Cíl studie: V roce 2018 Evropská společnost pro gynekologickou onkologii (ESGO), Evropská společnost pro radioterapii a onkologii (ESTRO) a Evropská společnost pro patologii (ESP) společně vypracovaly klinicky relevantní doporučení založené na recentních vědeckých datech s cílem standardizovat a zlepšit kvalitu léčby u žen se zhoubným nádorem děložního hrdla. Doporučení rovněž zahrnují racionální přístup k diagnostice a stagingu onemocnění. Typ studie: Přehledový článek. Název a sídlo pracoviště: Gynekologicko-porodnická klinika, 1. lékařská fakulta Univerzity Karlovy a Všeobecná fakultní nemocnice v Praze. Metodika: Literární přehled prací, které se staly podkladem mezinárodního doporučení racionální diagnostiky a stagingu zhoubného nádoru děložního hrdla. Výsledky: V souladu s těmito doporučeními je gynekologické vyšetření s odběrem biopsie vhodné pouze ke stanovení diagnózy zhoubného nádoru, nikoliv rozsahu onemocnění. Stanovení rozsahu onemocnění a prognostických parametrů nezbytných pro plánování individualizované péče je založeno na moderních zobrazovacích metodách. Metodou první volby v lokálním stagingu zhoubného nádoru děložního hrdla je magnetická rezonance pánve (MR). Poprvé v historii však byla akceptována role transvaginálního nebo transrektálního ultrazvuku jako efektivní alternativní metody první volby, srovnatelně přesné s magnetickou rezonancí pánve. Obě metody na základě vysokého tkáňového rozlišení v pánvi umožní přesné stanovení přítomnosti a rozsahu onemocnění v pánvi, včetně stanovení hloubky nádorového šíření do močového měchýře a rekta. Cystoskopie a rektoskopie jsou proto považovány za obsolentní vyšetření, která vedou pouze k zátěži pacientky a zbytečnému prodloužení času do zahájení léčby. Podobně vylučovací intravenózní urografie již není součástí stagingových vyšetření. Kromě lokálního rozsahu je znalost postižení lymfatických uzlin nezbytným parametrem pro stanovení prognózy onemocnění a plánování léčby. Přesnost zobrazovacích metod v záchytu infiltrovaných lymfatických uzlin závisí na prevalenci metastáz s ohledem na stadium onemocnění a na velikosti uzlinových metastáz. V časném stadiu onemocnění (T1a, T1b1, T2a1) u pacientek s negativními pánevními uzlinami na ultrazvuku či MR je chirurgický staging pánevních lymfatických uzlin metodou volby pro záchyt malých metastáz. Je důležité zdůraznit, že ultrazvuk ani MR nezachytí drobná nádorová ložiska v uzlině, ale obě metody mají velmi nízký počet falešně pozitivních nálezů, jelikož dovedou spolehlivě posoudit přítomnost infiltrované uzliny na základě charakteristických změn. V případě lokálně pokročilého onemocnění (T1b2 a vyšší stadium, kromě T2a1) nebo časného onemocnění s postiženými pánevními uzlinami podle ultrazvuku či MR je doporučeno doplnit CT nebo PET/CT s cílem zachytit extrapelvické vzdálené metastázy (paraaortální uzliny, hrudník). PET/CT je preferováno v případě primární chemoradioterapie. Bohužel žádná zobrazovací modalita nevyloučí přítomnost mikroskopických paraaortálních metastáz. Proto v případě negativního nálezu na CT nebo PET-CT v zobrazení paraaortálních uzlin je ke zvážení doplnění chirurgického stagingu v rozsahu paraaortální lymfadenektomie. Vzhledem k riziku falešně pozitivních nálezů v zobrazovacích metodách je v případě nejistých známek extrauterinního šíření doporučena biopsie sporné léze silnou jehlou pod ultrazvukovou nebo CT navigací. Závěr: Přehledový článek předkládá vědecká data, která umožnila racionální změny ve stagingu zhoubného nádoru děložního hrdla.
Objective: In 2018 three European societies have joined to create clinically relevant guidelines on the diagnosis and management of cervical cancer. The European Society of Gynaecological Oncology (ESGO), the European Society for Radiotherapy and Oncology (ESTRO), and the European Society of Pathology (ESP) agreed on diagnostic approaches in cervical cancer staging. Design: Review article. Setting: Department of Obstetrics and Gynecology, First Faculty of Medicine, Charles University and General University Hospital in Prague. Methods: A literature review of published data on cervical cancer staging. Results: Physical examination with biopsy still has its place in histological confirmation of malignancy but doesn‘t offer much information on the extent of the disease. It is historically the first time when transvaginal/ transrectal ultrasound (TVS/TRS) is recommended as an alternative to the magnetic resonance (MRI) in a primary workup. Both imaging modalities offer excellent soft tissue contrast resolution, which is crucial in tumour detection and evaluation of local extent of tumour, including the depth of tumour infiltration in the bladder and rectal wall. These new advances in imaging rendered the use of cystoscopy and rectoscopy redundant. Similarly, with the implementation of modern imaging in pretreatment staging, intravenous urography has lost its role in the staging. Apart from the local extent of the disease, it is necessary to accurately evaluate the lymph node status in order to plan optimal treatment. The detection rate of imaging reflects the prevalence of lymph node metastases depending on tumor stage and size of metastasis. In the early stage disease (T1a, T1b1, T2a1) with negative lymph nodes on TVS/TRS or MRI, surgicopathological staging of pelvic lymph nodes is a method of choice for detection of small volume metastases. Both imaging modalities might not detect small metastatic lesions within non-enlarged lymph nodes, but by identifying the characteristic changes of the infiltrated lymph nodes they have very low rate of false positives. In locally advanced cervical cancer (T1b2 and higher, except T2a1) or early stages with positive lymph nodes detected on ultrasound or MRI, computed tomography (CT) or CT in combination with positron emission tomography (PET-CT) are recommended to assess distant spread including paraaortic lymph nodes and chest. PETCT is the preferred option in cases indicated for primary chemoradiation. Unfortunatelly no imaging method is accurate enough to exclude small volume metastasis in paraaortic nodes. In the cases with negative paraaortic lymph nodes on CT or PET-CT, surgicopathological staging with dissection of the paraaortic lymph nodes may be considered. In order to reduce false positive findings by imaging methods, it is recomended to obtain an ultrasound or CT-guided tru-cut biopsy from any equivocal extrauterine lesion to avoid inappropriate treatment. Conclusion: This review offers scientific evidence that led to the recent changes in the cervical cancer staging.

A European consensus conference on endometrial carcinoma was held in 2014 to produce multidisciplinary evidence-based guidelines on selected questions. Given the large body of literature on the management of endometrial carcinoma published since 2014, the European Society of Gynaecological Oncology (ESGO), the European SocieTy for Radiotherapy & Oncology (ESTRO) and the European Society of Pathology (ESP) jointly decided to update these evidence-based guidelines and to cover new topics in order to improve the quality of care for women with endometrial carcinoma across Europe and worldwide. ESGO/ESTRO/ESP nominated an international multidisciplinary development group consisting of practicing clinicians and researchers who have demonstrated leadership and expertise in the care and research of endometrial carcinoma (27 experts across Europe). To ensure that the guidelines are evidence-based, the literature published since 2014, identified from a systematic search was reviewed and critically appraised. In the absence of any clear scientific evidence, judgment was based on the professional experience and consensus of the development group. The guidelines are thus based on the best available evidence and expert agreement. Prior to publication, the guidelines were reviewed by 191 independent international practitioners in cancer care delivery and patient representatives. The guidelines comprehensively cover endometrial carcinoma staging, definition of prognostic risk groups integrating molecular markers, pre- and intra-operative work-up, fertility preservation, management for early, advanced, metastatic, and recurrent disease and palliative treatment. Principles of radiotherapy and pathological evaluation are also defined.

The European Society of Gynaecological Oncology (ESGO), the International Society of Ultrasound in Obstetrics and Gynecology (ISUOG), the International Ovarian Tumour Analysis (IOTA) group, and the European Society for Gynaecological Endoscopy (ESGE) jointly developed clinically relevant and evidence-based statements on the pre-operative diagnosis of ovarian tumors, including imaging techniques, biomarkers, and prediction models. ESGO/ISUOG/IOTA/ESGE nominated a multidisciplinary international group, including expert practising clinicians and researchers who have demonstrated leadership and expertise in the pre-operative diagnosis of ovarian tumors and management of patients with ovarian cancer (19 experts across Europe). A patient representative was also included in the group. To ensure that the statements were evidence-based, the current literature was reviewed and critically appraised. Preliminary statements were drafted based on the review of the relevant literature. During a conference call, the whole group discussed each preliminary statement and a first round of voting was carried out. Statements were removed when a consensus among group members was not obtained. The voters had the opportunity to provide comments/suggestions with their votes. The statements were then revised accordingly. Another round of voting was carried out according to the same rules to allow the whole group to evaluate the revised version of the statements. The group achieved consensus on 18 statements. This Consensus Statement presents these ESGO/ISUOG/IOTA/ESGE statements on the pre-operative diagnosis of ovarian tumors and the assessment of carcinomatosis, together with a summary of the evidence supporting each statement.
