: Lymphatic spreading of the cervical cancer is the most significant negative prognostic factor of the disease with a direct impact on the curative (treatment) plan. There is no sutiable non-invasive examination method of the involvement of the lymphnodes. Lymphatic mapping with identification of sentinel nodes combined with ultrastaging histopathological and imunohistochemistry examination. Confirmed negativity of sentinel nodes excludes the metastatic involvement of the regional lymphnodes and permitsto decrease the radicality of the surgical treatment. The combination of investigative methods will increase the sensitivity and specificity of the sentinel node detection. The combination of ultrastaging and imunohistochemistry will increase the sensitivity of the metastatic involvement.

Lymfogenní šíření karcinomu hrdla děložního je nejzávažnějším negativním prognostickým faktorem nemoci, ovlivňující léčebný plán. Neexistuje vhodná neivazivní vyšetřovací metoda metastatického postižení lymfatických uzlin. Novou možností je lymfatické mapování s identifikací a ultrastagingovým histopatologickým a imunohistochemickým vyšetřením. Prokázána negativita sentinelových uzlin vylučuje metastatické postižení regionálních lymfatických uzlin a dovoluje snížit radikalitu chirurgické léčby. Pozitivita sentinelové uzliny prokazuje lymfogenní šíření a vhodnost primární radiační léčby. Kombinace vyšetřovacích metod detekce sentinelových uzlin zvýší senzitivitu a specificitu záchytu sentinelových uzlin. Kombinace ultrastagingového a imunohistochemického vyšetření zvýší senzitivitu průkazu metastatického postižení.
