V tomto článku se budeme věnovat současnému využití imunoterapie v léčbě renálního karcinomu, zejména u metastazujícího onemocnění. Zabývá se jak adjuvantní, tak paliativní léčbou, popisuje účinnost pembrolizumabu ve studii KEYNOTE-564 a další kombinace léčebných přípravků, jako je nivolumab s ipilimumabem, avelumab s axitinibem, pembrolizumab s lenvatinibem a nivolumab s cabozantinibem. Článek rovněž nastiňuje možnosti „rechahenge" terapie a vyšší linie paliativní léčby, včetně nivolumabu po selhání cílené léčby. Závěrem zdůrazňuje význam komplexního přístupu v rozhodovacím procesu o léčbě.

In this article, we will focus on the current use of immunotherapy in the treatment of renal carcinoma, particularly in metastatic disease. It addresses both adjuvant and palliative treatment, describing the effectiveness of pembrolizumab in the KEYNOTE-564 study and other combinations of treatment approaches, such as nivolumab with ipilimumab, avelumab with axitinib, pembrolizumab with lenvatinib, and nivolumab with cabozantinib. The article also outlines the possibilities of "rechallenge" therapy and higher lines of palliative treatment, including nivolumab after the failure of targeted therapy. It concludes by emphasizing the importance of a comprehensive approach in the decision-making process about treatment.
