Ambient air particulates derived from a magnesite-processing plant contain magnesium oxide (85-90%), iron oxide (7-8%), calcium oxide (about 2.5-3%), Al- and Si oxides (0.1-0.5%) as well as additional elements in trace amounts as follows: Ni, Cr, Mn, Co, Hg, Cd, Pb, V, Ba, Ag, Cu and Sn. Magnesite clinker is manufactured from raw magnesite by baking in rotary furnaces using the Leseps-Polysius system, which represents the source of particulate pollutants entering the atmosphere. In the area heavily polluted by magnesite particles, the health impact using the data obtained from the exposed humans and animals was assessed. The reason for this study were the data on the increased frequency of respiratory diseases in children in the polluted area. In a group of children (9-10 years of age) residing in the area, the immune response to ambient air pollutants exposure in comparison to a non-exposed group living outside the polluted area was investigated. A statistically significant decrease in the amount of salivary lysozyme, SlgA, IgG and albumin in the exposed group was observed. Simultaneously, a 6-month respiratory exposure of a group of Chinchilla rabbits in that polluted area aimed at the simulation of the human exposure was conducted. In the exposed rabbits, enormously increased number of pulmonary alveolar macrophages (PAM) has been found. Using atomic absorption spectrometry, a statistically significant increase in magnesium content (e.g. a bioindicator of the exposure) in the body tissues of the exposed animals was found. The laboratory data obtained from the animal study contributed to the elucidation of the depressed immunity (especially in the respiratory system) in the children exposed to ambient air magnesite particulates.

The authors examined IgG sub-classes, using the ELISA method, in 35 patients with relapsing respiratory diseases and 27 children of a control group aged 3-15 years. In children with relapsing respiratory diseases without deficiency of the main immunoglobulin classes they found a statistically significantly reduced value in sub-class IgG2 (P < or = 0.01). The changes found in other sub-classes were not statistically significant. The authors draw attention to the fact that deficiency in IgG sub-classes should be considered in case of repeated respiratory infections caused by encapsulated microorganisms--H. influenzae, Str. pneumoniae--and also when reduced levels of IgG and IgA are found. However, even normal levels do not rule out deficiency.

Two gammaglobulin preparations have been developed: Alga (10% injection solution of serum bovine gamma-globulin and albumin) and Algalev (with addition of levamisole 10 mg/ml, patented under PV 4069/87); their administration to calves in field conditions was evaluated. The Alga preparation at a rate of 1 ml/kg liveweight (two administrations) had good preventive effects in the diarrhoeic syndrome of new-born hypogammaglobulinaemic calves. In comparison with the untreated calves, these parameters were determined: lower incidence of diarrhoea (41% against 75%; Tab. I), its later onset (by 24 hours), shorter duration (by 50 hours; P < 0.01), lower intensity and easier therapeutical handling (Tab. II). The Algalev preparation (two administrations at a rate of 1 ml/kg liveweight) was suitable for metaphylactic use (combined with antibiotic application) in the respiration syndrome of calves. In comparison with the untreated calves, these parameters were determined: lower incidence of this disorder (42% against 78%), lower intensity of clinical symptoms (intensive symptoms in 20% of the calves against 72%) and importantly higher daily weight gains (0.64 kg against 0.26 kg; Fig. 3). The concentrations of total serum Ig (CS-Ig), serum proteins (SB) and serum albumin (S-Alb) were similar in the compared groups of calves in the whole period of observation (Fig. 1). Certain differences were observed in the dynamics of anti-PI 3 titre of serum antibodies as shown by investigation of specific serum antibodies (Fig. 2); the level of their production was in agreement with morbidity incidence and clinical symptom intensity in both groups of calves.

The authors treated with Biostim (Roussel Uclaf Co., France) 20 children aged 3 to 15 years with relapsing and chronic diseases of the respiratory system. A favourable clinical effect was observed in 45% and improvement in 40% of the patients. In the investigated immunological indicators the response to treatment Biostim was manifested in the antibody immunity by a reduction of IgE values (P < 0.01) and of IgM (P < 0.05). As to cellular immunity, there was a significant increase of the phagocytic activity (P < 0.05). The tolerance of the preparation was very good.

Changes of the selected immunological parameters before and after the speleotherapy are studied. It seems, that CIK and Complement parameters can be effected during the treatment in caves while Ig and LSZ parameters did not show any systematic changes. Details are available in the paper.

HLA B27 antigen was found in 10/25 patients with arthritis after acute respiratory disease (ARD) and in 21/75 direct relatives of 23 of these probands, where a genealogical investigation could be made. With regard to the 9.5% incidence of HLA B27 in the Czech population this implies a positive association in patients (RR = 6.35) and direct relatives (R = 3.70). In the sub-group of HLA B27 positive probands HLA B27 positivity was found in 19/27 (70.4%) of direct relatives (RR = 22.6). In one female relative a recurrent monoarthritis of the talo-crural joint was found, the remainder did not suffer from inflammatory changes of the joints. The revealed positive associations indicate the probability of arthritis after ARD in HLA B27 positive subjects and their families. They indicate also that in arthritis after ARD reactive arthritis is involved with a less common organ manifestation of the initial infection.

In 1981-1984 from a total of 288 children aged 6-36 months, suffering from acute and relapsing respiratory disease, in 52 children in the nasopharyngeal secretion IgM was detected in average amounts of 11.3 +/- 9.7 mg/100 ml. In 13 of the children in the course of 1-2 years after the first examination the IgM values in the nasopharyngeal secretion doubled or increased several times (20-50 mg/100 ml). The values in saliva at the age of 5-9 years were in seven of these children lower than the mean physiological values of SIgA in saliva (34.3 +/- 15.9 mg/100 ml). In 12 children in the course of 5-9 years chronic respiratory disease developed--obstructive bronchitis. In six children, where during school age in saliva mean SIgA values lower than normal were recorded, more frequent relapses and complications were recorded (pneumonia, sinobronchial syndrome, otitis media). Elevated values of SIgM in the nasopharyngeal secretion of infants and toddlers with acute and relapsing bronchitis can be considered a risk factor for the development of chronic disease, in particular in case of a permanently reduced SIgA formation also in saliva.
