Infekční komplikace TEP kolenního kloubu je závažná komplikace. V jejím řešení je s úspěchem používaná metoda dvoudobé reimplantace se zavedením statického nebo artikulovaného spaceru. Kazuistika popisuje případ starší pacientky po primární implantaci TEP kolenního kloubu pro gonartrózu, u které byl průběh příznivý až do pádu, kdy došlo k částečnému poškození m. quadriceps femoris a hemartrosu. Následně došlo postupně k uvolnění TEP v terénu opožděné infekce, 39 měsíců po primární implantaci byla provedena extrakce TEP, zaveden statický spacer. Z důvodu zhoršení zdravotního stavu již nebylo možné reimplantaci TEP v druhé době provést. Výsledek 5 let od zavedení spaceru je uspokojivý, koleno je klinicky stabilní, zkrat končetiny je 3 cm. Pacientka je spokojená, mobilní a soběstačná. Statický spacer je možnost definitivního řešení infekční komplikace TEP kolenního kloubu ve výjimečném případě, kdy celkový stav pacienta neumožní další reoperaci.

Total knee arthroplasty infection is a serious complication. The two- stage revision with the static or articulated spacer implantation is a successful treatment option. This case report describes one case of an older woman after primary total knee arthroplasty for gonarthrosis, in which the postoperative course was positive, until a fall with partial m. quadriceps femoris rupture with haemarthros. The prosthesis was loosed in the area of delayed infection at that point. 39 months after the primary implantation, the extraction of the prosthesis with the static spacer implantation was performed. Re-implantation was not possible in second stage due to the health condition deterioration. The result 5 years after the spacer implantation is acceptable. The knee is stable; the leg is shorter by 3cm. The patient is satisfied and mobile. The static spacer is the final option of total knee arthroplasty infection treatment in the rare cases when re-implantation is not possible due to the patient’s health condition.
