Těžká plicní hypertenze představuje u nemocných s terminálním srdečním selháním kontraindikaci transplantace srdce pro riziko selhání pravé komory srdeční v časném pooperačním období, kdy je tato komora náhle vystavena vysokým tlakům v malém oběhu. Popisujeme kasuistiku nemocného s pokročilým srdečním selháním a závažnou plicní hypertenzí, u něhož došlo k příznivému ovlivnění hemodynamických parametrů léčbou sildenafilem.

In patients with end-stage heart failure, severe pulmonary hypertension is a contraindication to heart transplantation because of the risk for right ventricular failure in the early postoperative period, when the ventricle becomes suddenly exposed to high pressure in the pulmonary circuit. We present the case report of a patient with advanced heart failure and severe pulmonary hypertension, whose hemodynamic parameters were favorably modulated with sildenafil therapy.
