Obsedantně-kompulzivní porucha je v domácí literatuře líčena jako porucha s náhledem. Je to v rozporu s četnými zahraničními poznatky o významné proporci pacientů, kteří nemají dobrý nebo žádný náhled, tj. nejenže nevyhledávají léčbu, ale zcela jim chybí pocit vlastního zdravotního problému. Interpretace vyhýbání se nebo pozdního vyhledání léčby těmito pacienty a připisování pozdní léčby výhradně obavě ze stigmatizace musí napříště s častým chybením náhledu počítat. Zhruba čtvrtina obsedantů v populaci nemá náhled. Je pravděpodobné, že představují svéráznou podskupinu heterogenní obsedantně-kompulzivní poruchy.

In the home literature the obsessive-compulsive disorder has been described as comprising full insight. This is in contrast with the numerous findings abroad concerning the substantial proportion of patients having poor or no insight, e.g. seeking no help and not feeling any health problem. In future the explaining the avoidance of the patients and their late help-seeking solely by their fear of the stigmatization must include also their frequent absence of insight. Fourth part of the people suffering from the obsessive-compulsive disorder are probably lacking insight. It seems they represent a special subgroup of the heterogenous obsessive-compulsive disorder.
