Vzácná příčina hypoglykemie u novorozence – Beckwith-Wiedemannův syndrom

Autoři prezentují případ novorozence s hypoglykemiemi, u kterého vzniklo podezření na vzácně se vyskytující Beckwith-Wiedemannův syndrom (BWS). V klinickém obraze tohoto syndromu dominují vedle hypoglykemií makrosomie, makroglosie, hemihypertrofie a různě vyjádřená patologie pupečníku. V teoretické části je věnována pozornost vedle klinických příznaků i genetickým souvislostem BWS a diferenciální diagnóze novorozenecké hypoglykemie.
We present a case report of a 64years old patient with the type 2 diabetes mellitus (DM) with a progression of his disease. His treatment with metformin was gradually augmented by a non-sulphonylurea secretagogue repaglinide with excellent result. After 12 years from the diagnosis of the type 2 diabetes mellitus the intensification of the therapy was essential. Insulin basal in the bed-time regimen was added to the pharmacotherapy. Due to higher postprandial glycemia after breakfast the long-acting analogue was supplemented by a short-acting analogue in a single daily dose, which corrected the insulinoresistance that had been remaining till morning hours.
