Současné postavení durvalumabu v léčbě pacientů s plicním karcinomem – přehled
[Current status of durvalumab in the treatment of patients with lung cancer – an overview]

Imunoterapie nádorů plic zaznamenala v posledních letech výrazný posun, když se v případě durvalumabu dostala do léčebných algoritmů jak malobuněčného, tak nemalobuněčného plicního karcinomu. V případě lokálně pokročilého nemalobuněčného karcinomu získala terapie durvalumabem registraci i úhradu pro pacienty exprimující ligand programované buněčné smrti 1 (programmed cell death-ligand 1, PD-L1) na povrchu nádorových buněk a současně absolvovali kurativní chemoradioterapii bez známek progrese onemocnění. V případě pokročilého malobuněčného plicního karcinomu přidání durvalumabu ke dvojkombinaci chemoterapie zaznamenalo statisticky signifikantní zlepšení přežití a tvoří jednoznačně nový standard léčby těchto pacientů. U metastazujícího nemalobuněčného karcinomu získala trojkombinace chemoterapie s durvalumabem a tremelimumabem registraci, nicméně bez úhrady a s ohledem na řadu molekul s úhradou v této indikaci nebude lehké najít jednoznačné postavení v léčebném algoritmu.
There was a significant progress in the lung cancer immunotherapy in recent years. Durvalumab is currently in the treatment algorithm of small cell as well as non-small cell lung cancer. Immunotherapy with durvalumab gained the registration and also health care reimbursement in the treatment of the locally advanced non-small cell lung cancer with programmed cell death-ligand 1 (PD-L1) expression on the tumor cell surface after radical chemoradiotherapy without any signs of disease progression. In the case of small cell lung cancer adding durvalumab to chemotherapy combination showed statistically significant improvement in overall survival and is currently gold standard for these patients. In the case of metastatic non-small cell lung cancer the combination of chemotherapy with durvalumab and tremelimumab gained the registration, however without the reimbursement so far. I will be difficult to find appropriate position of this combination with respect to number of molecules available in this indication.
