Simultaneous involvement of the hypopharynx and esophagus makes subsequent treatment very difficult, because the reconstruction of this area interferes with swallowing and breathing. If the replacement is required, it is technically easier to perform total laryngectomy and resection of the affected part of the swallowing tract with subsequent reconstruction. Morbidity associated with total laryngectomy is generally unacceptable for patients without corrosive laryngeal injury. Therefore, the technique of colopharyngoplasty has been developed. A 35 years old patient swallowed hydrochloric acid in a suicidal attempt. Esophagectomy with cervical esophagostomy and total gastrectomy was performed urgently. The patient was indicated for delayed reconstruction of the swallowing tract. Due to the fully preserved larynx, reconstruction of the hypopharynx and esophagus was performed by colopharyngoplasty technique without laryngeal resection. After intensive rehabilitation of voice and swallowing, successful decannulation was performed and full oral intake was restored.

Kyselina mravenčí byla pro svoje leptavé účinky v 10–90% koncentraci používána jako jedna z metod léčby virových bradavic. Přestože je v dnešní době tato léčba považována za metodu obsolentní (pro možnost vzniku iritační dermatitidy, rizika poleptání a nekrózy s následnou jizvou), stále se s touto praxí v dermatologických ordinacích setkáváme. Autoři referují dvě kazuistiky poleptání kojenců, jejichž společnou příčinou byla nešťastná záměna lékovek při perorálním podání léčiva. U obou pacientek došlo k významnému poleptání postihující obličej, krk, dutinu ústní, u jedné z nich i jícen. V obou případech došlo též k různě vyjádřeným systémovým účinkům koncentrované kyseliny mravenčí.
Formic acid of 10–90% concentration has been used for its caustic effect as one of the methods of treatment of viral warts. Although nowadays considered obsolete for the risk of irritant dermatitis, burns and necrosis followed by scarring, the topical treatment is still being prescribed by dermatologists. The authors report two cases of burns in infants. The cause was common to both cases, the unfortunate erroneous administration of topical solution orally. The patients suffered significant burns affecting the face, neck, oral cavity and oesophagus. In both cases, the systemic effects of concentrated formic acid were expressed differently.
