Využitie n-acetyl-β-d-glukozaminidázy/kreatinínu v diagnostike akútnej pyelonefritídy [Utilisation of n-acetyl-β-d-glucosaminidase/creatinine in diagnostics of acute pyelone phritis]

Východisko: Akútna pyelonefritída je časté a závažné ochorenie detského veku. Cieľom našej štúdie bolo zistiť aká je výpovedná hodnota N-acetyl-β-D-glukozaminidáza/kreatinínu (NAG/kreatinín) pri diagnostike akútnej pyelonefritídy. Súbor a metódy: Sledovali sme súbor 61 detí (47 dievčat, 14 chlapcov) vo veku 0-19 rokov (vekový priemer 4,4 roka) s 1. atakom akútnej pyelonefritídy. U pacientov sme vyhodnocovali hladinu C-reaktívneho proteínu (CRP) a množstvo leukocytov v krvi a prítomnosť leukocytózy a bakteriúrie v moči. Zároveň bola vyhodnocovaná hladina NAG/kreatinín v moči u 39 pacientov. Tieto parametre sme porovnávali s hodnotami získanými z kontrolného súboru 29 zdravých detí. Výsledky: Deti s pyelonefritídou vykazovali signifikantne vyšší počet leukocytov a vyššie hodnoty CRP a NAG/kreatinínu ako deti v kontrolnej skupine. Záver: Vzhľadom na naše výsledky odporúčame zvážiť vyšetrenie NAG/kreatinínu hlavne v nejednoznačných prípadoch akútnej pyelonefritídy a pri sledovaní úspešnosti liečby.
Background: Acute pyelonephritis is a frequent and serious disease of childhood. The aim of our study was to find out what is the statement value of N-acetyl-β-D-glucosaminidase/creatinine (NAG/creatinine) in diagnostics of acute pyelonephritis. Patient collection and methods: We observed the collection of 61 children (47 girls, 14 boys) at the age of 0-19 years (age average 4.4 years) with 1st attack of acute pyelonephritis. In patients we evaluated the level of C-reactive protein (CRP) and the amount of leucocytes in blood and the presence of leucocytes and bacteriuria in urine. At the same time the level of NAG/creatinine was evaluated in the urine of 39 patients. We compared these parameters with the values from control collection of 29 healthy children. Results: The children with pyelonephritis showed significantly higher number of leucocytes and higher values of CRP and NAG/creatinine than in the children in the control group. Conclusion: Regarding our results we are recommending to consider the examination of NAG/creatinine mainly in ambiguous cases of acute pyelonephritis and in the monitoring of treatment success.
