BACKGROUND: Food insecurity is one of the social determinants of health and affects dietary quality and well-being. This study aimed to examine the associations among food insecurity, sociodemographic and economic factors, and health-diet characteristics, with a particular focus on fresh fruit and vegetable (FV) consumption during the late COVID-19 pandemic in Czechia. METHODS: Data from a cross-sectional survey, 'Living through the Pandemic', collected in October 2022 as part of a Czech longitudinal survey, were analysed. The study included a representative sample of Czech adults (N = 1,499, aged 20 years and above). Binary logistic regression was performed to assess associations among food insecurity, sociodemographic-economic factors and health-diet factors. Food insecurity was assessed as experiencing or worrying about a lack of food. Sociodemographic-economic factors included sex, age, education, income, number of children and home food production. Health-diet factors included BMI, limited mobility and daily fresh FV intake, defined as eating fresh FV at least once per day. Determinants of daily fresh FV intake were analysed separately. RESULTS: Over 30% of respondents (N = 486) were at risk of food insecurity. Individuals aged 20-34 years, those with lower educational attainment, and those with limited mobility were more likely to report food insecurity. Compared with the high-income category, individuals in the lowest income category had a sevenfold higher likelihood of reporting food insecurity. Food-insecure individuals had approximately twofold higher odds of not having fresh FV daily. The odds of not having fresh FV were particularly higher among younger adults (20-34 years) and males. Individuals with BMI ≥ 25 kg/m2 had 30% higher odds of not having daily FV, with marginal significance (p = 0.05). Educational attainment, rather than income, was a key predictor of FV consumption. Home food production contributed to better food security and higher FV consumption. CONCLUSION: In Czechia food insecurity and the limited FV intake relate to younger adults, socioeconomically disadvantaged individuals, and those with limited mobility. Lower education attainment, rather than income, predicts limited consumption of FV, underscoring the long-term impact of early education on healthy eating. Given the high prevalence of overweight and obesity, inadequate FV intake presents a public health concern. Policies should aim to improve access to affordable and nutritious foods, and strengthen education on healthy eating habits to mitigate long-term health disparities.

OBJECTIVES: Many studies draw attention to the negative consequences of the pandemic or lockdown on the well-being and lifestyle of different sections of the population. This study considers whether changes occurred in dietary regime and level of physical activity during three periods - before the pandemic, during the lockdown, and during the present in older Slovak adults. We also investigate whether individual weights changed during the pandemic. METHODS: The study has a cross-sectional design. We collected 436 questionnaires from older adults (60+) living in Slovakia; 277 come from the first phase of online data collection (04/2022-06/2022) in university students of the third age, and 159 from the second phase of offline collection (07/2022-11/2022) in social service facilities. We monitored if there were any changes within the observed periods in the variables of diet regime, physical activity, and BMI. The database has been processed in IBM SPSS Statistics. RESULTS: During the lockdown, the frequency of physical activity decreased (p < 0.001), the quality of movement worsened (p < 0.001), and the time respondents devoted to physical activity during the week decreased (p < 0.001). At present, the variables have increased. Within the dietary regimen, statistically significant changes occurred in appetite and non-alcoholic beverage intake (p = 0.002). However, there were no significant changes in the number of daily meals (p = 0.190), or the amount of alcohol consumed (p = 0.066). The BMI of older men and women did not change after the pandemic (p = 0.519), and it did not differ between men and women either before the pandemic (p = 0.281) or after it (p = 0.449). Also there is no difference between UTA students and social facility living seniors before the pandemic (0.933) and now (0.317). The results showed that even though physical activity significantly decreased during the lockdown, the older adults in our study did not eat more during the lockdown, nor did they gain weight. However, this probably had negative consequences for the quality of their movement. CONCLUSION: It is essential to simultaneously create compensatory measures while implementing future anti-epidemic (or other) measures. They would help maintain the desired amount of physical activity or other healthy habits on an optimal level.

OBJECTIVES: This study aimed to determine trend in polyphenol consumption in the Czech Republic during the last three decades. Additionally, it provides a brief overview of the beneficial effects of polyphenols in several body systems. METHODS: Data from the Phenol-Explorer 3.6, a specialized database of polyphenolic substances, were assigned to the resources of the Czech Statistical Office on the consumption of food and beverages in the Czech Republic for the years 1989-2022. The average daily intake of polyphenols was determined by multiplying the average annual consumption of each type of food by the polyphenol content obtained from the database; results were given in milligrams of polyphenols per inhabitant and day. Since the food items in the data sources are not identical, it was necessary to create an extensive model of food categories. RESULTS: The current value of polyphenol intake is 1,673 mg per day per inhabitant; however, this level most likely reflects methodological underestimation. The favourable increase in dietary polyphenol intake in the Czech population - doubling, to be precise - which we observed from 1989 to 2007, has been replaced by the opposite trend in the last 15 years. The current intake of polyphenols corresponds to the level that was already achieved in 2004. Hydroxycinnamic acids (from the group of phenolic acids) are the most prevalent dietary polyphenols, followed by flavanols (from the group of flavonoids). The most frequent source of polyphenols in the Czech population are non-alcoholic beverages such as coffee, tea and juices, followed by fruits, cereals, and vegetables, respectively. CONCLUSION: Current trend of dietary polyphenol intake in the Czech population is slightly decreasing. This tendency, lasting since 2008, is indisputably negative. Plant polyphenols offer opportunities for inexpensive interventions in health promotion.

Fatty acid (FA) profiles as potentially relevant components of Crohn's disease (CD) have been insufficiently analysed. We sought to explore the plasma profiles of n-3 and n-6 polyunsa-turated fatty acids (PUFAs) in newly diagnosed untreated active CD. We included 26 consecutive CD pediatric patients (<19 years) and 14 healthy controls (HCs). Disease characteristics, including inflammatory markers, dietary histories, and the Pediatric Crohn's Disease Activity Index (PCDAI), were obtained. The profiles of plasma FAs in plasma lipid classes were analysed by gas chromatography with FID detection of methyl esters. The erythrocyte sedimentation rate, C-reactive protein level and fecal calprotectin level (all p<0.001) were significantly higher in CD patients than in HCs. Most changes were observed in plasma phospholipids (PLs), such as a higher content of n-3 and changes in n-6 long-chain PUFAs in the CD group. The CD group had a lower ratio of n-6/n-3 PUFAs in PLs (p<0.001) and triacylglycerols (TAGs) (p<0.01). Correlations of the FA content in plasma PLs with disease activity scores of CD were also observed, which were positive for the sum of monounsaturated fatty acids (MUFAs) as well as oleic acid (18:1n-9) (both p<0.05). The metabolism of PUFAs is significantly altered even in treatment-naive newly diagnosed active pediatric CD, and the content of major FAs in PLs correlates with disease activity and inflammatory markers, thus probably contributing to the still unclear early disease pathogenesis.

BACKGROUND: Early adulthood is a period of rapid personal development when individuals experience major life transitions (e.g. leaving the parental home, leaving education, beginning employment, cohabitation and parenthood). Changes in social and physical environments associated with these transitions may influence development of health-related behaviours. Consumption of fast food is one behaviour associated with poor diet and long-term health outcomes. In this study we assess how frequency of fast food consumption changes across early adulthood, and how major life transitions are associated with changes in fast food intake. METHODS: Data were collected across four waves of the Project EAT study, from mean age 14.9 (SD = 1.6) to mean age 31.1 (SD = 1.6) years. Participants reporting data at two or more waves were included (n = 2902). Participants reported past week frequency of eating food from a fast food restaurant and responded to questions on living arrangements, education and employment participation, and having children. To assess changes in fast food we developed a latent growth model incorporating an underlying trajectory of fast food intake, five life transitions, and time-invariant covariates. RESULTS: Mean fast food intake followed an underlying quadratic trajectory, increasing through adolescence to a maximum of 1.88 (SE 0.94) times/week and then decreasing again through early adulthood to 0.76 (SE 2.06) times/week at wave 4. Beginning full-time employment and becoming a parent both contributed to increases in fast food intake, each resulting in an average increase in weekly fast food intake of 0.16 (p < 0.01) times/week. Analysis of changes between pairs of waves revealed stronger associations for these two transitions between waves 1-2 (mean age 14.9-19.4 years) than seen in later waves. Leaving the parental home and beginning cohabitation were associated with decreases in fast food intake of - 0.17 (p = 0.004) and - 0.16 (p = 0.007) times/week respectively, while leaving full-time education was not associated with any change. CONCLUSIONS: The transitions of beginning full-time employment and becoming a parent were associated with increases in fast food intake. Public health policy or interventions designed to reduce fast food intake in young adults may benefit from particular focus on populations experiencing these transitions, to ameliorate their impact.

OBJECTIVES: Fruit and vegetable intake has been reported as one of the significant protective factors against the development of cardiovascular diseases (CVD). This study aimed to assess the possible preventive effect of fruit and vegetable consumption on developing CVD. METHODS: A total of 398 participants (205 cases and 193 controls) referred for elective coronary angiography with clinical suspicion of coronary artery disease to Prince Hamza Hospital in Amman were enrolled in this case-control study. Dietary data were collected separately from each patient using interview-based food frequency questionnaire. RESULTS: The findings of the present study revealed that a total consumption of 3 servings of vegetable per day decreased significantly the risk of CVD to about 54% (OR = 0.46, 95% CI: 0.22-0.97, p = 0.033). Consumption of banana was found to reduce the risk of CVD to about 44% and 62% when consuming 1-2 and 3-6 servings/week, respectively, with p-value for trend 0.004. For the vegetables, the consumption of grape leaves and stuffed vegetables in general was significantly associated with lower risk of CVD. Increasing cauliflower consumption of 1-2 servings per week decreased CVD risk to about 37% (OR = 0.63, 95% CI: 0.38-0.98). Consuming up to 3-6 servings per week of mixed vegetables (OR = 0.10, 95% CI: 0.01-0.83) and onion (OR = 0.42, 95% CI: 0.22-0.80) revealed an inverse association with CVD development. CONCLUSIONS: Adding to the present evidence, consumption of some fruits and vegetables could be considered as preventive factor against developing CVD. However, the association of consuming vegetables with preventing CVD was higher than the fruit consumption.

OBJECTIVE: The present research aimed to estimate macronutrients intake among adults in North Macedonia and to identify their predominant food sources. METHODS: Within this cross-sectional study dietary data were collected using repeated 24h recall interviews. Nationwide, adults aged 18+ were recruited. Dietary data, anthropometric measures and socio-demographic characteristics were available for 496 participants. Nutrient intake was analyzed using the Balkan Food Composition Database and Diet Assess and Plan platform. Macedonian dietary guidelines were used for estimation of inadequacy. RESULTS: Significant differences in macronutrients intake are noticed between age groups within certain socio-demographic factors. Total sugar intake was significantly higher among young adult females comparing with those older than 25 years (p = 0.049). Young urban females have significantly higher fats intake than older participants (p = 0.038). Higher total daily energy, proteins, fats and carbohydrates intake (p = 0.033, p = 0.043, p = 0.032, p = 0.042, respectively) was noticed among young urban males when compared to older ones. Only dietary fibre intake was higher among older urban males (p = 0.030). Univariate linear regression models showed that obese participants had significantly higher relative proteins intake comparing with those having BMI within recommended range (p = 0.024, β = 1.21). Relative carbohydrates intake was significantly lower among males (p = 0.018, β = -2.077) and among highly educated participants (p = 0.018, β = -4.304). Participants with tertiary education had higher relative fats intake (p = 0.012, β = 4.213). CONCLUSION: Macronutrients intake of adults should be improved. There is higher intake of dietary fats and need for an increase of complex carbohydrates intake, particularly dietary fibre. Findings of this survey should be used in shaping, fine-tuning and implementing food and nutrition policies that will stimulate healthier diets for prevention of diet related non-communicable diseases.

OBJECTIVES: The paper aims to identify the priorities for cardiovascular health promotion research in Central and Eastern Europe (CEE), the region with the highest cardiovascular diseases (CVD) burden in the world. METHODS: This narrative review covered peer-reviewed publications and online databases using a nonsystematic purposive approach. RESULTS: In despite of a steady decrease in CVD burden in the region, the East-West disparities are still significant. There is minimal continuity in the past and current CVD prevention efforts in the region. Many challenges still exist, including an opportunity gap in research funding, surveillance and population-based preventive interventions. A comprehensive approach focusing on multisectoral cooperation, quality and accessibility of healthcare and equity-oriented public policies and supported by well-designed epidemiologic studies is needed to overcome these challenges. CONCLUSION: The current level of effort is not adequate to address the magnitude of the CVD epidemic in CEE. It is imperative to strengthen the epidemiological base concerning cardiovascular health in the region, to foster surveillance and progress in implementation of CVD preventive strategies in the most affected populations of Europe.

OBJECTIVE: To evaluate the dietary diversity and the nutrient contribution of traditional foods (locally cultivated and wild) by conducting a food intake study in rural Ecuador. DESIGN: Repeated 24 h recalls over a 14 d interval and frequency of consumption served to simulate the usual diet by the Multiple Source Method. Data on missing visits (n 11) were imputed using multivariate imputation by chained equations. The intakes of three macro- and six micronutrients were reported. Nutrient Adequacy Ratios, Mean Adequacy Ratio (MAR), Dietary Species Richness (DSR) and Minimum Dietary Diversity for Women were used as measures of dietary quality. A linear quantile mixed model was used to investigate the association between DSR, local species, MAR, age, education and occupation. SETTING: Guasaganda, Cotopaxi (Ecuador). PARTICIPANTS: Rural, indigenous adult women, non-pregnant and not breast-feeding. RESULTS: The studied diet had MAR of 0·78. Consumption of traditional foods contributed 38·6 % of total energy intake. Daily requirements for protein, carbohydrates, Fe and vitamin C were reached. An extra level of consumption of local species was associated with an increase in median MAR for macronutrients of 0·033 (P < 0·001). On the other hand, an extra level of consumption of local species was associated with an increase in median MAR for micronutrients of 0·052 (P < 0·001). CONCLUSIONS: We found statistical evidence that traditional foods contribute to adequate intakes of macro- and micronutrients and dietary diversification in the studied population. Future public health interventions should promote the cultivation and consumption of traditional foods to increase the quality of the local diet.

OBJECTIVES: The Sahel/Savannah belt is a region where two sympatric human subsistence strategies-nomadic pastoralism and sedentary farming-have been coexisting for millennia. While earlier studies focused on estimating population differentiation and genetic structure of this ecologically remarkable region's inhabitants, less effort has been expended on understanding the morphological variation among local populations. MATERIALS AND METHODS: To fill this gap, we used geometric morphometrics to analyze the facial features of three groups of pastoralists and three groups of sedentary farmers belonging to three language families (Niger-Congo, Nilo-Saharan, and Afro-Asiatic) whose mitochondrial DNA sequences have been published previously. RESULTS: Our results show that pastoralists differ from farmers with several facial features. We also found that individuals who bear maternally inherited haplotypes of Eurasian ancestry do not significantly morphologically differ from individuals whose maternal ancestry is sub-Saharan. CONCLUSIONS: Our study follows up and builds upon population genetic and phylogeographic studies of Eurasian haplogroups in the Fulani pastoralists and sub-Saharan haplogroups in the Arab pastoralists, as well as studies on the spread of lactase persistence mutations and other genetic markers. Our results suggest that recent gene flows across the Sahel/Savannah belt were not strong enough to erase a genetic structure established by Paleolithic foragers and further shaped by the adoption of agropastoral food-producing strategies.
