Uvedená kazuistika představuje nepříliš typického diabetika 2. typu, u něhož nebylo nutné řešit neuspokojující kompenzaci, ale pouze mírné zhoršení jeho doposud úžasně korigovaného diabetu. Tento případ poukazuje na důležitost edukace pacienta v době záchytu diabetes mellitus 2. typu s nastavením mezí doporučených glykemií, které pacient ochotně převzal za své, a byl proto úspěšný v jejich dodržování. Současně se potvrdila hypotéza stran dlouhodobě působícího inzulínového analoga degludek, jenž u pacienta nahradil do té doby vyhovující analog s kratší dobou působení (detemir) a zajistil nejen udržení parametrů kompenzace diabetes mellitus 2. typu ve zcela vyhovujících hodnotách, ale dokázal je ještě zlepšit.

The case report presents a case of a rather atypical type 2 diabetic. There was no significant issue with unsatisfactory control to solve, only a slight worsening of his hitherto wonderfully corrected diabetes. This case highlights the importance of educating the patient at the time of detection of type 2 diabetes mellitus and setting the limits of the recommended glycaemia, which the patient was willing to adopt as his own and was therefore successful in adhering to them. At thesame time, a hypothesis was confirmed about the long-acting insulin analogue degludec, which replaced a hitherto satisfactory analogue with shorter duration of action (detemir) and not only maintained the parameters of type 2 diabetes mellitus control in completely satisfactory values, but was even able to improve them.
