Corynebacteria, non-spore-forming, gram-positive, aerobic or facultative anaerobic, pleomorphic bacilli, are part of the normal skin, oropharyngeal, and intestinal flora in humans. However, this microorganism can rarely be associated with invasive infections such as bone and joint infections, bacteremia, endocarditis, meningitis, liver and spleen abscesses. We present a case of bacteremic arthritis of a native knee joint caused by non-toxigenic Corynebacterium diphtheriae in a patient with alcoholic liver cirrhosis. This case report emphasizes the importance of differential diagnosis and careful examination of immunocompromised patients and reviews the criteria for administration of C. diphtheriae antitoxin in case of invasive disease.

The authors report a case of complicated sepsis caused by nontoxigenic Corynebacterium diphtheriae biotype gravis in a young immunocompetent female with no predisposing factors. She acquired the infection from her boyfriend, an asymptomatic carrier of nontoxigenic strain. The primary site of infection and subsequent sepsis was not identified. The occurence and importance of carriage and invasive infections caused by nontoxigenic strains of Corynebacterium diphtheriae are discused.

Diphtheria morbidity in the Czech part of former Czechoslovakia showed a continuous downward trend between 1946 and 1974. Afterwards, sporadic cases of diphtheria were reported in some years. Compulsory vaccination against diphtheria was started in 1946 with a monovaccine, later replaced by bivaccine DiTe. Since 1958, newborns have been vaccinated with DiTePe vaccine. As many as 98% to 100% of the population of age groups under 50 years likely to have been vaccinated have antibody levels > 0.01 lU/ml. About 83% to 88% of the older age groups who represent a naturally immunized population have antibodies as well. This immune status excludes the possibility that diphtheria could spread massively if accidentally imported into the Czech Republic.

The objective of the immunological survey (IS) was in addition to assessment of the overall immunity of the Slovak population against diphtheria to test the effectiveness of vaccination and suitability of the used vaccination pattern and to assess the persistence of antibodies against diphtheria in more advanced age, and possibly the need of revaccination. The sera were tested by means of the neutralization test on cell cultures. The level of diphtheria antitoxin was assessed quantitatively. In the reaction standards were used--diphtheria toxin 79/1 1000 Lf, produced in the National Institute of Public Health and the Environment Bilthoven, Netherlands and the diphtheria antitoxin Diphtheria antitoxin equine British Standard 66/153, obtained from WHO International Laboratory for Biological Standards, Great Britain. The overall immunity assessed on a nationwide scale in Slovakia was adequate (88.2%). In the child population the level of antibodies correlated well with the administration of individual vaccinations against diphtheria. In 6- to 10-year old children the serum positivity was as high as 100%, in older children it did not drop below 95%. A more marked decline of serum positivity was found in the population above 40 years. The highest proportion of sensitive subjects was in the group of 50-59-year-old ones (45.2%). The antidiphtheritic immunity of the population under 50 years is very good. Inadequate immunity of adults above 50 years revealed the need of administering a booster dose against diphtheria which will have a positive impact on the antibody level of this age group of the population.

The adhesive properties of one Cor. parvum and two Cor. diphtheriae strains were investigated by means of the direct haemagglutination reaction (HAR). Using a specifically developed HAR procedure, it was proved beyond doubt that the studied Corynebacteria possess adhesive properties apparently associated with the presence of pili (fimbrias) on their surfaces. In addition, a correlation was found between the agglutinating activity of bacteria and the conditions of culture growth. Based on the reported results, it is suggested that there exists an intraspecies stability of adhesive properties and a possible link between corynebacterial adhesins and reticulostimulin.

Examinations of 297 sera for diphtheria antitoxin and 160 sera for tetanus antitoxin were carried out in 1981. All sera were obtained from the cord blood of mothers between 15 and 34 years of age. The mothers were divided into four age groups each of which was further subdivided into the primipara and multipara subgroups. The aim was to assess the age-specific variations in response to active immunization against diphtheria and tetanus. The protective level of diphtheria antitoxin (at least 0.01 I.U./ml) was recorded in the serum of 96.3% of examinees and the rates of seropositivity were found to fall with increasing age. The protective level of tetanus antitoxin (at least 0.1 I.U./ml) was found in the serum of 95.2% of mothers. The serologic response encountered in groups of older mothers was a clear-cut demonstration that the country-wide mass immunization against tetanus carried out between 1974 and 1975 was highly effective and fully justified. The variations in the diphtheria and tetanus antitoxin levels found in the primipara and multipara subgroups were not statistically significant.
