Léčba onkologických pacientů je velmi specifická, zvláště jde-li o mladé jedince. Ještě složitější situace nastává u pacientů, jež postihlo onkologické onemocnění v dětském věku a u nichž byla součástí léčby radioterapie, která často vede ke značnému poškození tvrdých i měkkých tkání obličeje. Následkem tohoto procesu bývá dentice postižena agenezemi, malformacemi zubních zárodků, resorpcemi kořenů nebo ankylózou zubů. Autoři demonstrují léčbu 16letého pacienta, u něhož byl ve 3 letech věku diagnostikován maligní nádor sítnice, retinoblastom. Léčba spočívala v kombinaci chirurgické terapie (enukleace oka) a radioterapie. Jejím následkem došlo k malformacím měkkých a tvrdých tkání obličeje, zvláště devastující byl vliv na zubní zárodky. V horní čelisti byly u daného pacienta pozorovány ageneze několika stálých zubů, ankylózy dočasných a stálých zubů, transpozice a malformace zbylých zubních zárodků. Ve správné pozici a bez poškození byly v horní čelisti prořezány pouze dva stálé zuby. Dolní zubní oblouk nebyl vyjma ageneze jednoho zubu poškozen. Autoři prezentují složitý léčebný protokol založený na interdisciplinární spolupráci mezi ortodontistou, implantologem, dentoalveolárním chirurgem a protetikem, jehož základem byla autotransplantace tří zubů a zavedení tří implantátů. U pacienta však po ortodontické fázi terapie, z důvodu velké diskrepance mezi střední čárou horního zubního oblouku a výraznou asymetrií hrotu nosu a tváře, došlo ke změně protetického léčebného plánu. Místo jednotlivých korunek na implantátech byla preferována částečná šroubovaná konstrukce nesená implantáty, která umožnila korekci polohy střední čáry horního zubního oblouku vzhledem k asymetrii obličeje. Tak mohlo být dosaženo co nejlepší estetiky úsměvu a dentofaciální harmonie.

The treatment of oncology patients is very specific – especially for young patients. Moreover, the patients who underwent the oncology treatment together with radiotherapy in their childhood have to face even more complex situations. Radiation therapy often damages facial tissues, bones as well as teeth. There occur tooth agenesis, malformation of tooth germs and root resorption or ankylosis. The authors show here a case study of a 16-year-old patient who suffered from retinoblastoma at the age of three. His treatment included surgical therapy (enucleation of the eye) and radiation therapy. Due to the radiotherapy, soft and hard facial tissues got malformed and tooth germs were severly damaged: situation in the upper jaw – malformed tooth germs, agenesis of several teeth, ankylosis and transposition. Only two teeth in their right position and without any damage erupted in the upper jaw. The authors present a complex treatment protocol based on interdisciplinary cooperation among the orthodontist, implantologist, dentoalveolar surgeon and prosthodontist. The core of the treatment consisted of autotransplantation of three teeth and insertion of three implants. Because of distinct nasal and facial asymmetry, the prosthodontic part of the treatment plan had to be modified. Using a partial screw denture set on implants, the midline of the upper dental arch got corrected according to the facial and nasal midline. The best smile esthetics and dentofacial harmony were reached this way.
