Správná diagnostika, přesná indikace k operačnímu léčení, optimální timing i precizní provedení operace jsou rozhodující faktory pro bezchybný výsledek operačního léčení zlomenin patní kosti. Autoři se v tomto sdělení zabývají diagnostikou, indikacemi k operačnímu léčení. Prezentována operační technika, zhodnoceny výsledky. V období od II. 2004 do XII. 2008 bylo na Klinice úrazové chirurgie operováno celkem 89 pacientů se 100 zlomeninami. Operace je prováděna průměrně 8. den od úrazu. Používána je nejčastěji metoda otevřené repozice se stabilní osteosyntézou dlahou Synthes, extenzivním laterálním přístupem, v anemizaci, ATB cloně. Doléčení se děje formou aktivní rehabilitace, bez adjuvantní rigidní fixace. Dosavadní výsledky - minimum závažných komplikací, dobré klinické i RTG nálezy jsou očekávaným a správným výsledkem činnosti kliniky. Schopnost řešení možné následné poúrazové artrózy za užití subtalární artroskopie je pak samozřejmou součástí léčebného postupu a managementu ošetření zlomenin kalkanea.

The crucial elements for errorless result of operating treatment of calcaneus fractures are: correct diagnostics, accurate indication of operating treatment, optimal timing and precisely performed operation. In this announcement authors are dealing with the diagnostics, indication of operating treatment. The operating menthod is presented, the results are evaluated. From February 2004 to December 2008 89 patients suffering from 100 fractures were operated at the Clinic of Accident Surgery. The operation is performed, on average, on the eighth day from the accident. The method of open reduction with stable osteogenic synthesis with the use of splint Synthes, carried out through the extensive lateral acces, in anaemic state of health, covered by antibiotics is used most often. The treatment is finished through active rehabilitation, without accessory rigid fixation. The existing results – minimum of serious complications, good clinical and X-ray findings are the expected and correct results of the activities at the Clinic. The ability to solve the possible subsequent afteraccident arthrosis using the subtalar arthroscopy is the apparent part of the medical treatment and management of calcaneus fractures.
