INTRODUCTION: The aim of this study was to evaluate a group of bariatric patients operated at the Military University Hospital in Prague during the last 10 years (20112020), in whom laparoscopic sleeve gastrectomy was performed. METHODS: Retrospective survey of the internal operation database. The search used the following combination of keywords: “sleeve“, “LSG“ and the diagnosis “E6*“. A total of 279 operated patients were enrolled. We evaluated the sex, age at the time of surgery, complications, need for drainage, weight, BMI, presence of type two diabetes mellitus and any effect of the surgery on its improvement, length of hospital stay, follow-up duration and % excess weight loss. RESULTS: A total of 279 patients, including 195 women and 84 men, underwent laparoscopic sleeve gastrectomy in the period of 10 years. The mean age was 44.46 years. The average operating time was 111 minutes. The mean BMI of the patients before surgery was 42.24 and the weight was 123.4 kg. The mean BMI one year after the surgery corresponded to a decrease of approximately 10 and the mean weight of 93.8 kg. Rather severe acute postoperative complications occurred in 2.87% patients. An improvement or complete cure of type two diabetes mellitus was observed in 57.8% patients. CONCLUSION: Currently, laparoscopic sleeve gastrectomy is the most common bariatric operation at the Military University Hospital in Prague. This study demonstrates a satisfactory effect of bariatric surgery in terms of long-term significant weight loss and an improvement or even cure of associated diseases such as type two diabetes mellitus, arterial hypertension and others.

The purpose of this contribution is to inform about the activities of the local ethics commission established at one of our largest teaching (University) hospitals. In 1995 the following priorities were recognized: studying international ethical standards of research involving human subjects and strictly adhering to them; bilingual administrative agenda in Czech and English; enforcing the patients' rights in health care and emphasizing them in education of medics and other students of health related disciplines; stimulating effective communication between doctors, other health care workers and lay persons in the commission as well as between the members of the commission and authors of research projects applying for ethical evaluation. Since 1993 the commission has been revising and storing in the archives more than 600 research projects. In 1996, 181 new research projects were reviewed, which is 35% more than in 1995. Approval was granted in 154 cases (85.08%), 3 cases (1.66%) were rejected, 15 cases (8.29%) approved on condition of complying with the recommendations of the commission, 6 cases (3.31%) recommended for another commission, and in 3 cases reviewing the project was postponed for lack of necessary data. The members of the commission agree that the future activities should lead to forming a broader group of consultants of the commission and other professional and lay people interested in clinical ethics. According to their meaning, legislation concerning ethical commissions in the Czech Republic, including material and financial support and administrative help, is urgently needed.

AIM: Evaluation of screening and treatment of retinopathy of prematurity (ROP) at the Department of Pediatric Ophthalmology. MATERIAL AND METHODS: Retrospective evaluation of the medical records of premature babies, born in the period 2012-2022 and treated at the Neonatology Department and the Neonatology ICU at the University Hospital Brno. On average 150 children annually are put forward for screening of ROP. A total of 1694 premature infants were examined during the 11-year monitored period. RESULTS: There were 100 patients who reached the stage of ROP requiring treatment, and the results showed that the most endangered group were those with a birth weight below 1000 g. The gestational age at the start of therapy was also assessed. Treatment of ROP was most often carried out by means of a combination of laser photocoagulation of the retina and intravitreal application of anti-VEGF (35 patients), as well as monotherapies: application of anti-VEGF (30 patients), laser photocoagulation of the retina (28 patients) and cryotherapy (2 patients in total). None of the patients reached ROP stage 4 or 5. The number of patients in need of treatment in the past three years has decreased significantly. CONCLUSION: The article documents the screening and trend in the treatment of retinopathy of premature infants in recent years. Of interest here are the changes in the incidence of ROP from 2020, which reflect the positive development of neonatological care. Retinopathy of prematurity is still a disease that can lead to blindness of premature infants, but early screening and treatment can prevent this. Progress in the care of newborns born prematurely is also positively reflected in the incidence of this serious disease.
