Karotická endarterektómia predstavuje v porovnaní s medikamentóznou liečbou účinnejšiu metódu prevencie cievnej mozgovej príhody pacientov so symptomatickou alebo asymptomatickou formou aterosklerotického ochorenia karotických tepien. Stentovanie s použitím emboloprotekcie je menej invazívnou alternatívou liečby s porovnateľnými až lepšími výsledkami ako endarterektómia. Úmerne s nárastom operačnej rizikovosti pacienta dokázateľne stúpa aj jeho potenciálny benefit z karotického stentingu. S dôrazom na prax, článok rozoberá vedeckú úroveň medicínskych dôkazov (levels of scientific evidence - evidence) a faktov svedčiacich v prospech spomenutých konštatovaní, ako aj výsledky karotického stentovania u nás.

Carotid endarterectomy is more effective than medical management in the prevention of stroke in patients with severe symptomatic or asymptomatic atherosclerotic carotid artery stenosis. Stenting with the use of an embolic-protection devices is a less invasive treatment strategy with comparable and even better results than endarterectomy. The benefit from carotid stenting rising parallel with increase of patients surgical risk. The aim of article is to discuss from practical point of view the levels of scientific evidence (evidence) of these facts and also our local results with carotid stenting.
