Autoři ve své práci prezentují techniku bilaterální rekonstrukce prsu u pěti pacientek, umožňující současný odběr obou myokutánních laloků latissimus dorsi. Tři pacientky podstoupily oboustrannou okamžitou rekonstrukci po profylaktické mastektomii. U jedné pacientky byla provedena sekundární rekonstrukce a u jedné pacientky byl použit myokutánní lalok latissimus dorsi k rekonstrukci po profylaktické mastektomii a rekonstrukci implantáty s následnou oboustrannou nekrózou kůže. Popsaná technika umožňuje bezpečnou rekonstrukci prsu v rámci jednoho operačního výkonu. Průměrná doba operace byla asi 4 hodiny, čímž se tato operace řadí do středu spektra rekonstrukcí, mezi rekonstrukce volnými laloky a prsními implantáty.
The authors present a technique of bilateral breast reconstruction in case series of 5 patients, allowing simultaneous harvest of both latissimus dorsi myocutaneous flaps. Three patients underwent bilateral immediate reconstruction after prophylactic mastectomy. One patient underwent a delayed reconstruction, in 1 patient latissimus dorsi myocutaneous flap was used after prophylactic mastectomy and reconstruction with implants followed by bilateral necrosis of the skin flaps. The described technique enables safe breast reconstruction in one procedure. The average reconstruction time was about 4 hours, which represents bilateral latissimus dorsi procedure to the centre of the breast reconstructions range, between the reconstructions with double free tissue transfer and the breast implants.
Úvod: Defekty dolních končetin mohou být způsobeny různými příčinami, vč. infekcí, cévních onemocnění, resekcí nádorů a drtivých nebo avulzních poranění. Léčba defektů dolní končetiny představuje složitý problém, zvláště pokud jsou defekty rozsáhlé s hlubokou ztrátou měkkých tkání. Tato poranění je obtížné krýt lokálními kožními laloky, vzdálenými kožními laloky nebo dokonce konvenčními volnými laloky kvůli ohroženým recipientním cévám. V takových případech by mohla být cévní stopka volného laloku dočasně anastomozována k recipientním cévám kontralaterální zdravé nohy a poté rozdělena po adekvátní neovaskularizaci laloku z lůžka rány. Ideální doba pro rozdělení takových pediklů by měla být prozkoumána a přesně vyhodnocena, aby byla u těchto náročných stavů a postupů dosažena maximální možná úspěšnost. Metody: Šestnáct pacientů, kteří neměli vhodnou přilehlou recipientní cévu pro rekonstrukci volným lalokem, bylo v období od února 2017 do června 2021 operováno volným lalokem latissimus dorsi ve formě cross-leg flap. Průměrný rozměr defektu měkkých tkání byl 12 × 11 cm (nejmenší 6 × 7 cm; největší 20 × 14 cm). U 12 pacientů byly přítomny zlomeniny tibie typu Gustilo 3B, zatímco u dalších 4 pacientů nebyly přítomny žádné zlomeniny. Předoperačně byla u všech pacientů provedena arteriální angiografie. Po 4. týdnu po operaci byla kolem pediklu na 15 minut nasazena nedrticí svorka. Každý další den byla doba svorky prodloužena o 15 minut (průměrně 14 dní). Poslední 2 dny byl pedikl klemován na 2 hodiny a krvácení bylo hodnoceno vpichovým testem jehlou. Výsledky: Ve všech případech byla hodnocena doba klemování, aby bylo možné vědecky vypočítat adekvátní dobu cévní perfuze potřebnou k plné výživě laloku. Všechny laloky zcela přežily s výjimkou dvou případů nekrózy distální části laloku. Závěr: Cross-leg latissimus dorsi může být řešením pro rozsáhlé defekty měkkých tkání na dolních končetinách, zejména při absenci vhodných recipientních cév nebo v případech, kdy by použití žilních štěpů nebylo možné. Je však třeba stanovit ideální dobu před oddělením zkřížené cévní stopky, aby byla dosažena maximální možná úspěšnost rekonstrukce.
Background: Lower limb defects may be present due to various causes including infections, vascular diseases, tumor resections, and crush or avulsion injuries. Management of lower leg defects is a complex problem, especially when they are large with deep soft tissue loss. These wounds are difficult to be covered with local skin flaps, distant skin flaps or even conventional free flaps because of the compromised recipient vessels. In such cases, the vascular pedicle of the free flap could be anastomosed to the recipient vessels of the contralateral healthy leg temporarily and then divided after adequate neo-vascularization of the flap from the wound bed. The ideal time to divide such pedicles should be investigated and accurately assessed to have the maximum success rate possible for these challenging conditions and procedures. Methods: Sixteen patients who did not have a suitable adjacent recipient vessel for free flap reconstruction had been operated with cross leg free latissimus dorsi flap between February 2017 and June 2021. The mean soft tissue defect dimension was 12 × 11 cm (the smallest 6 × 7 cm; the largest 20 × 14 cm). Gustilo type 3B tibial fractures were present in 12 patients, while no fractures were present in the other 4 patients. Preoperatively, arterial angiography was performed on all patients. After the fourth week post-operatively, non-crushing clamp was applied around the pedicle for 15 minutes. The clamping time was increased by 15 minutes on each consequent day (average 14 days). On the last 2 days, the pedicle was clamped for 2 hours, and bleeding was assessed by a needle prick test. Results: Clamping time was assessed in each case to reach a scientific calculation of the adequate vascular perfusion time needed for full flap nourishment. All flaps survived completely except two cases of distal flaps necrosis. Conclusion: Cross-leg free latissimus dorsi can provide a solution for large soft-tissue defects in lower extremities especially with absence of any suitable recipient vessels or when the use of vein grafts would not be feasible. However, ideal time before dividing the cross vascular pedicle should be identified to have the maximum success rate possible.

The suprascapular nerve (SSN) is commonly reconstructed by spinal accessory nerve (SAN) transfer. However, reinnervation of its branch to the infraspinatus muscle (IB-SSN) is poor. Reconstruction of the SSN in cases of scapular fractures is frequently neglected in clinical practice. The morphological study was performed on 25 adult human cadavers. The course and the length of SSN of minimal diameter of 2 mm within the trapezius muscle, the length of the distal stump of IB-SSN to its branching point and the length of the SSN available for reconstructive procedure were measured. The feasibility study of the SAN - IB-SSN neurotization performed by using a bony canal under the spine of scapula was performed. The mean distance of the SAN from the spine was 8.5 cm (±0.88) at the point where it perforates the trapezius muscle and 4.49 cm (±0.72) at the most distal part of the nerve. The mean length of the intramuscular portion of the nerve was 14.74 cm (±1.99). It ran under a mean latero-medial angle of 15.54° (±2.51). The mean distance between the medial end of the scapular spine and the SAN was 2.44 cm (± 0.64). The mean length of the IB-SSN was 3.6 cm (± 0.67). The mean length of the SAN stump which was mobilized from its original course and transferred to the infraspinous fossa to reach distal stump of the IB-SSN was 8.09 cm (±1.6). Direct SAN to IB-SSN transfer is anatomically feasible in the adult population.

OBJECTIVE: The extreme lateral supracerebellar infratentorial (ELSI) approach has the potential to access several distinct anatomical regions that are otherwise difficult to reach. We have illustrated the surgical anatomy through cadaveric dissections and provided an extensive review of the literature to highlight the versatility of this approach, its limits, and comparisons with alternative approaches. METHODS: The surgical anatomy of the ELSI has been described using 1 adult-injected cadaveric head. Formalized noninjected brain specimens were also dissected to describe the brain parenchymal anatomy of the region. An extensive review of the literature was performed according to each targeted anatomical region. Illustrative cases are also presented. RESULTS: The ELSI approach allows for wide exposure of the middle and posterolateral incisural spaces with direct access to centrally located intra-axial structures such as the splenium, pulvinar, brainstem, and mesial temporal lobe. In addition, for skull base extra-axial tumors such as petroclival meningiomas, the ELSI approach represents a rapid and adequate method of access without the use of extensive skull base approaches. CONCLUSIONS: The ELSI approach represents one of the most versatile approaches with respect to its ability to address several anatomical regions centered at the posterior and middle incisural spaces. For intra-axial pathologies, the approach allows for access to the central core of the brain with several advantages compared with alternate approaches that frequently involve significant brain retraction and cortical incisions. In specific cases of skull base lesions, the ELSI approach is an elegant alternative to traditionally used skull base approaches, thereby avoiding approach-related morbidity.

Bolesti dolní části zad (low back pain; LBP) jsou mimořádně častým a závažným zdravotním problémem. Paraspinální svaly hrají důležitou funkci při pohybech trupu, přispívají k udržení postury a stability páteře. U pacientů s chronickými LBP dochází ke strukturálním i funkčním změnám paraspinálních svalů. Makroskopické změny lze vyšetřit na CT a zejména na MR, mikroskopické změny pomocí svalové biopsie. V dostupné literatuře existují důkazy, že atrofie paraspinálních svalů a zvýšení podílu tukové tkáně uvnitř paraspinálních svalů jsou významně asociovány s nespecifickými LBP, a to zejména s chronickými bolestmi. Z diferenciálně diagnostického hlediska je nutné změny v paraspinálních svalech asociované s LBP odlišit od postižení paraspinálních svalů u pacientů s axiálními myopatiemi, které jsou však vzácné. Cílem tohoto sdělení je upozornit na strukturální i funkční změny paraspinálních svalů u pacientů s chronickými nespecifickými LBP. Je uveden diagnostický postup při chronických LBP spojených s degenerací paraspinálních svalů k vyloučení přítomnosti axiální myopatie.
Low back pain (LBP) is a frequent and major health problem. Paraspinal muscles play an important role in trunk movements, helping to maintain posture and spinal stability. Changes of paraspinal muscle morphology and function are described in patients with chronic LBP. Macroscopic changes can be evaluated by CT and especially by MRI, while microscopic changes are assed by muscle biopsy. There is evidence available in literature stating that atrophy of paraspinal muscles and increased fat deposits in the paraspinal muscles are significantly associated with non-specific LBP, especially with chronic LBP. From a ferential diagnostic point of view, changes in paraspinal muscles associated with LBP must be distinguished from paraspinal muscle involvement in patients with axial myopathies, but these are rare. The aim of this paper is to highlight the structural and functional changes of paraspinal muscles in patients with chronic non-specific LBP. A diagnostic algorithm for chronic LBP associated with degeneration of paraspinal muscles to exclude the presence of axial myopathy is presented.
