Autori prezentujú pacientku s diastematomyéliou (DSM) a k nej pridruženými vrodenými vývojovými chybami chrbtice a miechy, ktoré boli pričinou syndrómu pripútanej miechy (TCS). Pacientka mala od narodenia prítomné kožné známky malformácie - vlasy a névus v bedrovej oblasti. Vrodená chyba bola diagnostikovaná a pacientka operovaná v 5. roku života. MRI a operačný nález potvrdili prítomnosť DSM, zhrubnutého a pripútaného filum terminále a lipómu filum terminále. Pri operácii bolo odstránené kostěné septum, excidovaný lipóm a prerušené zhrubnuté pripútané filum terminále, teda uvoľnená miecha. Každá z uvedených anomálií svojím podielom zapríčiňovala pripútanie miechy a bránila jej fyziologickému vzostupu. Dôkladná chirurgická revízia a kompletné odstránenie všetkých príčin pripútania miechového povrazca boli základom dobrého pooperačného výsledku. DSM je vrodená chyba, ktorá má mnoho variácií a môžu k nej byť pridružené ďalšie malformácie. Uvedená kazuistika je typickým príkladom toho, ktoré z chýb sa môžu vyskytovať spolu s DSM.

The authors present a patient with diastematomyélia and associated congenital malformations of the spinal cord. They caused tethering of the spinal cord. Patient had cutaneous marks of malformation from birth. The congenital malformation was operated at the age 5 years. MRI and surgical findings confirm the existence of diastematomyélia together with thick tethered filum terminále and lipoma of filum terminále. On surgery the bony septum was removed, the filum terminále was resected and the lipoma was excised. Each of the anomalies caused the tethering of the spinal cord and restricted its physiological ascent. Only careful surgical revison with subsequent removal of all causes of the tethering of the spinal cord produced good result. Diastematomyélia is a congential malformation with some morphological variations and coexistence of other malformations. The presented case is a typical example of malformations associated with diastematomyelia.
