Předmětem grantu je zvýšení operability dosavadními postupy neresekabilních JMKRK. Metodika řešení bude spočívat v předoperačním zhodnocení celkového stavu nemocného, jaterních funkcí, velikosti a rozsahu JMKRK (USG,CT s volumometrií, MRI - s aplikací kontrastní látky). Další postupem bude stanovení optimálního léčebného postupu spočívajícího v kombinaci PVE, downstaging chemoterapie,etapových resekcí jater, RFA. Peroperačně bude využívána duplexní sonografie s kontrastní látkou sloužící ke zpřesnění lokalizace a rozsahu JMKRK. Kritériem hodnocení úspěšnosti postupu bude morbidita a mortalita nemocných, jejich dlouhodobé přežívání a recidiva metastáz.; The subject of grant is increasing of resectability of colorectal carcinoma liver metastases. The methods of project will be in evaluation of general health status of patien t, his or her liver functions, the volume and number of liver metastases (USG,CTwith volumometry, MRI with contrast material application). The next step will be the statement of optimal therapeutic procedure - combination of portal vein embolisation, downstaging chemoteraphy, staged liver resections, radiofrequency ablation. We will use peroperative ultrasonography with contrast material intravenous application. The end point of research project will be the morbidity and mortality of patients, their long term survival and metastatic process recurrence.
