Léčba by vždy měla vést k dosažení optimální kompenzace diabetu. Názory na léčbu inzulinem u pacientů s diabetes mellitus 2. typu nejsou vždy jednotné. Zatímco inzulinoterapie u diabetes mellitus 1. typu je nezpochybnitelná, u diabetu 2. typu je spojena s řadou možných problémů a z toho vyplývajících chyb. U těchto pacientů je vždy nutno pečlivě zvažovat celkový zdravotní stav pacienta a indikaci léčby inzulinem, to by však nemělo znamenat oddalování či nezahájení léčby u indikovaných pacientů.Mnoho nemocných je k léčbě inzulinemindikováno i po krátkém trvání diabetu pro selhání perorálních antidiabetik, pozdní komplikace či jiné důvody. Nelze zapomenout ani na nutnost selfmonitoringu a komplexní edukace těchto pacientů. Článek podává jednoduchý přehled o indikacích a způsobech léčby inzulinemdiabetes mellitus 2. typu.

Treatment should always lead to optimal compensation of diabetes. Views regarding insulin treatment of patients with type 2 diabetes are not always uniform. While insulin therapy in type 1 diabetes is beyond doubt, in type 2 diabetes it is associated with a number of possible problems and errors ensuing from them. In these patients it is always necessary to consider carefully the general health status of patients and indication of insulin treatment but this should not imply postponement or abondonement of treatment in indicated patients. Some patients are indicated for insulin treatment even after brief persistence of diabetes on account of failure of oral antidiabetics, late complications or other reasons.We must not forget the need of self-monitoring and comprehensive education of these patients. The paper gives a brief account of indications and ways of insulin treatment in patients with type 2 diabetes mellitus.
