Na základě zpráv o nežádoucích účincích léků (NÚL), které jsou shromažďovány ve Státním ústavu pro kontrolu léčiv v Praze, byla vypracována analýza nežádoucích účinků perorálních biguanidových antidiabetik. Pozornost byla zaměřena především na výskyt laktátové acidózy. Analýza byla provedena z 37 341 zpráv o NÚL, z nichž 95 popisovalo nežádoucí reakce biguanidových antidiabetik. Laktátová acidóza se objevila 14krát, a to v devíti případech po přípravku Silubin retard a v pěti případech po pripravku Buformm retard. Deset pacientů zemřelo. Tyto údaje spolu s dalšími jsou graficky znázorněny na diagramech. Byl zde sledován výskyt nežádoucích účinků podle věku, pohlaví, podle zdroje zprávy (ambulantiií, lůžková zarizení). Bylo provedeno i srovnání nežádoucích reakcí po biguanidech s normálním spekfrem hlášení z celé kartotéky NÚL České republiky. Byla zachycena také spotřeba biguanidových antidiabetik v posledních více než dvaceti letech. V diskusi jsou uvedeny zkušenosti s výskytem laktátové acidózy u nás i v zahraničí. Jsou naznačeny možnosti předcházení vzniku této vážné nežádoucí reakce, která bezprostředně ohrožuje život pacienta.

Based on reports on undesirable dmg effects which are assembled in the State Institiite for Dmg Control in Prague tiie authors elaborated an analysis of undesirable effects of oral biguanide antidiabetics. Attention was focused above all on the incidence of lactate acidosis. The analysis was based on 37 341 reports on tmdesirable dmg effects, incl. 95 which described undesirable reactions to biguanide antidiabetics. Lactate acidosis was recorded 14 times, in nine instances after the preparation Silubin retard and in five instances after the preparation Buformin retard. Ten patients died. These data along with others are illustrated in diagrams. The undesirable effects were investigated with regard to age, sex, the source of the report (ambulatory, in- patient facilities). The authors compared also undesirable reactions after biguanides with the general spectiimi of reports from the files on undesirable dmg effects recorded in the Czech republic. The authors registered also the consumption of biguanide antidiabetics during the last more than twenty years. In the discussion they give an account on the incidence of lactate acidosis in this country and abroad. The authors indicate possible ways how to prevent this serious undesirable reaction which threatens the patient's life.
