Autoři sdělují své zkušenosti se subarachnoidální anestezií 0,5% hyperbarickým bupivacainem s přídavkem morfinu a srovnávají ji s anestezií celkovou a se subarachnoidální anestezií samotným hyperbarickým bupivacainem. Tato metoda zajistila vynikající pooperační analgezii se statisticky prokázanou nižší potřebou analgetik v pooperačním období. Její nežádoucí účinky nejsou závažné a nejsou častější než po jiných typech anestezie. Finanční náklady na tento typ anestezie nepřevyšují náklady na anestezii celkovou. Autoři zdůrazňují nutnost použít morfin bez konzervačních a stabilizačních přísad a nezbytnost pooperačního intenzivního sledování a monitorace.

The authors present their experience with subarachnoideal anesthesia with 0,5% hyperbaric bupivacaine with morphine supplement, comparing it with general anesthesia and subarachnoideal anesthesia with hyperbaric bupivacaine only. This method resulted in excellent postoperative analgesia with statistically proven decreased need for analgesics in the postoperative period. The adverse effects are not severe and are not more frequent compared to other types of anesthesia. According to financial aspects, the expenditures associated with this type of anesthesia do not raise above expenditures on general anesthesia. Preservative-free morphine only can be used, and close postoperative observation and monitoring should be available.
