Cíl práce: Hodnocení pacientů zařazených do registru biologické/cílené léčby BIOREP v České republice za rok 2020. Metody: Bylo provedeno retrospektivní hodnocení pacientů k datu 31. 12. 2020 zařazených do registru BIOREP. Předmětem analýzy byli pacienti na biologické/cílené léčbě v daném období v jednotlivých kategoriích a hodnocení populace pacientů s psoriázou a hidradenitidou. Výsledky: K 31. 12. 2020 bylo v registru BIOREP evidováno celkem 3 121 pacientů. S psoriázou bylo sledováno 2 870 (92,0 %) pacientů, 243 (7,8 %) pacientů s hidradenitidou a 8 (0,3 %) pacientů s použitím léčby "off-label". Ve skupině léčených pro psoriázu bylo 63,6 % mužů, průměrný věk pacientů (mužů i žen) ke konci roku 2020 byl 51,8 let, průměrný věk při diagnóze psoriázy byl 24,8 let a při nasazení první biologické/cílené léčby 46,1 let. Průměrná doba od diagnózy do zahájení první cílené léčby byla 21,4 let. Ke konci roku 2020 byli pacienti léčeni biologickou léčbou v průměru 5,4 let. Přidružené onemocnění se vyskytovalo u 72,2 %, nejčastější byla metabolická/endokrinní onemocnění (53,8 %) a kardiovaskulární onemocnění (53,2 %). Z jednotlivých komorbidit byla nejčetnější hypertenze (50,6 %), dyslipidémie (38,6 %) a diabetes mellitus (16,3 %). Celkem 75,9 % pacientů trpělo nadváhou či obezitou a v registru bylo 31,6 % kuřáků. Souběžnou psoriatickou artritidu mělo 35,1 % pacientů. Při zahájení léčby a při poslední návštěvě bylo průměrné PASI 18,8, resp. 3,0 a DLQI 16,4, resp. 2,4. V roce 2020 nově zahájilo léčbu celkem 366 pacientů. K 31. 12. 2020 bylo v registru celkem 243 pacientů s diagnózou hidradenitis suppurativa, z toho 52,3 % mužů, průměrný věk byl 45,0 let. V době diagnózy bylo pacientům v průměru 34,2 let a v době nasazení v průměru 43,2 let. Průměrná doba od diagnózy do zahájení první biologické léčby byla 9,0 let, pacienti byli léčeni v průměru 2,1 let. V roce 2020 svou první biologickou léčbu zahájilo celkem 52 pacientů. Na konci sledovaného roku 2020 bylo léčeno celkem 206 pacientů a průměrná délka léčby jedním léčivým přípravkem byla 2,2 roku. Závěr: BIOREP je prvním registrem pacientů biologické léčby psoriázy v zemích střední a východní Evropy. Výsledky při porovnání s evropskými registry ukazují srovnatelnou nebo vyšší prevalenci souběžných onemocnění a rizikových faktorů a dlouhé období nedostatečné léčby před nasazením biologické léčby.

Background and objectives: Evaluation of patients included in the registry of biological/targeted therapy BIOREP in the Czech Republic in 2020. Methods: A retrospective evaluation of patients enrolled in the BIOREP registry to date 31 December 2020 was performed. The objects of our study were patients on biological/targeted treatment in the given period in each category and the evaluation of the population of patients with psoriasis and hidradenitis. Results: As of 31 December 2020, a total of 3121 patients were registered in the BIOREP registry: 2870 (92.0%) patients with psoriasis, 243 (7.8%) with hidradenitis and 8 (0.3%) treated with off label treatment. In the psoriasis group, 63.6% of patients were men. The average age of patients at the end of 2020 was 51.8 years, the mean age at the time of diagnosis was 24.8 years, and the age at the time of first biological/targeted treatment was 46.1 years. In 2020, the average time from diagnosis to the introduction of biological treatment was 21.4 years, the average duration of biological/targeted treatment was 5.4 years. Comorbidities occurred in 72.2% of patients, mostly metabolic/endocrine diseases (53.8%) and cardiovascular diseases (53.2%), more specifically hypertension (50.6%), dyslipidemia (38.6%) and diabetes mellitus (13.3%) were the most common. More than 75% of patients were overweight or obese and 31.6% of patients were smokers. Psoriatic arthritis was observed in 35.1% of patients. At the start of the treatment and at the last visit, the mean PASI was 18.8 and 3.0, respectively and DLQI 16.4 and 2.4 respectively. In 2020, a total of 366 patients newly started treatment. As of 31 December 2020, there were 243 patients with hidradenitis suppurativa in the registry, 52.3% were men and the mean age was 45 years. The mean age at the time of diagnosis was 34.2 years and the age at the time of the initiation of the first biological treatment was 43.2 years. The average time from diagnosis to the start of the biological treatment was 9.0 years and patients were treated for an average of 2.1 years. In 2020, a total of 52 patients started the biological treatment. At the end of 2020, a total of 206 patients were treated and the average duration of treatment with one medication was 2.2 years. Conclusion: BIOREP is the first registry of patients for biological treatment of psoriasis in Central and Eastern Europe. The results, when compared to European registries, show a comparable or higher prevalence of co-morbidities and risk factors and a long period of inadequate treatment before initiating biologic therapy.
