Background and Aims: The pathogenesis and risk factors for early postoperative endoscopic recurrence of Crohn's disease (CD) remain unclear. Thus, this study aimed to identify whether histological inflammation at the resection margins after an ileocaecal resection influences endoscopic recurrence. Methods: We have prospectively followed up patients with CD who underwent ileocaecal resection at our hospital between January 2012 and January 2018. The specimens were histologically analysed for inflammation at both of the resection margins (ileal and colonic). We evaluated whether histological results of the resection margins are correlated with endoscopic recurrence of CD based on colonoscopy 6 months after ileocaecal resection. Second, we assessed the influence of known risk factors and preoperative therapy on endoscopic recurrence of CD. Results: A total of 107 patients were included in our study. Six months after ileocaecal resection, 23 patients (21.5%) had an endoscopic recurrence of CD. The histological signs of CD at the resection margins were associated with a higher endoscopic recurrence (56.5% versus 4.8%, p < 0.001). Disease duration from diagnosis to surgery (p = 0.006) and the length of the resected bowel (p = 0.019) were significantly longer in patients with endoscopic recurrence. Smoking was also proved to be a risk factor for endoscopic recurrence (p = 0.028). Conclusions: Histological inflammation at the resection margins was significantly associated with a higher risk of early postoperative endoscopic recurrence after an ileocaecal resection for CD.

Zánět v oblasti ileocekální chlopně a okolí je klasicky spojován s Crohnovou nemocí. Ale existuje velké množství dalších onemocnění s patologickým nálezem v této oblasti. Patří sem ostatní idiopatické střevní záněty, infekce, spondylartropatie, polékové změny, infiltrace, postiradiační enteritida, celiakie i vaskulitidy.
Inflammation of the ileocaecal area is classically connected with Crohn's disease, but a wide variety of diseases is associated with inflammation in this region. These include other inflammatory bowel diseases, infections, spondyloarthropathies, drug-related enteritis, infiltration (e.g. eosinophil enteritis), post-irradiation enteritis, endometriosis, celiac disease and vasculitis.

Úvod: V současné době je pozorován vzestup prevalence Crohnovy choroby (CD) v různých oblastech světa. Ačkoliv je patrný intenzivní výzkum nových léčebných přístupů, výsledky konzervativní léčby nejsou stále povzbudivé a velký počet pacientů vyžaduje chirurgickou léčbu. Cíl: Rozbor průběhu CD u nemocných hospitalizovaných na Gastroenterologické a hepatologické klinice Wroclavské lékařské fakulty v období 1993–2003. Materiál a metody: Přehled anamnestických údajů u 156 nemocných s CD. Ve sledované skupině, která byla složena z 91 žen ( průměrný věk 38 let ± 15,2) a 65 mužů (průměrný věk 35 let ± 14,9) byly analyzovány: lokalizace zánětlivých změn, aktivita onemocnění, komplikace, metody a výsledky léčby. Výsledky: CD byla diagnostikována u 53 % nemocných ve druhé a třetí věkové dekádě. Zánětlivé změny byly lokalizovány nejčastěji v ileocékální oblasti. Extraintestinální komplikace CD byly pozorovány u 26 % nemocných (nejčastěji artritida, artralgie, erythema nodosum). Konzervativní léčba během hospitalizace vedla k poklesu CDAI u 70 % pacientů. 52 % nemocných si vyžádalo chirurgický zásah v průběhu trvání Crohnovy nemoci. Nejčastější indikací k chirurgickému zákroku byl zánětlivý tumor a stenóza. Závěr: Rozbor desetiletého sledování nemocných s CD potvrzuje známé literární skutečnosti: chronický průběh, neuspokojivé dlouhodobé výsledky konservativní léčby a častý výskyt komplikací vyžadující chirurgický zákrok.
Introduction: Recently an increase in Crohn's disease (CD) prevalence frequency in different world regions is observed. Although there is an intensive development of new approaches to the CD treatment, outcome of conservative management in most cases is disappointing and high percentage of patients with CD requires surgical procedures. Aim: Analysis of CD natural history in patients hospitalized in Department of Gastroenterology and Hepatology of Wroclaw Medical University in the period 1993–2003. Material and method: Review of 156 histories of patients wit CD. Studied group consisted of 91 females (mean age 38 ± 15.2) and 65 males (mean age 35 ± 14.9). Localization of inflammatory changes, CD activity, complications, methods and outcome of treatment were analysed. Results: CD was diagnosed in 53 percent of cases in the second or third life decade. Inflammatory changes were localized the most frequent in the ileocaecal region. Extraintestinal complications of CD were observed in 26 percent of cases (the most common were arthritis, arthralgia, erythema nodosum). Conservative treatment during hospitalization resulted in decreasing of CDAI scores in 70 percent of cases. 52 percent of patients required surgical intervention during CD course. The most frequent indications for surgery were inflammatory tumor and stenosis. Conclusions: Analysis of 10 years period experience in CD treatment confirms known from the literature CD features: chronic course, disappointing long-term outcome of conservative management and frequent occurrence of complications requiring surgical intervention.
