Cíl práce: Zjistit účinnost, snášenlivost a bezpečnost vaginálně podávaného estriolu u postmenopauzálníchžen s atrofi zací urogenitálního ústrojí.Typ studie: Prospektivní klinická studie.Název a sídlo pracoviště: Gynekologicko-porodnická klinika VFN a 1. LF UK, Praha.Metodika: Do studie bylo zařazeno 68 postmenopauzálních žen s dělohou a s příznaky organickéhoestrogen-defi citního syndromu, které si 24 měsíců 1-3krát týdně aplikovaly vaginální kréms obsahem estriolu. Zjišťovali jsme subjektivní i objektivní změny v urogenitálním systému, změnylaboratorních hodnot, denzity prsní tkáně a kostí.Výsledky: Potvrdily se příznivé účinky lokálně podávaného estriolu především na močové obtížea na rekurentní infekce dolních cest močových. Kontinence výrazně ovlivněna nebyla. U všechžen byly prokázány výrazně pozitivní účinky již po 3 měsících užívání (65 %), s maximem po 6měsících (85 %), kdy spokojenost s léčbou přetrvávala, ale dále se výrazně nezvyšovala. Nebylypozorovány žádné změny v biochemii, ultrazvukovém vyšetření endometria, mamografi i, denzitometrii,krevním tlaku a hmotnosti. K statistickému zhodnocení byl použit t-test.Závěr: Aplikace vaginálního estriolu je vhodnou volbou pro ženy s klinickými symptomy (urgence,dysurie, polakisurie, nykturie, rekurentní infekce dolních cest močových) při atrofi zaci tkánídolních cest močových a pochvy.

Objective: The objective of this work was to evaluate the effi cacy, acceptability and safety of thevaginal estriol on symptoms in postmenopausal women with urogenital atrophy.Design: Prospective clinical study.Setting: Department of Obstetrics and Gynecology of the 1st Medical Faculty and General FacultyHospital, Prague.Methods: We included in our study 68 postmenopausal women with proved urogenital symptomsof the atrophic genitourinary tract. We administration them two years vaginal estriol 0.5 mg daily,later 1-2 times a week and than detected subjective and objective changes in their low urinarytract and vagina. These women had a normal biochemistry, uterus, mammary and bone density.These parameters were evaluated before starting the treatment and after 3 and 6 month. Thestatistical signifi cance of the phenomen was evaluated by exact t-test.Results: Statistically signifi cant positive differences and remission were observed in all symptomsand recurrent infection of the atrophical urogenital tissues.The fi rst satisfaction was after 3th month and the best after 6th month of this therapy, and inthe end of this study (24 month) it was the same. We recorded no difference in other monitoredparameters (endometrium, biochemistry, mammography, densitometry, blood pressure, weight)and compliance was satisfactory (85% after 6 months). In the 6 month profi le of estriol we founda signifi cant decrease of the Kupperman index and the Menopause rating scale.Conclusion: Administration of vaginal estriol is an advisable choice for every postmenopausalwoman with the genitourinary problems (urgency, pollakisuria, dysuria, nycturia, recurrent infectionof the low urinary tract). It has very good clinical effects and minimum side effects.
