Postihnutie hornej časti tráviaceho traktu u pacientov s Parkinsonovou chorobou sa v literatúre zmieňuje menej často než postihnutie jeho dolnej časti. Jeho klinický význam je ale značný, pretože dysfágické ťažkosti významne znižujú kvalitu života pacientov a môžu viesť k život ohrozujúcim komplikáciám. Napriek tomu, že klinické ťažkosti sa spájajú skôr s pokročilejšími štádiami ochorenia, poruchy motility pažeráka ako dôsledok Parkinsonovej choroby možno pomocou pažerákovej manometrie s vysokým rozlíšením zachytiť oveľa skôr. Autori v článku popisujú základné princípy diagnostického manažmentu dysfágie u pacientov s Parkinsonovou chorobou a v klinickej časti prezentujú analýzu vlastného súboru pacientov. Zistili, že nešpecifické a rôznorodé poruchy motility sú prítomné u väčšiny pacientov aj v neprítomnosti dysfágických symptómov. V experimentálnej časti sme načrtli nové možnosti diagnostických postupov pomocou najmodernejších metodík detekcie patologických foriem alfa-synukleínu a pokročilých mikroskopických metód.
Involvement of the upper part of the gastrointestinal tract in patients with Parkinson disease is reported less frequently than the involvement of the lower part. Its clinical impact is, however, substantial because dysphagic symptoms considerably decrease the quality of life and may lead to life threatening complications. Even though the clinical symptoms related to esophageal motility disorders as a result of Parkinson disease are more common in the advanced stages of the disease, these could be diagnosed much earlier using high resolution manometry. The authors describe the basic principles of diagnostic management of dysphagia in patients with Parkinson disease and in the clinical part they present an analysis of their patient cohort. They found out that nonspecific and diverse motility disorders are present in most patients, even without dysphagic symptoms. In the experimental part, we outlined new possibilities for diagnostic procedures using the most up-to-date methods for the detection of pathological forms of alpha-synuclein and advanced microscopic methods.

Circulating extracellular DNA (ecDNA) is known to worsen the outcome of many diseases. ecDNA released from neutrophils during infection or inflammation is present in the form of neutrophil extracellular traps (NETs). It has been shown that higher ecDNA concentration occurs in a number of inflammatory diseases including inflammatory bowel disease (IBD). Enzymes such as peptidyl arginine deiminases (PADs) are crucial for NET formation. We sought to describe the dynamics of ecDNA concentrations and fragmentation, along with NETosis during a mouse model of chemically induced colitis. Plasma ecDNA concentration was highest on day seven of dextran sulfate sodium (DSS) intake and the increase was time-dependent. This increase correlated with the percentage of cells undergoing NETosis and other markers of disease activity. Relative proportion of nuclear ecDNA increased towards more severe colitis; however, absolute amount decreased. In colon explant medium, the highest concentration of ecDNA was on day three of DSS consumption. Early administration of PAD4 inhibitors did not alleviate disease activity, but lowered the ecDNA concentration. These results uncover the biological characteristics of ecDNA in IBD and support the role of ecDNA in intestinal inflammation. The therapeutic intervention aimed at NETs and/or nuclear ecDNA has yet to be fully investigated.

The main role of research in medicine is to provide relevant knowledge which, after successful translation to clinical practice, improves the quality of healthcare. The sex bias which is still present in the majority of research disciplines prefers male subjects despite legislation changes in the US grant agencies and European research programme Horizon 2020. Male subjects (cells, animals) still dominate in preclinical research and it has detrimental consequences for women's health and the quality of science. Opposite bias exists for data obtained mainly in animal models utilizing female subjects (e.g. research in multiple sclerosis, osteoporosis) with skewed outcomes for men affected by these diseases. Either way, scientists are producing results which compromise half of the population. Assumptions that females as cohorts are more variable and another assumption that the oestrous cycle should be tracked in case the females are enrolled in preclinical studies were proven wrong. Variability of male versus female cohorts are comparable and do not only stem from hormonal levels. The widespread prevalence of sex differences in human diseases ultimately requires detailed experiments performed on both sexes, unless the studies are specifically addressing reproduction or sex-related behaviors.
Východiska: Viscerálne aferentné nervy monitorujú vnútorné orgány a sprostredkujú informácie centrálnemu nervovému systému. Úloha aferentných nervov v patogenéze a symptómoch gastrointestinálnych ochorení je často podceňovaná. Preto treba zdôrazniť, že bolesť, pálenie záhy, plnosť, nevoľnosť a vracanie sú všetko symptómy sprostredkované viscerálnymi aferentnými nervami. Dôležité je tiež vyzdvihnúť, že stimulácia viscerálnych aferentných nervov zápalom pri chorobách narúša normálnu reguláciu sekrécie a motility, čím prispieva k symptómom od dysfágie po hnačku a zápchu. Z tohto výpočtu symptómov je zrejmé, že terapeutické ovplyvnenie viscerálnych aferentných nervov by mohlo pomôcť pri liečbe mnohých gastrointestinálnych ochorení. Vývoj nových liekov zameraných na viscerálne aferentné nervy však brzdí nedostatok informácií o týchto nervoch. Napríklad ani receptory pre kyselinu v pažerákových nervoch sprostredkujúcich pyrózu, ako ani iónové kanály zodpovedné za mechanickú aktiváciu v nervoch sprostredkujúcich koliku neboli celkom objasnené. Nedostatok informácií o viscerálnych aferentných nervoch možno vo veľkej miere pripísať ťažkostiam spojeným s ich štúdiom, typicky po jednom nervovom vlákne metódou klasickej elektrofyziológie. Cieľ: V tomto článku prezentujeme adaptáciu špičkovej vysoko výkonnej metódy na štúdium gastrointestinálnych aferentných nervov, ktorá umožňuje súčasné zobrazovanie aktivity v stovkách neurónov. Táto metóda je založená na genetickej expresii fluorescenčného vápnikového indikátora GCaMP6 a multifotónovej mikroskopie v transgénnej myši. Očakáva sa, že okrem poskytovania základných informácií o viscerálnych aferentných nervoch budúce iterácie tejto metódy umožnia štúdium nervovej aktivity v biopsiách od pacientov s gastrointestinálnymi ochoreniami.
Background: Visceral afferent nerves project from internal organs and provide information to the central nervous system. The role of afferent nerves in the pathogenesis and symptoms of gastrointestinal diseases is often underappreciated. Nonetheless, pain, heartburn, fullness, nausea, and vomiting are all mediated by visceral afferent nerves. Furthermore, stimulation of visceral afferent nerves by inflammation in diseases disrupts the normal regulation of gastrointestinal secretion and motility, thereby contributing to symptoms ranging from dysphagia to diarrhea and constipation. Thus, therapeutic targeting of visceral afferent nerves may aid the treatment of many gastrointestinal diseases. However, the development of novel drugs targeting visceral afferent nerves is hindered by the limited knowledge of these nerves. For example, the identities of the acid receptors in esophageal nerves that cause heartburn and the ion channels responsible for mechanical activation in nerves that mediate colic pain have not been fully elucidated. The limited knowledge of visceral afferent nerves is largely attributable to difficulties associated with their study; only one nerve fiber can typically be studied at a time by classical electrophysiology. Purpose: Here, we report the adaptation of a cutting-edge high-throughput method to study gastrointestinal afferent nerves that allows the activity in hundreds of neurons to be imaged simultaneously. This method is based on genetic expression of the fluorescent calcium indicator GCaMP6 and multiphoton microscopy in transgenic mice. In addition to providing fundamental information about visceral afferent nerves, future iterations of this method are expected to enable studies of nerve activity in biopsies from patients with gastrointestinal diseases.
