BACKGROUND: Preoperative FOLFIRINOX chemotherapy is increasingly administered to patients with borderline resectable (BRPC) and locally advanced pancreatic cancer (LAPC) to improve overall survival (OS). Multicenter studies reporting on the impact from the number of preoperative cycles and the use of adjuvant chemotherapy in relation to outcomes in this setting are lacking. This study aimed to assess the outcome of pancreatectomy after preoperative FOLFIRINOX, including predictors of OS. METHODS: This international multicenter retrospective cohort study included patients from 31 centers in 19 European countries and the United States undergoing pancreatectomy after preoperative FOLFIRINOX chemotherapy (2012-2016). The primary end point was OS from diagnosis. Survival was assessed using Kaplan-Meier analysis and Cox regression. RESULTS: The study included 423 patients who underwent pancreatectomy after a median of six (IQR 5-8) preoperative cycles of FOLFIRINOX. Postoperative major morbidity occurred for 88 (20.8%) patients and 90-day mortality for 12 (2.8%) patients. An R0 resection was achieved for 243 (57.4%) patients, and 259 (61.2%) patients received adjuvant chemotherapy. The median OS was 38 months (95% confidence interval [CI] 34-42 months) for BRPC and 33 months (95% CI 27-45 months) for LAPC. Overall survival was significantly associated with R0 resection (hazard ratio [HR] 1.63; 95% CI 1.20-2.20) and tumor differentiation (HR 1.43; 95% CI 1.08-1.91). Neither the number of preoperative chemotherapy cycles nor the use adjuvant chemotherapy was associated with OS. CONCLUSIONS: This international multicenter study found that pancreatectomy after FOLFIRINOX chemotherapy is associated with favorable outcomes for patients with BRPC and those with LAPC. Future studies should confirm that the number of neoadjuvant cycles and the use adjuvant chemotherapy have no relation to OS after resection.

PURPOSE: Napabucasin is an investigational, orally administered reactive oxygen species generator bioactivated by intracellular antioxidant NAD(P)H:quinone oxidoreductase 1 that has been evaluated in various solid tumors, including metastatic colorectal cancer (mCRC). Phosphorylated signal transducer and activator of transcription 3 (pSTAT3) is hypothesized to predict response in napabucasin-treated patients with mCRC. PATIENT AND METHODS: In the multi-center, open-label, phase III CanStem303C (NCT02753127) study, adults with histologically confirmed mCRC that progressed on first-line fluoropyrimidine plus oxaliplatin ± bevacizumab were randomized to twice-daily napabucasin plus FOLFIRI (napabucasin) or FOLFIRI alone (control). The primary endpoint was overall survival (OS) in the general study population and in patients with pSTAT3-positive tumors (biomarker-positive). RESULTS: In the general study population (napabucasin, n = 624; control, n = 629), median OS was 14.3 months for napabucasin and 13.8 months for control (hazard ratio [HR], 0.976, one-sided P = .74). Overall, 44% of patients were biomarker-positive (napabucasin, n = 275; control, n = 272). In the biomarker-positive population, median OS was 13.2 months for napabucasin and 12.1 months for control (HR, 0.969; one-sided P > .99). In the control arm, median OS was shorter for biomarker-positive versus biomarker negative patients (12.1 vs. 18.5 months; HR, 1.518; nominal 2-sided P = .0002). The most common treatment-emergent adverse events (TEAEs) were diarrhea (napabucasin, 84.6%; control, 53.9%), nausea (60.5%, 50.5%), vomiting (41.2%, 29.3%), and abdominal pain (41.0%, 25.2%). Grade ≥3 TEAEs occurred in 73.8% of napabucasin-treated and 66.7% of control-treated patients, most commonly diarrhea (21.2%, 7.0%), neutrophil count decreased (13.7%, 19.2%), and neutropenia (13.3%, 15.2%). Safety was similar in biomarker-positive patients. CONCLUSION: In patients with previously treated mCRC, adding napabucasin to FOLFIRI did not improve OS. Results from the control arm indicate that pSTAT3 is an adverse prognostic factor in mCRC.

Problém nedostupnosti léčiv a výpadků je častým problémem současnosti. Omezení dodávek může ohrozit některé léčebné režimy onkologicky nemocných. Článek podává systematický přehled studií, které hodnotí účinnost a bezpečnost chemoterapeutických režimů kombinujících fluorouracil a leukovorin ve standardní a snížené dávce v terapii kolorektálního karcinomu. Celkem bylo do přehledu zahrnuto 13 prospektivních i retrospektivních prací. Nebyly zahrnuty metaanalýzy a přehledové práce. Z přehledu je zřejmé, že nižší dávka leukovorinu zásadně neovlivňuje účinnost režimů kombinujících 5-fluorouracil s leukovorinem v terapii pacientů s kolorektálním karcinomem. Podobně ani v případě porovnání bezpečnosti nemělo snížení dávky leukovorinu zásadní vliv na frekvenci a tíži sledovaných nežádoucích účinků. Naopak, ve třech studiích se překvapivě některé nežádoucí účinky objevovaly častěji u vyšší dávky leukovorinu. Dále článek předkládá výsledky dotazníkového průzkumu managementu výpadku leukovorinu na odděleních přípravy cytostatik (N = 46) v rámci České republiky. Zpětnou vazbu poskytlo 35 pracovišť. V 17 případech musela být na odděleních přípravy cytostatik v České republice přijata omezení stran podávání plné dávky leukovorinu. Tato omezení spočívala ve snížení dávky injekční formy leukovorinu, změně chemoterapeutického režimu, podávání perorální formy kalcium folinátu, vynucené terapeutické přestávce nebo kombinaci těchto přístupů.
The problem of unavailability of drugs and shortages have been a common problem in recent years. Shortages may threaten some treatment regimens for oncological patients. This article presents a systematic review of studies evaluating the efficacy and safety of chemotherapy regimens combining fluorouracil and standard or low-dose leucovorin in treating colorectal cancer. A total of 13 prospective and retrospective studies were included in the review. Meta-analyses and review papers were excluded. It is apparent from the systematic review that a lower dose of leucovorin does not fundamentally affect the efficacy of regimens combining 5-fluorouracil with leucovorin in treating patients with colorectal cancer. Similarly, even in the case of safety, reducing the dose of leucovorin did not influence the frequency and severity of observed adverse effects. Surprisingly, in three studies, some of the adverse effects occurred more often with the higher dose of leucovorin. Furthermore, the article presents the results of a questionnaire survey of the management of leucovorin shortages at the departments of preparation of cytostatics (N = 46) within the Czech Republic. In total, 35 workplaces provided feedback. In 17 cases, the departments for the preparation of cytostatics in the Czech Republic had to accept restrictions on administering the full dose of leucovorin. These restrictions consisted of reducing the dose of the injectable form of leucovorin, changing the chemotherapy regimen, administering the oral form of calcium folinate, forcing a therapeutic break, or a combination of these approaches.

Liposomální irinotecan (Onivyde) je první léčivo specificky schválené pro použití v kombinaci s 5-fluorouracilem/leucovorinem v léčbě metastazujícího adenokarcinomu pankreatu u pacientů, u nichž došlo k progresi po terapii založené na gemcitabinu. Tato kazuistika pojednává o prvních vlastních zkušenostech s tímto přípravkem ve druhé linii léčby tohoto onemocnění.
Liposomal irinotecan (Onivyde) is the first drug specifically approved for use in combination with 5-fluorouracil/leucovorin in the treatment of metastatic pancreatic adenocarcinoma in patients who have progressed after gemcitabine-based therapy. This case report discusses the first in-house experience with this agent in the second-line treatment of this disease.

Karcinom pankreatu je nádorové onemocnění s velmi špatnou prognózou, které představuje třetí nejčastější příčinu úmrtí na zhoubný nádor ve vyspělých zemích a jehož incidence a mortalita dle predikcí nadále významným způsobem porostou. Téměř 80 % pacientů je diagnostikováno s pokročilým neresekabilním onemocněním, a je tudíž odkázáno na paliativní protinádorovou léčbu s limitovanou účinností. V případě 10–20 % nemocných, kteří úspěšně absolvovali radikální resekci lokalizovaného onemocnění a následnou adjuvantní chemoterapii, však u naprosté většiny dojde k relapsu onemocnění do 2–3 let od operace. Příčiny lze hledat v pozdním stanovení diagnózy v důsledku dlouhého klinicky asymptomatického průběhu, v komplikované anatomické lokalizaci, ve výrazné nádorové heterogenitě znesnadňující testování nových léčiv a v neposlední řadě i v přítomnosti denzního nádorového stromatu omezujícího přístup cytostatik i cílených léčiv do nádorové tkáně. V tomto sdělení uvádíme souhrn aktuálních možností diagnostiky a léčby lokalizovaných i pokročilých forem karcinomu pankreatu včetně možností molekulární diagnostiky a cílené léčby podskupin.
Pancreatic ductal adenocarcinoma has a very poor prognosis and poses the third-leading cause of cancer-related deaths. Its incidence and mortality have been predicted to increase significantly in the upcoming years. Almost 80% of patients are diagnosed with advanced unresectable disease and rely on palliative anticancer treatment with limited efficacy. However, in 10-20% of patients who have successfully undergone radical surgical resection of the localized disease and subsequent adjuvant chemotherapy, the vast majority will relapse within 2-3 years of surgery. The reasons can be found in late diagnosis due to the prolonged clinically asymptomatic course of the disease, complicated anatomical localization, significant tumor heterogeneity, which makes it difficult to test new drugs and, finally, the presence of dense tumor stroma, which complicates the access of cytostatics and targeted drugs into the tumor tissue. Here we present a summary of current treatment options for localized and advanced pancreatic cancer, including molecular diagnostics and targeted treatment of small patient subgroups,

IMPORTANCE: The benefit of adjuvant chemotherapy after resection of pancreatic cancer following neoadjuvant combination treatment with folinic acid, fluorouracil, irinotecan, and oxaliplatin (FOLFIRINOX) is unclear. OBJECTIVE: To assess the association of adjuvant chemotherapy with overall survival (OS) in patients after pancreatic cancer resection and neoadjuvant FOLFIRINOX treatment. DESIGN, SETTING, AND PARTICIPANTS: This international, multicenter, retrospective cohort study was conducted from January 1, 2012, to December 31, 2018. An existing cohort of patients undergoing resection of pancreatic cancer after FOLFIRINOX was updated and expanded for the purpose of this study. All consecutive patients who underwent pancreatic surgery after at least 2 cycles of neoadjuvant FOLFIRINOX chemotherapy for nonmetastatic pancreatic cancer were retrospectively identified from institutional databases. Patients with resectable pancreatic cancer, borderline resectable pancreatic cancer, and locally advanced pancreatic cancer were eligible for this study. Patients with in-hospital mortality or who died within 3 months after surgery were excluded. EXPOSURES: The association of adjuvant chemotherapy with OS was evaluated in different subgroups including interaction terms for clinicopathological parameters with adjuvant treatment in a multivariable Cox model. Overall survival was defined as the time starting from surgery plus 3 months (moment eligible for adjuvant therapy), unless mentioned otherwise. RESULTS: We included 520 patients (median [interquartile range] age, 61 [53-66] years; 279 [53.7%] men) from 31 centers in 19 countries. The median number of neoadjuvant cycles of FOLFIRINOX was 6 (interquartile range, 5-8). Overall, 343 patients (66.0%) received adjuvant chemotherapy, of whom 68 (19.8%) received FOLFIRINOX, 201 (58.6%) received gemcitabine-based chemotherapy, 14 (4.1%) received capecitabine, 45 (13.1%) received a combination or other agents, and 15 (4.4%) received an unknown type of adjuvant chemotherapy. Median OS was 38 months (95% CI, 36-46 months) after diagnosis and 31 months (95% CI, 29-37 months) after surgery. No survival difference was found for patients who received adjuvant chemotherapy vs those who did not (median OS, 29 vs 29 months, univariable hazard ratio [HR], 0.99; 95% CI, 0.77-1.28; P = .93). In multivariable analysis, only the interaction term for lymph node stage with adjuvant therapy was statistically significant: In patients with pathology-proven node-positive disease, adjuvant chemotherapy was associated with improved survival (median OS, 26 vs 13 months; multivariable HR, 0.41 [95% CI, 0.22-0.75]; P = .004). In patients with node-negative disease, adjuvant chemotherapy was not associated with improved survival (median OS, 38 vs 54 months; multivariable HR, 0.85; 95% CI, 0.35-2.10; P = .73). CONCLUSIONS AND RELEVANCE: These results suggest that adjuvant chemotherapy after neoadjuvant FOLFIRINOX and resection of pancreatic cancer was associated with improved survival only in patients with pathology-proven node-positive disease. Future randomized studies should be conducted to confirm this finding.
