A 72-year-old non-smoker appeared for clinical examination complaining about occasional pain in the second and third quadrant. He had visited different dental offices before where different surgical and prosthetic approaches had been proposed. He wanted to get another opinion. Several implants were placed about 15 years ago the lateral segments of lower and upper jaw followed by a prosthetic reconstruction. Clinical examination revealed good overall condition of oral tissues, except for a chronic marginal inflammation around tooth 25 and implant 46. Large periodontal and periimplant defects are visible around tooth 25 and implant 46. Tooth 25 was extracted due to a vertical root fracture, whareas and attepmt of saving implant 46 was made by ultrasonic cleaning of the surface and a free gingival graft placed buccally from the implants 46 and 47 to stabilize soft tissues. One year later a surprising healing effect was confirmed by both clinical and radiological examinations. Removal of the implant is not a reasonable option anymore and a implant reconstruction is beeing planned for the second quadrant.
