HER2-pozitivní karcinom prsu se vyskytuje u 15–20 % nemocných s diagnózou maligního onemocnění prsu a je charakterizován jako agresivní podtyp tohoto onemocnění. S rozvojem cílené anti-HER2 terapie v kombinaci s chemoterapií došlo k zásadnímu zlepšení léčebných výsledků a prognózy nemocných s metastatickým i časným HER2- -pozitivním karcinomem prsu. Neoadjuvantní anti-HER2 duální blokáda s trastuzumabem a pertuzumabem v kombinaci s chemoterapií umožňuje zvýšit počet nemocných, které dosahují patologické kompletní odpovědi, což vede ke zlepšení dlouhodobých léčebných výsledků v parametrech doby bez relapsu choroby (EFS) i celkového přežití (OS). Pacientky s reziduální chorobou po neoadjuvanci mají prospěch z terapie s T-DM1 a z léčby neratinibem.

HER2-positive breast cancer is a particularly aggressive subtype that affects 15% to 20% of patients with breast cancer. HER2-targeted therapies in combination with chemotherapy have dramatically improved treatment results and prognosis of patients with metastatic and early HER2 positive breast cancer. Neoadjuvant treatment with trastuzumab and pertuzumab in combination with chemotherapy demonstrated a higher pathological complete response rate and improved long-term outcomes in parameters of event-free survival (EFS) and overall survival (OS). Patients with residual disease after neoadjuvant treatment have profit from salvage therapy with adjuvant T-DM1 and neratinib.
