During the period between 2/2000 and 11/2000 21 patients with severe ventricular dysfunction were investigated (EF less than 30%) where in a planned manner intraaortic balloon counterpulsation was introduced before cardiac surgery. The low mortality (5%), zero incidence of complications caused by the method and the improvement of the contractility of the heart muscle, evaluated from a rise of the ejection fraction (EF) make so-called prophylactic use of contrapulsation possible as a method of first choice in severe myocardial dysfunction.

The authors summarize their experience with intraaortic balloon counterpulsation (IABC) at the Institute for Clinical and Experimental Medicine between 1978-1984. A total of 756 cardiac surgery procedures were performed for ischaemic heart disease (IHD), and 266 procedures for rheumatic heart disease (RHD). Of patients operated on for IDH, IABC was used in 38 (5%) and, of those undergoing surgery for RHD, in 38 cases (14%). Overall, IABC was employed in 76 patients, i.e., 7%. Ventricular assistance using IABC was successful in 55% of patients. The technique was significantly more successful in IHD (68%) than in RHD (42%) patients. IABC was less successful in IHD patients suffering myocardial infarction intraoperatively. Moreover, the success of IABC depended on its early initiation. Eleven patients (14%) developed complications associated with IABC. The data do not differ from results reported by world's leading centres.

Sixteen critically ill atherosclerotic cardiac patients required intra-aortic balloon counterpulsation (IABP) for uncontrollable chest pain, hypotension or shock in connection with percutaneous transluminal coronary angioplasty (PTCA). 1.75 coronary artery angioplasty/patient were attempted. An angiographic and an immediate clinical success was achieved in 27 out of 28 attempts (96% primary success rate). All patients received heparin and antiplatelet agents. After PTCA, haemodynamic stabilization was achieved and IABP support was stopped in 14 to 96 hours. The percutaneous therapeutic procedures were well tolerated. Three patients died: one of cerebrovascular accident not related to cardiovascular problems, two other patients died suddenly 1 and 3 months later. Two patients required coronary bypass surgery, one a repeated angioplasty on a reoccluded left anterior descending saphenous vein bypass. 12 of the 13 survivors are asymptomatic after a 15-months follow-up period. IABP is a useful adjunct to PTCA in patients with severely damaged coronary circulation and compromised haemodynamics.

AIM: To evaluate the use of intra-aortic balloon pump (IABP) at the Department of Cardiac Surgery, Charles University, Faculty of Medicine in Hradec Králové, University Hospital Hradec Králové and identifying the complications of this method and their frequency in long-term follow-up. METHOD: Retrospective analysis of the outcome of IABP use. From September 1994 to September 2007, 10,024 cardiosurgery operations were performed at the Department of Cardiac Surgery, University Hospital Hradec Králové, Czech Republic. IABP was applied in 363 cases (3.6%). Therapeutic indication for initiation of IABP at our department was the syndrome of low cardiac output in the postoperative period and during operation (difficulty in weaning from cardio-pulmonary bypass) and severe acute ischemic changes of myocardium refractory to pharmacotherapy. Prophylactic IABP was elective introduction of support before cardiac surgery in hemodynamically stable high risk patients. OUTCOME: From the group of 363 counterpulsated patients 192 were succesfully treated (52.9%), 171 patients (47.1%) died in consequence of cardiogenic shock and multiorgan failure. In 20 cases (5.5%) the IABP was introduced before the operation. Complications were observed in 61 patients (16.8%). Vascular complications were found in 27 cases (7.4%) including ischemic changes of the limb (14 cases 3.9%), significant bleeding occurred at the site of puncture (7 cases 1.9%), dissection of the femoral and iliac arteries (2 cases--0.55%), perforation of the iliac artery (1 case--0.3%). In 1 case (0.3%) the balloon was removed for intramural hematoma of the descending aorta without dissection. In 2 cases (0.55%) the balloon was led into the venous system. Thrombocytopenia occurred in our group in 23 patients (6.3%). Technical complications were observed in 7 cases (1.9%). None from our group of couterpulsated patinets had infectious complication. CONCLUSION: IABP is an effective and clinically verified mechanical cardiac support. Our results are similar to other studies.

To determine whether haemodynamic effective support of the failing heart can prevent spontaneous irreversible ventricular fibrillation (IVF) after experimental cardiogenic shock, 63 open chest dogs were assigned randomly into four groups--two control groups (combined heart failure--CHF) and two MCHSS-treated (combined heart failure and non-pulsatile bypass with intraaortic counterpulsation). The CHF was produced by occlusion of the anterior descending coronary artery (group I) or the left circumflex coronary artery (group II) and by an artificial shunt between the pulmonary artery and right atrium. IVF were observed in 10 out of 22 control dogs (group I) and im 17 out of 21 control dogs (group II). In the 20 dogs with the CHF (10 animals in each group) the MCHSS was performed in the course of two hours. During the heart support no IVF were observed in any of the dogs. The difference between the incidence of IVF in the control and treated animals is statistically significant (Chi square = 21.9767 greater than Chi square 0.005).
