Using an active targeting approach of chemotherapeutics-loaded nanocarriers (NCs) with monoclonal antibodies is a potential strategy to improve the specificity of the delivery systems and reduce adverse reactions of chemotherapeutic drugs. Specific targeting of the human epidermal growth factor receptor-2 (HER-2), expressed excessively in HER-2-positive breast cancer cells, can be achieved by conjugating NCs with an anti-HER-2 monoclonal antibody. We constructed trastuzumab-conjugated chitosan iodoacetamide-coated NCs containing doxorubicin (Tras-Dox-CHI-IA-NCs) as a tumor-targeted drug delivery system, during the study. Chitosan-iodoacetamide (CHI-IA) was synthesized and utilized to prepare trastuzumab-conjugated NCs (Tras-NCs). The morphology, physicochemical properties, drug loading, drug release, and biological activities of the NCs were elucidated. The Tras-NCs were spherical, with a particle size of approximately 76 nm, and had a positive zeta potential; after incorporating the drug, the size of the Tras-NC increased. A prolonged, 24-h drug release from the NCs was achieved. The Tras-NCs exhibited high cellular accumulation and significantly higher antitumor activity against HER-2-positive breast cancer cells than the unconjugated NCs and the drug solution. Therefore, Tras-Dox-CHI-IA-NCs could be a promising nanocarrier for HER-2-positive breast cancer.

PURPOSE: Trastuzumab deruxtecan (T-DXd) is a human epidermal growth factor 2 (HER2)-directed antibody-drug conjugate approved in HER2-expressing breast and gastric cancers and HER2-mutant non-small-cell lung cancer. Treatments are limited for other HER2-expressing solid tumors. METHODS: This open-label phase II study evaluated T-DXd (5.4 mg/kg once every 3 weeks) for HER2-expressing (immunohistochemistry [IHC] 3+/2+ by local or central testing) locally advanced or metastatic disease after ≥1 systemic treatment or without alternative treatments. The primary end point was investigator-assessed confirmed objective response rate (ORR). Secondary end points included safety, duration of response, progression-free survival (PFS), and overall survival (OS). RESULTS: At primary analysis, 267 patients received treatment across seven tumor cohorts: endometrial, cervical, ovarian, bladder, biliary tract, pancreatic, and other. The median follow-up was 12.75 months. In all patients, the ORR was 37.1% (n = 99; [95% CI, 31.3 to 43.2]), with responses in all cohorts; the median DOR was 11.3 months (95% CI, 9.6 to 17.8); the median PFS was 6.9 months (95% CI, 5.6 to 8.0); and the median OS was 13.4 months (95% CI, 11.9 to 15.5). In patients with central HER2 IHC 3+ expression (n = 75), the ORR was 61.3% (95% CI, 49.4 to 72.4), the median DOR was 22.1 months (95% CI, 9.6 to not reached), the median PFS was 11.9 months (95% CI, 8.2 to 13.0), and the median OS was 21.1 months (95% CI, 15.3 to 29.6). Grade ≥3 drug-related adverse events were observed in 40.8% of patients; 10.5% experienced adjudicated drug-related interstitial lung disease (ILD), with three deaths. CONCLUSION: Our study demonstrates durable clinical benefit, meaningful survival outcomes, and safety consistent with the known profile (including ILD) in pretreated patients with HER2-expressing tumors receiving T-DXd. Greatest benefit was observed for the IHC 3+ population. These data support the potential role of T-DXd as a tumor-agnostic therapy for patients with HER2-expressing solid tumors.

HER-2 low karcinom prsu je název pro zcela heterogenní podskupinu karcinomů prsu, jejímž společným znakem je nízká až střední exprese HER-2 receptoru nesplňující kritéria tzv. HER-2 pozitivity (HER-2 low nádory jsou definovány jako nádory s expresí HER2 na úrovni 1+ nebo 2+ a současně ISH negativní). Pro tuto skupinu je charakteristická vysoká léčebná účinnost protilátkových konjugátů schopných vyvolávat bystander efekt (ADC 3. generace, především trastuzumab-deruxtecan) oproti starším ADC bez této schopnosti (T-DM1) a monoklonálním protilátkám (trastuzumab, pertuzumab, margetuximab), které vzhledem k absenci onkogenní HER-2 signalizace u HER-2 low tumorů mají účinnost téměř nulovou. Kromě HER-2 low nádorů prsu dále můžeme definovat i podtyp "HER-2 ultra low" (IHC na úrovni nekompletního a slabého barvení u ≤ než 10 % nádorových buněk), u kterého je též zaznamenána účinnost T-DXd. Vzhledem k časté HER-2 heterogenitě v čase nadále narůstá potřeba pravidelných rebiopsií ke znovuzhodnocení stavu HER-2 exprese a z toho plynoucí možnosti zahájení terapie pomocí T-DXd i v pozdějších fázích léčby metastatického karcinomu prsu s příznivým ovlivněním prognózy.
HER-2 low breast cancer is newly established term of completely heterogeneous subgroup of breast cancer sharing one common phenotype which is low to moderate expression of HER-2 protein and not meeting criteria for HER-2 positivity. The specific attribute of this subgroup is high responsiveness to antibody-drug conjugates (ADC ́s) that can cause bystander killing effect (ADC ́s of the 3rd generation - especially T-DXd) in comparison to older ADC ́s lacking this disposition (T-DM1) and to monoclonal antibodies (trastuzumab, pertuzumab, margetuximab) with almost zero efficacy due to the absence of oncogenic HER-2 signalling in HER-2 low tumours. HER-2 ultra-low subtype is defined as ≤ 10% of infiltrating cancer cells showing incomplete and faint/weak membrane staining with proven efficacy of T-DXd as well. Due to the frequent HER-2 heterogenity during the course of time the re-biopsy approach is of crucial importance to reassess the up-to-date HER-2 status enabling subsequent treatment with T-DXd.

Nově definovaná kategorie HER2-low karcinomu prsu představuje onemocnění s nízkou expresí receptoru HER2 napříč luminálními a triple negativními subtypy nemoci. K dosud užívaným léčebným možnostem, jakými jsou chemoterapie, cílená léčba a imunoterapie checkpoint inhibitory, jsou nově zaváděny tzv. konjugáty protilátka-léčivo 2. generace, ke kterým patří trastuzumab deruxtekan a sacituzumab govitekan. Následující sdělení se věnuje vymezení pojmu HER2-low karcinom prsu, definici konjugátu protilátka-léčivo a jejich významu pro léčbu HER2-low karcinomu prsu.
The newly defined category of HER2-low breast cancer represents disease with low HER2 receptor expression across luminal and triple negative subtypes of the disease. In addition to previously used treatment options such as chemotherapy, targeted therapy and checkpoint inhibitor immunotherapy, second-generation antibody-drug conjugates, which include trastuzumab deruxtecan and sacituzumab govitecan, are newly introduced. The following review discusses the definition of HER2-low breast cancer, the definition of antibody-drug conjugates and their relevance to the treatment of HER2-low breast cancer.
Současným standardem péče u HER2-pozitivního karcinomu prsu s vyšším rizikem rekurence je kombinace chemoterapie s duální anti-HER2 léčbou pro její synergický protinádorový účinek. Neoadjuvantní terapie karcinomu prsu vede ke zlepšení jeho operability, zvýšení počtu prs šetřících operací a šance na dosažení patologické kompletní remise (pCR), zlepšení prognózy a možnosti sledovat odpověď nádoru na léčbu. Může také pomoci identifikovat rizikové pacientky s invazivním nádorovým zbytkem v resekátu, u nichž je vhodná postneoadjuvantní terapie konjugátem protilátky a léku. Kromě standardních ukazatelů výsledku léčby, jako je procento recidiv, přežití bez nemoci a celkové přežití, jež se často používají k posouzení účinnosti léčby v adjuvanci, je u pacientů, kteří dostávají neoadjuvantní terapii, užitečným měřítkem účinnosti léčby patologická odpověď včetně míry pCR. V současné době se začínají uplatňovat strategie deeskalace s cílem bezpečně snížit zátěž chemoterapií při optimalizaci léčby cílené na HER2. Pro správnou volbu deeskalačních strategií a personalizaci léčby je nezbytný vývoj a validace spolehlivého biomarkeru. Kromě toho jsou v současné době prověřovány také nové terapie, které by dále zlepšily výsledky léčby HER2-pozitivního karcinomu prsu.
The current standard of care for HER2-positive breast cancer with a higher risk of recurrence is the combination of chemotherapy with dual anti-HER2 therapy for its synergistic antitumor effect. Neoadjuvant therapy for breast cancer leads to improved operability, increased breast-conserving surgery and the chance of achieving pathological complete remission (pCR), improved prognosis and the ability to monitor tumour response to treatment. It may also help identify at-risk patients with invasive tumour residual, for whom post-neoadjuvant therapy with an antibody-drug conjugate is appropriate. In addition to standard outcome measures such as recurrence rate, disease-free survival, and overall survival, which are often used to assess the efficacy of treatment in the adjuvant setting, pathological response, including pCR rate, is a useful measure of treatment efficacy in patients receiving neoadjuvant therapy. Currently, de-escalation strategies are beginning to be applied to safely reduce the chemotherapy burden while optimizing HER2-targeted therapy. The development and validation of a reliable biomarker is essential for the proper selection of de-escalation strategies and personalization of treatment. In addition, novel therapies are currently being investigated to further improve treatment outcomes in HER2-positive breast cancer

Clinical trials frequently include multiple end points that mature at different times. The initial report, typically based on the primary end point, may be published when key planned co-primary or secondary analyses are not yet available. Clinical Trial Updates provide an opportunity to disseminate additional results from studies, published in JCO or elsewhere, for which the primary end point has already been reported.Final overall survival (OS) in SOPHIA (ClinicalTrials.gov identifier: NCT02492711), a study of margetuximab versus trastuzumab, both with chemotherapy, in patients with previously treated human epidermal growth factor receptor 2-positive advanced breast cancer, is reported with updated safety. Overall, 536 patients in the intention-to-treat population were randomly assigned to margetuximab (15 mg/kg intravenously once every 3 weeks; n = 266) plus chemotherapy or trastuzumab (6 mg/kg intravenously once every 3 weeks after a loading dose of 8 mg/kg; n = 270) plus chemotherapy. Primary end points were progression-free survival, previously reported, and OS. Final OS analysis was triggered by 385 prespecified events. The median OS was 21.6 months (95% CI, 18.89 to 25.07) with margetuximab versus 21.9 months (95% CI, 18.69 to 24.18) with trastuzumab (hazard ratio [HR], 0.95; 95% CI, 0.77 to 1.17; P = .620). Preplanned, exploratory analysis of CD16A genotyping suggested a possible improvement in OS for margetuximab in CD16A-158FF patients versus trastuzumab (median OS, 23.6 v 19.2 months; HR, 0.72; 95% CI, 0.52 to 1.00) and a possible improvement in OS for trastuzumab in CD16A-158VV patients versus margetuximab (median OS, 31.1 v 22.0 months; HR, 1.77; 95% CI, 1.01 to 3.12). Margetuximab safety was comparable with trastuzumab. Final overall OS analysis did not demonstrate margetuximab advantage over trastuzumab. Margetuximab studies in patients with human epidermal growth factor receptor 2-positive breast cancer with different CD16A allelic variants are warranted.

Trastuzumab se stal v kombinaci s dalšími chemoterapeutiky nebo hormonální léčbou vysoce účinnou léčbou HER2 pozitivního karcinomu prsu a žaludku. Jeho klinické benefity znevažuje řada nežádoucích účinků, z nichž jako významná vystupuje do popředí trastuzumabem způsobená kardiotoxicita (TZK). Jedná se o známý účinek poškození srdce způsobené užíváním této cílené protinádorové molekuly. Článek si dává za cíl diskutovat mechanismus, významné rizikové faktory a management nežádoucí reakce. Na závěr přináší možnosti minimalizace tohoto rizika a možné způsoby jeho prevence nebo léčby.
Trastuzumab has become a highly effective treatment for HER2-positive breast and gastric cancer in combination with other chemotherapeutic agents or hormone therapy. Its clinical benefits are counterbalanced by a number of adverse effects, of which trastuzumab-induced cardiotoxicity (TZK) is a prominent one. This is a known effect of cardiac damage caused by the use of this targeted anti-cancer molecule. This article aims to discuss the mechanism, important risk factors and management of the adverse reaction. Finally, it presents options to minimize this risk and possible ways to prevent or treat it.

HER2 pozitivní nádory jsou agresivním podtypem karcinomu prsu a vyžadují komplexní onkologickou léčbu. U pacientek s časným onemocněním se snažíme o dosažení dlouhodobé remise onemocnění, u pacientek s metastatickým onemocněním je cílem léčby dosažení dlouhodobé kontroly onemocnění, prodloužení celkového přežití pacientky a ovlivnění symptomů onemocnění s cílem zlepšení kvality života. Léčba mladých pacientek s karcinomem prsu je v mnoha ohledech specifická. U této podskupiny pacientek jsou častěji diagnostikovány agresivní podtypy, pokročilejší onemocnění, nádory vyššího gradu s nižší expresí steroidních receptorů. Více zastoupeny jsou hereditárně podmíněné karcinomy, diskutována je problematika zachování fertility, samostatnou kapitolou je léčba pacientek gravidních a pacientek v laktaci. V rámci chirurgických výkonů je řešena otázka rekonstrukčních výkonů. Co se systémové léčby týče, mladé pacientky jsou vzhledem k odlišnému zastoupení jednotlivých podtypů karcinomu prsu a pokročilejším stadiím onemocnění více léčeny neoadjuvantní terapií, častěji volíme intenzifikaci léčby, např. v podobě dose-dense režimů. V rámci adjuvantní hormonální léčby je u mladých pacientek v indikovaných případech volena hormonální léčba v kombinaci s ovariální supresí. Léčba mladých pacientek s HER2 pozitivním onemocněním vychází ve většině případů ze stejných doporučení jako léčba ostatních pacientek s karcinomem prsu. V klinické praxi se však vyskytují určitá specifika, která budou diskutována v rámci tohoto článku. Zásadní je indikace jednotlivých diagnostických a léčebných postupů v rámci multidisciplinárního týmu, jedině tak lze dosáhnout nejlepších výsledků.
HER2-positive cancer is an aggressive subtype of breast tumors and requires complex oncological treatment. In patients with early-stage disease, the goal is to achieve long-term remission of the disease, while in patients with metastatic disease, the goal of treatment is to achieve long-term disease control, prolong overall survival and influence disease symptoms to improve quality of life. The treatment of adult young breast cancer patients is specific in many ways. This subset of patients is more likely to be diagnosed with aggressive subtypes, more advanced diseases, and higher-grade tumors with lower steroid receptor expression. Hereditary carcinomas are more represented, the issue of fertility preservation is discussed, and the treatment of pregnant and lactating patients is a separate chapter. In the context of surgical procedures, the issue of reconstructive procedures is addressed. As far as systemic treatment is concerned, adult young patients are more often treated with neoadjuvant therapy due to the different representation of individual subtypes of breast cancer and more advanced stages of the disease; more often we choose intensification of treatment, e. g. in the form of dose-dense regimens. In the context of adjuvant hormonal therapy, hormone therapy combined with ovarian suppression is chosen in young patients when indicated. The treatment of young patients with HER2-positive disease is in most cases based on the same guidelines as for other breast cancer patients. However, some specificities in clinical practice will be discussed in the context of this article. The indication of the different diagnostic and therapeutic procedures within a multidisciplinary team is essential, as this is the only way to achieve the best results.
