INTRODUCTION: Existing advance care planning (ACP) definitional frameworks apply to individuals with decision-making capacity. We aimed to conceptualize ACP for dementia in terms of its definition and issues that deserve particular attention. METHODS: Delphi study with phases: (A) adaptation of a generic ACP framework by a task force of the European Association for Palliative Care (EAPC); (B) four online surveys by 107 experts from 33 countries, September 2021 to June 2022; (C) approval by the EAPC board. RESULTS: ACP in dementia was defined as a communication process adapted to the person's capacity, which includes, and is continued with, family if available. We identified pragmatic boundaries regarding participation and time (i.e., current or end-of-life care). Three interrelated issues that deserve particular attention were capacity, family, and engagement and communication. DISCUSSION: A communication and relationship-centered definitional framework of ACP in dementia evolved through international consensus supporting inclusiveness of persons with dementia and their family. HIGHLIGHTS: This article offers a consensus definitional framework of advance care planning in dementia. The definition covers all stages of capacity and includes family caregivers. Particularly important are (1) capacity, (2) family, (3) engagement, and communication. Fluctuating capacity was visualized in relation to roles and engaging stakeholders.

Aim: In patients with severe illnesses, particularly in situations of unfavorable prognosis, it is essential to consider and decide on the appropriateness of further treatment. The ability to conduct a values-oriented conversation is crucial for providing care that aligns with the patient's values and preferences. Values History Taking (VHT) should be included in the curriculum of medical schools. Method: A questionnaire survey mapping the attitudes of internal medicine educators at the 1st Faculty of Medicine, Charles University, and the Faculty of Medicine in Plzeň, Charles University, regarding the significance, experiences, potential benefits, and barriers of incorporating value history taking into the teaching of standard medical history interviews. Results: A total of 123 questionnaires were evaluated in the study. Currently, value history taking, as well as its individual elements, are not widely integrated into medical education or practice. However, most physicians (83%) consider it very important. Among the potential benefits, they see higher patient and family satisfaction, greater ability to respect the patient's wishes, and the potential reduction in the number of unplanned or unwanted rehospitalizations towards the end of life. Conclusion: The positive attitude of educators supports the need for the integration of value history taking into both education and routine clinical practice. Value history taking becomes a part of the structured medical history interview in the subject of Internal Propaedeutics and Internal Medicine at the 1st Faculty of Medicine, Charles University, and the Faculty of Medicine in Plzeň, Charles University.

Respekt k autonomii pacienta je jedním ze čtyř základních etických principů platných pro současnou západní medicínu. Pacient musí udělit dobrovolný informovaný souhlas s postupem léčby. V něm by se měly odrážet pacientovy preference, jeho žádosti i hodnotový systém. Poskytovatel je povinen zajistit, aby byl pacient srozumitelným způsobem v dostatečném rozsahu informován o svém zdravotním stavu a o navrženém individuálním léčebném postupu a všech jeho změnách. Cílem příspěvku je ukázat způsob praktické výuky týkající se rozhodování o léčbě pacientů s těžkými komunikačními problémy, včetně možných řešení. Data jsou sbírána tázacími archy, do kterých lékaři zapisují eticky sporné situace z vlastní praxe. Data jsou důsledně generalizována a připravována metodou story-tellingu. Kazuistiky jsou vyučovány metodou Case Based Learning. Případová studie je příběhem pana Karla (80 let), který je v péči dcery. Je neslyšící, dorozumívá se znakovou řečí, s počínající demencí Alzheimerova typu, zhoršuje se mu zrak. Operací šedého zákalu je možnost problém vyřešit. Pan Karel zákrok odmítá. Dcera na lékaře naléhá, aby otci sdělil, že se o operaci nejedná. Argumentuje komplikovanou péčí o dementní hluchoslepou osobu. Podle lékařova názoru má operace pro pacienta jednoznačnou benefici a velmi nízkou pravděpodobnost poškození. Výsledkem analýzy případové studie jsou expertní názory (medicínský, etický a právní).
Respect for patient’s autonomy is one of the four basic ethical principles applicable to contemporary Western medicine. The patient must provide voluntary informed consent to the treatment procedure. It should reflect the patient’s preferences, requests, and value system. The provider is obliged to ensure that the patient is sufficiently informed in a comprehensible manner about his/her health condition. The aim of the study is to show the method of practical teaching regarding decision-making on the treatment of patients with severe communication problems, including possible solutions. Data are collected through questionnaires in which doctors write down ethically questionable situations from their own practice. Data are consistently generalized and prepared using the story-telling method. Case studies are taught using the Case Based Learning method. The case study is conceived as a story of Mr. Karel (80 years old), who is in the care of his daughter. He is deaf, communicates through sign language, has been diagnosed with Alzheimer’s type dementia, and his eyesight is deteriorating. Cataract surgery is an option to solve the problem. Mr. Karel, however, refuses the procedure. The daughter urges the doctor to tell the father that it is not an operation. In the doctor’s opinion, the operation has a definite benefit for the patient and a very low probability of harm. The result of the case study analysis are expert opinions (medical, ethical and legal).

Východiská: Etické aspekty sa stávajú čoraz dôleži- tejšou súčasťou postupov pri kardiopulmonálnej re- suscitácii (KPR) a rozhodovaniach u pacientov na konci života. Súbor a metódy: Ako metódu prieskumu sme použili anonymný dotazník, ktorý sme sprístupnili on- line pre širokú verejnosť. Nášho prieskumu sa zúčast- nilo 163 respondentov, rôznych vekových skupín, pohlavia a vzdelania. Výsledky: Z odpovedí vyplýva, že až 78,5 % respon- dentov by súhlasilo s predĺžením resuscitácie iba z dôvodu uchovania orgánov pre potreby transplan- tácie. Záver: Poznanie etických aspektov resuscitácie je dôležité v živote každého zdravotníckeho pracovníka, ako aj laika, hrá dôležitú rolu pri rozhodovaní o za- čatí a nezačatí resuscitácie, odmietnutí resuscitácie pacientom, rodinou a pri prítomnosti rodinného prí- slušníka pri oživovaní blízkej osoby.
Background: Ethical aspects are becoming an incre- asingly important part of cardiopulmonary resuscita- tion (CPR) procedures and end-of-life patient deci- sions. File and methods: As a survey method, we chose an anonymous questionnaire, which we made avai- lable online to the general public. 163 respondents, of different ages, sex and education, took part in our survey. Results: The answers show that up to 78,5% of res- pondents would agree to prolong resuscitation only because of the preservation of organs for transplan- tation. Conclusion: Knowledge of the ethical aspects of re- suscitation is important in the life of every healthca- re professional, as well as the layman, plays an im- portant role in deciding whether to resuscitate, refusing resuscitation to patients, family and the pre- sence of a family member in resuscitation.

BACKGROUND: the mySupport advance care planning intervention was originally developed and evaluated in Northern Ireland (UK). Family caregivers of nursing home residents with dementia received an educational booklet and a family care conference with a trained facilitator to discuss their relative's future care. OBJECTIVES: to investigate whether upscaling the intervention adapted to local context and complemented by a question prompt list impacts family caregivers' uncertainty in decision-making and their satisfaction with care across six countries. Second, to investigate whether mySupport affects residents' hospitalisations and documented advance decisions. DESIGN: a pretest-posttest design. SETTING: in Canada, the Czech Republic, Ireland, Italy, the Netherlands and the UK, two nursing homes participated. PARTICIPANTS: in total, 88 family caregivers completed baseline, intervention and follow-up assessments. METHODS: family caregivers' scores on the Decisional Conflict Scale and Family Perceptions of Care Scale before and after the intervention were compared with linear mixed models. The number of documented advance decisions and residents' hospitalisations was obtained via chart review or reported by nursing home staff and compared between baseline and follow-up with McNemar tests. RESULTS: family caregivers reported less decision-making uncertainty (-9.6, 95% confidence interval: -13.3, -6.0, P < 0.001) and more positive perceptions of care (+11.4, 95% confidence interval: 7.8, 15.0; P < 0.001) after the intervention. The number of advance decisions to refuse treatment was significantly higher after the intervention (21 vs 16); the number of other advance decisions or hospitalisations was unchanged. CONCLUSIONS: the mySupport intervention may be impactful in countries beyond the original setting.
