Ve sdělení jsou hodnoceny výsledky rekonstrukčních výkonů benigních striktur žlučovodů po cholecystektomii a resekci žaludku u 25 pacientů s důrazem na využití Hepp-Couinaudova přístupu bez transanastomotické drenáže. U 17 pacientů byla použita Heppova technika k založení široké a přesné muko-mukózní anastomózy side-to-side mezi levým hepatikem a Rouxovou kličkou. Pacienti byli sledováni 1-12 let (průměr 5.1 roku). Pooperační mortalita byla 4.1% (1 pacient). 88% operovaných je bez subjektivních obtíží. V daném souboru pacientů nebyla restenóza dosud zjištěna. Heppův přístup řeší problém ošetření iatrogenních striktur žlučových cest včetně hilových striktur bez potřeby transanastomotické drenáže.

The results of operative repair of benign postoperative strictures after cholecystectomy and partial gastrectomy with particular reference to the use of the Hepp technique for hilar strictures without the use of transanastomotic tubal stenting were analysed in 25 patients. The Hepp technique of wide, accurate mucosa to mucosa anastomosis between the left hepatic duct and jejunal Roux loop was used in 17 patients with hilar bile duct strictures. Patients have been observed of 1 to 12 years (median 5.1 years). Hospital mortality rate was 4.1% (1 patient). 88% of the patients have no subjective complaints. No recurrent strictures occured with use of the Hepp technique. The Hepp approach provides a safe durable and effective solution to the problem of iatrogenic strictures of the bile duct without necessity of transanastomotic tube stenting.
