AIM: The authors describe and validate a new technique of one-session, multiple-pass, low spot density, fractional ablative CO2 resurfacing in order to treat lower eyelid skin laxity and photoaging. METHODS: The investigation was conducted on 25 patients. The YouLaser CO2 (Quanta System s.p.a., Italy), a 10600-nm CO2 ablative fractional laser system, was used to treat the lower eyelids at a pulse energy of 25 to 35 J, with a pulse duration of 1 ms, 200 microm spot size and a density of 100 laser spots/cm2 for every passage. Each patient had 1 session only, and 1 to 3 passes per area, after application of topical anesthesia. Two blinded physicians assessed eyelid tightening from photographs of 25 patients using a 4-point evaluation scale (1 = 1-25%, 2 = 26-50%, 3 = 51-75%, and 4 = 76-100%. The grades of the 2 evaluating physicians were analyzed using t-test and Kruskal-Wallis analysis of variance. The same evaluation scale was used to assess patient satisfaction in a questionnaire. RESULTS: The median grades of the 2 physicians for all patients did not differ significantly (p = .88) from one another, and the Kruskal-Wallis assessment (p = 0.0011) indicated unbiased grading among the physicians. All patients achieved a degree of eyelid tightening; 8% achieved 1% to 25% improvement, 24% achieved 25% to 50%, 40% achieved 50% to 75%, and 28% achieved 75% to 100%. Results of patient questionnaires were similar to that of the physicians' evaluation: 4% achieved 1% to 25% improvement, 28% achieved 25% to 50%, 48% achieved 50% to 75%, and 20% achieved 75% to 100%. CONCLUSIONS: The fractioned form of CO2 laser delivery offers very good results with short downtime and easy patient recuperation. The new technique allowed the operator to control skin contraction immediately after laser skin exposure. Using high energy and lower laser spot density permits a control of the amount of contraction simply by the number of passes. Increasing this variable progressively increased skin damage and consequently the contraction.

Photoaging is an inevitable occurrence for people who have a fair skin type, who live in a sunny climate, and who enjoy the outdoors. In the past the remedies for this condition were limited to dermabrasion or chemical peeling. Both dermabrasion and chemical peels have hazards of their own. With phenol peels, cardiac and renal toxicity was real concern. There is an increasing demand for an effective and safe laser treatment that repairs photoaged skin. Two treatment modalities, ablative skin resurfacing (ASR), and nonablative dermal remodeling (NDR), have been developed to address this demand. All currently available laser treatments, however, exhibit significant problems, and these laser systems typically operate safely and effectively only over a narrow, patient-dependent treatment range. This study aimed to analyze a new protocol of fractional rejuvenation and intense pulsed light for skin rejuvenation regarding its efficacy, safety, and complications. Twenty-nine patients (27 women and 2 men), age ranging from 40 to 73 years, answered the questionnaire and were included in the study. The use of intense pulsed light after the fractional rejuvenation allows us to enhance the effects of this new procedure especially in skin pigmentation disorders and telangiectasia and to continue to stimulate the deep dermal component.

The author describes a new technique of fat grafting of the buttocks, GLADI (Gluteal Adipose Implant) based on Coleman procedure, associated with the lipoplasty of the gluteal-trochanteric region in order to improve harmony of the lower limbs. 132 patients were treated by this technique with very good results, no major complication, and a durable increase of the gluteal region in 125 (94.7%) of them. GLADI technique is a valid alternative to silicone implantations and offers durable improvements in gluteal shape without the need of additional surgery or the risk of implant rupture.

The Authors describe the case of a 40-year-old woman with a ruptured carboxi-methyl-cellulose mammary implant. The utilisation of this type of implants will spread rapidly, so it is very important to diffuse knowledge about these. The Authors conclude that carboxi-methyl-cellulose hydrogel, as a filler, is a valid alternative to silicone and saline solution (4, 5).

The Authors present their experience with carboxy-methyl-cellulose hydrogel mammary implants. 110 patients have undergone operations since October 1998 with the implantation of this type of prosthesis. The results are very good: no major complications, and only 22 patients (20%) showed a II stage capsular contracture and 4 patients (3.6%) a III stage, according to Baker's scale, with an average follow-up of 20 months. The low percentage of capsular contracture and the absence of major complications, let us to conclude that this type of implant is highly tolerated by the human body.

Laser resurfacing is a new method to treat atrophic scars. The pulsed erbium YAG laser has been shown to be effective in the treatment of this variations of the skin texture secondary to a lot of conditions. Er:YAG laser allows precise epidermal ablation with minimal thermal injury to surrounding tissues. The goal of this study was to evaluate the effectiveness of the Er:YAG laser in the treatment of atrophic scars with multiple applications. In agreement with previously reported studies our results show that multiple treatments with erbium YAG laser are safe and effective to improve atrophic scars.

The authors used a reductive mammaplasty technique with superior pedicled dermoglandular flap described by Lalardrie in 1972 and modified by Lauro. Twenty-eight patients who presented medium to heavy macromasty were treated. In all these cases the aesthetic result was very satisfactory and major complications, like extended tissue necrosis, were not encountered. This method proves safe and effective in cases of significant gigantomasty for the following reason: for the efficient vascularization of the flap, it is possible to carry out significant transposition of the areola-nipple complex without risk of complications. This technique can be used in less serious cases in conjunction with other equally easy techniques. Because of its simplicity, this technique also allows younger, less experienced, surgeons to approach this type of surgery with the necessary peace of mind.

The harmony of the lower limbs basically depends on the gluteal-trochanteric region. The authors intend to analyze the space geometry of the gluteal-trochanteric region, and describe the technique of liposculpture and lipofilling of this area. 4917 patients were treated since 1983. 3416 patients performed only liposculpture procedure, 1501 patients performed liposculpture and lipofilling of the gluteal-trochanteric region. No major complications. Fair good aesthetic results. Liposculpture and lipofilling procedures allow us to improve safely the aesthetic aspect and the harmony of both the lower limbs and the glutel.
