Sarcoidosis is a disease characterised primarily by lung tissue involvement. Extrapulmonary involvement, particularly in the genitourinary tract, is extremely rare, particularly when it comes to primary disease detection in this location. The gold standard in establishing a definitive diagnosis of sarcoidosis is a combination of the clinical picture, the results of imaging methods, and histopathological examination from the biopsy taken (thus ruling out other causes of granulomatous inflammation). However, it is common for the biopsy to be infeasible or for the patient to refuse such an examination, resulting in the neglect of this critical verification. We introduce the case of a young 29-year-old man of Czech nationality who had been complaining for some time about non-specific pain above the pubic bone and in the lower abdomen, which was combined with a painless enlargement of the right half of the scrotum. Due to suspected malignancy, it was, after considering clinical, imaging, and laboratory findings, decided to perform a radical orchiectomy as a treatment option. The histological examination revealed that it was not cancer, but rather a rare genitourinary form of extrapulmonary sarcoidosis. In this case, radical resection had been, therefore, unnecessary. We also present a review of the literature on published extrapulmonary, genitourinary, and testicular sarcoidosis cases. All the above demonstrates the importance of considering a possible atypical sarcoidosis manifestation and histological confirmation before pursuing radical solutions.
AIMS: Formal depiction of granulomatous inflammation associated with renal neoplasms has mainly consisted of case reports. Herein, we investigate the clinicopathological features and potential significance of granulomas associated with renal tumours from a large multi-institutional cohort. METHODS AND RESULTS: One hundred and eleven study cases were collected from 22 institutions, including 57 partial nephrectomies and 54 radical nephrectomies. Patient ages ranged from 27 to 85 years (average = 60.1 years; male = 61%). Renal neoplasms included clear cell renal cell carcinoma (RCC; 86%), papillary RCC (8%), chromophobe RCC (3%), clear cell papillary RCC (1%), mixed epithelial stromal tumour (1%) and oncocytoma (1%). Granulomas were peritumoral in 36%, intratumoral in 24% and both in 40% of cases. Total granuloma count per case ranged from one to 300 (median = 15) with sizes ranging from 0.15 to 15 mm (mean = 1.9 mm). Necrotising granulomas were seen in 14% of cases. Histochemical stains for organisms were performed on 45% of cases (all negative). Sixteen cases (14%) had a prior biopsy/procedure performed, and eight patients had neoadjuvant immunotherapy or chemotherapy. Eleven patients (10%) had a confirmed diagnosis of sarcoidosis, including five in whom sarcoidosis was diagnosed after nephrectomy. CONCLUSION: Based on this largest case-series to date, peri-/intratumoral granulomas associated with renal neoplasms may be more common than initially perceived. The extent of granulomatous inflammation can vary widely and may or may not have necrosis with possible aetiologies, including prior procedure or immunotherapy/chemotherapy. Although a clinical association with sarcoidosis is infrequent it can still occur, and the presence of granulomas warrants mention in pathology reports.

Sarkoidóza je systémové onemocnění neznámé etiologie, které je charakterizováno tvorbou nekaseifikujících granulomů. Nejčastěji postihuje nitrohrudní uzliny a plicní tkáň, ale může se projevit téměř ve všech orgánech. Asi u třetiny nemocných sarkoidóza probíhá asymptomaticky a spontánní zhojení plicní sarkoidózy lze očekávat v 50-60 %. Pokud ale ke spontánní remisi nedochází, pacient je symptomatický a dochází k poklesu plicních funkcí, je indikována léčba. Od 50. let 20. století jsou stále lékem první linie kortikoidy. Ve druhé linii se uplatňují další imunosupresiva, hlavně methotrexát, a ve třetí linii biologická léčba. Cílem léčby je zabránit přechodu onemocnění do vyšších stadií, snížit nemocnost, zachovat a/nebo zlepšit kvalitu života nemocného.
Sarcoidosis is a multisystem inflammatory disease of unknown etiology. It is characterized by the presence of noncaseating granulomas. The lungs and lymphatic system are most often affected, but sarcoidosis may affect any organ. Almost one third of patients with pulmonary sarcoidosis are asymptomatic and spontaneous remission occurs in 50-60% of patients. The indications for the treatment of sarcoidosis are pulmonary symptoms and reduced lung function. Since the beginning of the 1950s, corticosteroids have been used in the first line of treatment of sarcoidosis. The second-line treatment is other immunosuppressive medication mainly methotrexate. Biologicals have gained a place as third-line therapy in sarcoidosis. The goals of sarcoidosis management are to prevent or control organ damage, relieve symptoms, and improve the patient's quality of life.

INTRODUCTION: Sarcoidosis is a systemic granulomatous disease affecting in particular the respiratory tract. Cardiac magnetic resonance (CMR), including a measurement of T1 relaxation time, could potentially detect early stadia of sarcoidosis of the heart. The study aims to assess T1 mapping in the detection of early cardiac involvement in asymptomatic patients with sarcoidosis. METHODS: One hundred and twenty patients with extracardiac sarcoidosis and without any heart disease history were included. One hundred and thirteen of them underwent a CMR examination. The mean time from the diagnosis of sarcoidosis was 0.8 (0.2-3.3) years. Cine images for the assessment of left ventricular (LV) functional parameters and pre- and post-contrast saturation method using adaptive recovery times for cardiac T1 mapping (SMART1 Map) and modified Look-Locker inversion recovery (MOLLI) images were acquired for the assessment of native T1 relaxation time and extracellular volume (ECV). The measured parameters were compared between sarcoidosis patients and 22 controls. RESULTS: The sarcoidosis patients had normal global and regional systolic LV function-LV ejection fraction 65 ± 5% versus 66 ± 7% (p NS). The mean native T1 relaxation times were not prolonged-1465 ± 93 ms versus 1480 ± 88 ms (p NS) measured by SMART1 Map and 1317 ± 60 ms versus 1313 ± 83 ms (p NS) measured using a MOLLI sequence. Similarly, the mean ECV values did not increase-16.9 ± 3.9% versus 17.9 ± 3.7% (p NS) measured by SMART1 Map and 30.9 ± 2.9% versus 31.6 ± 8.3% (p NS) measured using a MOLLI sequence. CONCLUSION: Myocardial native T1 relaxation times were not prolonged and ECV was not increased in asymptomatic patients with extracardiac sarcoidosis.

Sarkoidóza je multisystémové granulomatózní onemocnění neznámé etiologie, které se nejčastěji projevuje nitrohrudním postižením s bilaterální hilovou lymfadenopatií a/nebo plicními infiltráty. Z mimoplicních postižení jsou nejběžnější oční a kožní léze, mohou být také postižena játra, slezina, lymfatické uzliny, slinné žlázy, srdce, nervový systém, svaly a kosti, vzácněji i jiné orgány. Muskuloskeletální projevy se manifestují až u 1/3 pacientů a jejich spektrum je široké od prostých artralgií až po lytické kostní léze. Zánětlivé léze šlach a periartikulární otok jsou častější než kloubní synovitida. Navzdory současným poznatkům o patogenezi onemocnění je diagnóza sarkoidózy pohybového systému obtížná. Definitivní diagnóza bývá mnohdy stanovena až histologickým nálezem sarkoidního granulomu ve tkáních v kombinaci s klinickými projevy. Postižení svalové a kosterní soustavy se objevuje obvykle již v časném průběhu onemocnění. Nové zobrazovací metody především PET/CT s použitím fluorodeoxyglukózy jsou vhodné pro posouzení rozsahu onemocnění a stanovení aktivity nemoci. Glukokortikoidy jsou lékem první volby v léčbě systémových projevů sarkoidózy, methotrexát či jiné chorobu modifikující léky lze použít jako kortikoidy šetřící medikaci. Biologická léčba především inhibitory TNF-α mají své místo u onemocnění refrakterního k běžné léčbě, ale důkazy pro jejich účinnost nejsou jednoznačné.
Sarcoidosis is a multisystem granulomatous disease of unknown etiology, most commonly manifested by intrathoracic involvement with bilateral hilar lymphadenopathy and / or pulmonary infiltrates. Ophthalmic and skin lesions are the most common extrapulmonary manifestations, liver, spleen, lymph nodes, salivary glands, heart, nervous system, muscles and bones, rarely other organs, may also be affected. Musculoskeletal manifestations is present in up to 1/3 of patients and their spectrum is broad from simple arthralgia to lytic bone lesion. Inflammatory tendon lesions and periarticular edema are more common than joint synovitis. Despite current knowledge of the pathogenesis of the disease, the diagnosis of sarcoidosis of the musculoskeletal system is difficult. The definitive diagnosis is often determined by histological finding of sarcoid granuloma in tissues in combination with clinical manifestations. Musculoskeletal involvement usually occurs early in the disease. New imaging methods, especially PET/CT using fluorodeoxyglucose, are useful for assessing the extent of disease and determining disease activity. Glucocorticoids are the first-line treatment in the management of systemic symptoms of sarcoidosis, methotrexate or other disease-modifying drugs can be used as corticosteroid-sparing medications. Biological therapy, in particular TNF alpha inhibitors, has a place in the treatment of refractory disease, but the evidence for its efficacy is not clear.

Práce se zabývá současnými názory na etiopatogenezi sarkoidózy, uvádí moderní diagnostické postupy a doporučenou léčbu. Jde o nemoc se stále nejasnou etiologií, charakterizovanou tvorbou nekaseifikujících granulomů, které jsou tvořeny především T lymfocyty a různě modifikovanými makrofágy. Zdůrazňuje se význam Th17+ buněk, které se akumulují v místě zánětu. Předpokládá se působení neznámé noxy u predisponovaného jedince. Výskyt nemoci stoupá a zvyšuje se udávaná mortalita. Klinické projevy mohou být neobvykle pestré, ale asi třetina případů je asymptomatická. Iniciační proces začíná v plicích, kde dochází k alveolitidě, plíce jsou postiženy v 95 % případů. Nemoc má systémový charakter a může se projevit téměř ve všech orgánech a tkáních. Závažné bývá postižení očí, centrální nervové soustavy a srdce. Diagnostika se opírá o soubor klinických projevů, radiologických změn (výpočetní tomografie s vysokým rozlišením), funkčního vyšetření a nálezu granulomu. Biopsie se provádějí většinou bronchoskopicky. K diagnóze může přispět bronchoalveolární laváž a vyšetření sérových biomarkerů. Až v polovině případů dochází ke spontánní remisi, po podání kortikosteroidů projevy nemoci většinou ustupují, ale existují perzistující i progredující formy nemoci, které nemocné ohrožují na životě. U refrakterních stavů se podávají v II. linii imunosupresiva (metotrexát, azatioprin, leflunomid a mykofenolát mofetil). Někdy je potřeba aplikovat léky III. linie, kterými jsou inhibitory TNFα (infliximab, adalimumab). Indikaci léčby je vždy třeba zvážit z pohledu zlepšení orgánových funkcí a kvality života při možných vedlejších účincích.
The papers deals with the current opinions on etiopathogenesis of sarcoidosis, presents modern diagnostic procedures and recommended treatment. The etiology of the disease is still unclear, characterized by the formation of non-caseating granulomas, consisting mainly of T lymphocytes and variously modified macrophages. The importance of Th17+ cells that accumulate at the site of inflammation is emphasized. Involvement of an unknown noxa in a predisposed individual is expected. The incidence of the disease is increasing, as well as the reported mortality. Clinical manifestations may vary greatly; however, about a third of cases are asymptomatic. The initiation process begins in the lungs where alveolitis occurs; the lungs are affected in 95% of cases. The disease is systemic in nature and may affect almost all organs and tissues. Manifestations in the eyes, central nervous system and heart are usually severe. Diagnosis is based on a set of clinical manifestations, radiological changes (high-resolution computed tomography), functional examination, and identification of granuloma. Biopsy is usually performed bronchoscopically. Bronchoalveolar lavage and serum biomarker testing may contribute to the diagnosis. Spontaneous remission occurs in up to half of cases. Administration of corticosteroids usually leads to symptom resolution; however, there are persistent and progressive forms of the disease that threaten patients lives. In cases of refractory disease, immunosuppressive agents (methotrexate, azathioprine, leflunomide, and mycophenolate mofetil) may be administred in second line therapy. Furthermore, TNFa; inhibitors (infliximab, adalimumab) may be administered in third line therapy, if needed. Improvement of organ function and quality of life should be weighed against the possible side effects when considering the therapy indication.
