Kochleárna implantácia je spoľahlivá metóda na rehabilitáciu sluchu prelingválne nepočujúcich detí a postlingválne nepočujúcich dospelých. Ako každá chirurgická metóda, aj kochleárna implantácia má svoje problémy, komplikácie a neúspechy. V práci sa analyzuje počet revíznych operácií a indikácie na tieto revízie. Hodnotia sa spoľahlivosť prístrojov v celom súbore a funkčné výsledky pacientov po reimplantácii. Na I. otolaryngologickej klinike LF UK, FN a SZU v Bratislave bolo od roku 1994 implantovaných 95 pacientov. U šiestich z nich (pät mužov, jedna žena, 6,3 %) bola potrebná revízna operácia. Indikáciami na revízne operácie boli: Zlyhanie implantátu v dvoch prípadoch, vylúčenie implantátu v dvoch prípadoch, poškodenie implantátu úrazomv jednoma iatrogénne poškodenie v jednom prípade. U všetkých operovaných, okrem jedného, sme dosiahli spoľahlivú reimplantáciu s primárnym hojením operačnej rany. V celom súbore našich 95 pacientov zlyhali dva implantáty Medel Combi 40+. Čas potrebný na dosiahnutie pôvodných výsledkov bol 6,3 mesiaca v priemere. V súčasnosti všetci implantovaní pacienti po reimplantácii dosahujú výsledky aspoň na takej úrovni ako pred reimplantáciou alebo lepšie. Revízna operácia po kochleárnej implantácii je bezpečná a úspešná operácia s veľmi dobrými anatomickými a funkčnými výsledkami.

Cochlear implantation is a safe and effective clinical method to restore hearing and communication in deaf patients. As is the case in any other clinical methods there are some problems, failures and complications. In this paper the series of patients that required surgical revision due to various reasons are evaluated. Surgical, anatomical and functional results are evaluated. From 1994 the authors have implanted 95 deaf patients. There were 6 (6,3 %) reoperations in this series of patients. The indication for reoperation were: device failure in 2 cases (Medel C40+), flap breakdown in 2 cases, head injury in 1 case, iatrogenic damage to electrodes in one case. Surgical and anatomical results of all except one revisions were excellent. Functional results in all 6 patients confirmed that they all reached at least the same results as with the first implant if not better soon after the revision surgery. The average time to reach original results was 6.3 months after revision. Cochlear re-implantation is a safe and effective surgical procedure that gives very good functional, anatomical and surgical results.
