Coequalitisation of the relation between the physician and his patient, which respects the professional dominancy of the first one, has promoted the full guidance before any serious diagnostic or therapeutic intervention. Patient has the right to be informed of his health status and of the therapeutic plan. He has also the right to refuse such information. Only after the full information, the patients can express his approval with the plan for therapy. Before any intervention, the patient's approval is necessary; exceptional are only by the law implicated situations. To get the lawful and therefore the qualified or informed approval, the patient should be able to accept the information, to consider its significance, to make decision, and the express his will. Incapable of doing that is not only the person with permanently decreased ability to resolve, but also those who are not able to accept the information because of language problem and those, whose psychical state is influenced by the actual state of the disease, therapy etc. For a child, the person who has the right to be informed and to give the consent is his protector (usually his parent), however, within the limits of his comprehension an older child has also the right to express his opinion. For the common treatment the written form of approval is not necessary, it is enough to get the oral consent. For intervention of a higher significance and risks, the written form is recommended. It should be connected with a possibility to discuss and get explained any questions. The information should include not only what is to be done, but also how it will be done and what are the benefits of the treatment. It is essential to inform the patient also about the after-effects, side effects and possible complications, all that considering their significance and incidence. Information about the intervention and its risks does not limit the patient's right for compensatory damages, but it frequently improves communication and mutual collaboration between the patient and health care workers.

An analysis of the current situation in the surgical treatment of solid malignant tumors in the Czech Republic demonstrates the need to impose a concept of oncosurgical care, which would lead to a steady improvement in the quality of care provided by surgeons in oncosurgery. The primary aim is that surgery plays an appropriate role in the complex care of oncology patients and that surgeons become equal partners to radiation and internal oncologists in the determination of diagnostic - therapeutic plans and in other decision-making processes. This aim is not possible without increasing the qualification of surgeons in oncosurgery. Our more than ten year effort culminated in 2011 with the introduction of the specialized field of oncosurgery into the postgradual education of surgeons; and as such, similarly to most other developed countries, provided the official opportunity to educate surgeons in the field of oncology. Other important tasks which would contribute to increasing the quality of oncosurgery include rational concentration of oncosurgical operations to a smaller number of surgical departments, where it is necessary to ensure both the improvement of surgeon qualification by way of oncosurgical specialization, as well as adequate material and financial support for complex oncosurgical procedures.

The authors examined in 45 patients with multiple injuries total serum cholesterol levels and the packed cell volume on admission, 24 hours and 48 hours after injury. The cholesterol level in these patients declined as compared with a control group to 0.61 on admission of the patient (on average 102 min. after injury) and to 0.51 and 0.53 24 and 48 hours after injury, while the packed cell volume declined to 0.76 on admission and to 0.64 24 and 48 hours after the injury. The lose correlation between cholesterol and the packed cell volume (r2 = 0.27) suggests the part played by another mechanism of development of hypocholesterolaemia in patients with multiple injuries than mere haemodilution. In eight patients who died no correlation with hypocholesterolaemia was found.
