Léky obsahující jak bylinný extrakt, tak syntetickou látku jsou velmi žádané kvůli jejich pozitivním účinkům a synergismu. Z hlediska farmakologické aktivity se nová kombinace salbutamol-sulfátu a extraktů Hedera helix jeví jako perspektivní. Pro zajištění kvality je však nutné určit a kvantifikovat nečistoty syntetické složky. Rostlinné extrakty se skládají z různých fytochemických složek, proto je složitější vyvinout selektivní analytickou metodu. Z důvodu prokázání bezpečnosti a účinnosti lékové formy byla vyvinuta a validována nová metoda HPLC pro analýzu nečistot salbutamol-sulfátu. Tato metoda byla použita k odhadu bezpečnosti nového sirupu provedením dlouhodobých stabilitních testů po dobu 24 měsíců. Získané výsledky naznačily absenci jak ve významném snížení obsahu hlavních složek, tak ve zvyšení hladiny látek příbuzných. Pro předpověď možnosti tvorby nečistot byla rovněž provedena tzv. síla degradace.
Medicines containing both a herbal extract and a synthetic substance are in high demand due to their beneficial effects and synergism. The novel combination of salbutamol sulfate and Hedera helix extracts seems to be prospective in terms of pharmacological activity. But for quality assurance, impurities of the synthetic component have to be determined and quantified. Plant extracts consist of various phytochemical components, therefore, it is more complicated to develop a selective analytical method due to the sample matrix. To prove the safety and efficacy of the dosage form, a new HPLC method for analysis of salbutamol sulfate impurities was developed and validated. The method was used to estimate the safety of the novel syrup by performing long-term stability studies for 24 months. Obtained results indicated the absence in both significant reducing of the main components content and increasing of related substances level. Also, force degradation was carried out to prognosticate the possibility of impurities producing.

Popularity of natural-based preparations supporting the sexual potency significantly increased in recent years, which also led to the increase of illegal use of phosphodiesterase type 5 inhibitor (PDE-5) in sexual performance enhancement products. In this study, a rapid U-HPLC‒HRMS/MS method has been developed to simultaneously determine 59 PDE-5 inhibitors and their analogues. Within the development of sensitive method for analysis of 59 PDE-5 inhibitors and their analogues, both sample preparation procedure, as well as separation / detection conditions have been optimized. Extraction efficiency of particular extraction solvents, influence of different mobile phase additives on target analytes separation, as well as impact of various settings of mass analyzer on sensitivity of detection were examined. Data were collected in the 'full MS/data dependent MS/MS' acquisition mode (full MS-dd-MS/MS). Before the U-HPLC‒HRMS/MS method was used for analysis of real samples, proper validation had been conducted. The precision of the method expressed as the relative standard deviation (RSD) was ≤4.2% and ≤5.2% at spiking concentrations 5 μg/g and 0.25 μg/g, respectively. The limits of quantification were in the range 0.25 - 0.05 μg/g and the recovery ranged between 71 and 90%. The optimized method was successfully applied for analysis of 64 real samples, and 10 of them were proved to contain both registered or unregistered synthetic PDE-5 inhibitors. Additionally, the acquired U-HPLC‒HRMS/MS fingerprints were demonstrated to serve as an efficient tool for revealing of other type of possible fraud in products labeling. Retrospective mining of markers of herbs declared on dietary supplements packaging allowed to assess the trueness / untruth in the declaration of medical herbs composition.

A rapid procedure for the determination of 2-aminoisobutyric acid in enzalutamide bulk drug substance based on hydrophilic interaction chromatography with fluorescence detection was developed. Fluorescence detection after postcolumn derivatization with o-phthaldialdehyde/2-mercaptoethanol was carried out at excitation and emission wavelength of 345 nm and 450 nm, respectively. The postcolumn reaction conditions such as reaction temperature, mobile phase and derivatization reagent flow rate and the reagents concentrations were studied and optimized due to steric hindrance of amino group of 2-aminoisobutyric acid. The derivatization reaction was applied for the hydrophilic interaction chromatography method which was based on COSMOSIL HILIC column with a mobile phase consisting of a mixture of 25 mmol/L acetic acid adjusted to pH 5.5 (using 1 mol/L potassium hydroxide) and acetonitrile using an isocratic elution (28:72, ν/ν). The benefit of the reported approach consists in a simple sample pretreatment and a quick and sensitive hydrophilic interaction chromatography method. The developed method was validated in terms of linearity, limit of detection, limit of quantification, accuracy, precision and selectivity according to the International Conference on Harmonisation guidelines. The developed method was demonstrated to be applied for the analysis of 2-AIBA in routine quality control evaluation of commercial samples of enzalutamide bulk drug substance.

An HPLC method with UV and electrospray ionization - mass spectrometry (ESI-MS) detection was developed for the separation and determination of obeticholic acid (OBE) and its related compounds in development process and quality control. OBE and its related compounds were classified into three major group based on the mass spectra profiles: (A) those containing a hydroxyl group at position 3 and 7, (B) those containing a hydroxyl group and/or carbonyl group at position 3, hydrogen, ethyl or ethylidene group at position 6 and a hydroxyl group and/or carbonyl group at position 7, and (C) those containing carbonyl groups at position 3 and 7. ESI-MS ionization of OBE and its related compounds often produced intense adduct ions [M+H+98]+ and/or [M+H+196]+ that were identified as the adduct ions of phosphoric acid ([M+H+H3PO4]+ and [M+H+2H3PO4]+) originating from the mobile phase. The separation on HPLC system was accomplished using stationary phase based on XSelect CSH C18 (3.0×150mm×2.5μm) and a linear gradient elution using acetonitrile and 0.05% of o-phosphoric acid. The condition of chromatographic system was set as follows: flow rate 0.7mL/min, temperature 45°C and UV detection at 192nm. The separation of the 19 compounds was finished in less than 18min (including equilibration time). The HPLC/UV method was partially validated according to International Council for Harmonisation of Technical Requirements for Pharmaceuticals for Human Use (ICH) guidelines in terms of repeatability, selectivity, linearity and limit of quantification and detection.

Supercritical Fluid Chromatography (SFC) has known a strong regain of interest for the last 10 years, especially in the field of pharmaceutical analysis. Besides the development and validation of the SFC method in one individual laboratory, it is also important to demonstrate its applicability and transferability to various laboratories around the world. Therefore, an inter-laboratory study was conducted and published for the first time in SFC, to assess method reproducibility, and evaluate whether this chromatographic technique could become a reference method for quality control (QC) laboratories. This study involved 19 participating laboratories from 4 continents and 9 different countries. It included 5 academic groups, 3 demonstration laboratories at analytical instrument companies, 10 pharmaceutical companies and 1 food company. In the initial analysis of the study results, consistencies within- and between-laboratories were deeply examined. In the subsequent analysis, the method reproducibility was estimated taking into account variances in replicates, between-days and between-laboratories. The results obtained were compared with the literature values for liquid chromatography (LC) in the context of impurities determination. Repeatability and reproducibility variances were found to be similar or better than those described for LC methods, and highlighted the adequacy of the SFC method for QC analyses. The results demonstrated the excellent and robust quantitative performance of SFC. Consequently, this complementary technique is recognized on equal merit to other chromatographic techniques.
Cíl studie: Pro svůj velmi rychlý nástup účinku je 2% lidokain v případě akutního císařského řezu u rodičky s již zavedeným epidurálním katétrem pro předchozí porodní analgezii lokálním anestetikem volby. V ČR je však 2% lidokain k dispozici pouze v 2ml ampulích, v emergentní situaci tak doba potřebná k přípravě epidurálního bolusu může být bariérou jeho použití. Předem připravená dávka lidokainu by v této situaci významně zkrátila dobu od rozhodnutí do aplikace, a tím i celkovou dobu nástupu účinku epidurální anestezie. Proti tomuto postupu ale většinou stojí obecné místní předpisy poukazující na riziko bakteriální kontaminace předem takto připravených léčiv. Naše studie měla proto za cíl zhodnotit sterilitu a bezpečnost lidokainu předem nataženého v injekční stříkačce a připraveného k podání na porodním sále. Typ studie: Observační, prospektivní. Typ pracoviště: Operační porodní sál fakultní nemocnice. Materiál a metoda: Celkem 59 vzorků 2% lidokainu, sterilně natažených z 2ml ampulí do 20ml injekčních stříkaček a opatřených sterilní zátkou, bylo 48 hodin ponecháno na anesteziologickém stolku a poté testováno na sterilitu. Výsledky: V žádné z 59 naplněných 20ml stříkaček s 2% lidokainem nebyl zaznamenán mikrobiální nárůst ani růst mikromycet. Všechny vzorky byly vyhodnoceny jako vyhovující zkoušce na sterilitu. Závěr: Lokální anestetika mají vlastní antimikrobiální účinky proti širokému spektru patogenů, sterilně natažený lidokain v injekční stříkačce zůstává proto bezpečný k okamžitému podání po dobu minimálně 48 hodin. Předpřipravení lidokainu pro potřebu top-up epidurální anestezie k urgentnímu císařskému řezu je nejen racionální, ale z hlediska sterility i zcela bezpečné.
Objective: Due to its fast onset of action, 2% lidocaine is our local anaesthetic of choice for topping-up an already established epidural catheter in case of urgent Caesarean Section. Unfortunately, 2% lidocain is only available in 2-ml ampoules in the Czech Republic, and the time needed for drawing up the epidural bolus can be a barrier for its use in emergency. Pre-filled syringes of lidocaine would significantly shorten the time from decision to administration in this situation as well as the overall time of onset of epidural anaesthesia. The general local protocols are mostly against this method, pointing to a risk of bacterial contamination of pre-drawn drugs. Our study had the objective to evaluate the sterility and safety of the pre-filled lidocaine syringe, ready for immediate use in the obstetric theatre. Design: Observational, prospective study. Setting: Obstetric operating theatre. Materials and methods: In total, 59 samples of lidocaine 2%, which were sterilely drawn up from 2-ml ampoules to a 20-ml syringe and closed with a sterile cap, were left on the anaesthetic trolley for 48 hours, and then tested for sterility. Results: Microbial or fungal growth was found in none of the 59 pre-filled syringes of 2% lidocaine. All the samples were assessed as suitable from the sterility point of view. Conclusion: Local anaesthetics have their own antimicrobial activity against a broad spectrum of pathogens. A sterilely pre-filled lidocaine syringe remains safe for use for at least 48 hours. Pre-filling lidocaine for epidural top-up of anaesthesia for the Caesarean Section is rational and safe in terms of sterility.

Propofol je intravenózní celkové anestetikum na bázi lipidové emulze bez antimikrobiální přísady s vysokým rizikem mikrobiálního růstu. Nedodržení doporučeného postupu podání léku dle SPC (Summary of Product Characteristics) bylo spojeno se vznikem závažných infekčních komplikací. Cílem článku je rozbor etických a právních problémů, které mohou být s nesprávnou praxí používání propofolu spojeny.
Propofol is an intravenous anaesthetic agent based on lipid emulsion without antimicrobial additive with high risk of microbial growth. Non-compliance with the recommended SPC (Summary of Product Characteristics) administration was associated with serious infectious complications. The aim of the article is to analyze the ethical and legal problems that can be associated with incorrect practice of using propofol.
