Laboratorní testy mají v diagnostice alergických onemocnění a monitorování alergenové imunoterapie důležité, ale vymezené postavení. V článku je diskutována problematika testů pro stanovení alergen- specifických imunoglobulinů, testů využívajících zjišťování aktivace bazofilů periferní krve po stimulaci alergenem (bazotestů, testů měřících uvolňování histaminu a CAST), testů na monitorování poměru Th1 a Th2 odpovědi po alergenové stimulaci i testů na stanovení vybraných biologicky aktivních látek podílejících se na průběhu alergické reakce (ECP, triptáza, histamin). Je upozorněno na význam standardizace alergenů používaných v těchto testech a na význam autoprotilátek, které vznikají především v průběhu dlouhotrvajících a intenzivních alergických reakcí.

Laboratory tests are of important but limited value in allergy diagnosis and monitoring of allergen immunotherapy. Questions linked with application of these tests for allergen-specific immunoglobulindetermination, allergen-induced basophile activation (basotests, histamine-release tests and CAST), tests for monitoring of Th1 and Th2 response after allergen challenge and the assessment of selected allergy-associated substance (ECP, tryptase, histamine) are discussed. Importance of allergen standardization for laboratory measures and autoantibodies produced in severe allergies and under long-term allergen challenge is also discussed.
