Možnosti systémové terapie pokročilého a metastatického renálního karcinomu jsou oproti jiným nádorům relativně široké. Pro všechny klinické situace máme několik možností léčby. V 1. linii léčby můžeme použít kombinaci imunoterapie a tyrosinkinázového inhibitoru (TKI) nebo kombinaci dvou imunoterapeutik; monoterapii TKI použijeme v případě, že imunoterapie není vhodná. V krátkém přehledu jsou uvedeny možnosti systémové léčby, které jsou součástí aktuálních doporučení ESMO a Modré knihy České onkologické společnosti pro rok 2023. Více než u jiných diagnóz si musíme v běžné praxi dávat pozor na úhradové podmínky jednotlivých léčivých přípravků a jejich kombinací.

The options for systemic treatment of advanced and metastatic renal cell carcinoma are relatively broad, compared to other tumours. We have several treatment options for all clinical situations. In the first-line treatment, combination of immunotherapy and a tyrosine kinase inhibitor or combination of two immunotherapeutics can be used. If immunotherapy is not suitable, monotherapy with a tyrosine kinase inhibitor can be used. In a short overview, we will mention the treatment options that are part of the current ESMO recommendations and also the Blue Book of the Czech Oncological Society for the year 2023. More than with other diagnoses, we must pay attention to the reimbursement conditions of individual drugs and their combinations in routine practice.
