Cíl práce: Hodnocení pacientů zařazených do registru biologické/cílené léčby BIOREP v České republice za rok 2022. Metody: Bylo provedeno retrospektivní hodnocení pacientů k datu 31.12.2022 zařazených do registru BIOREP s diagnózou psoriázy, hidradenitis suppurativa a atopické dermatitidy léčených biologickou/cílenou léčbou v daném období. Výsledky: K 31. 12. 2022 bylo v registru BIOREP evidováno celkem 5 335 pacientů. S psoriázou bylo sledováno 4 067 pacientů (76,2 %), s hidradenitidou 443 pacientů (8,3 %), s atopickou dermatitidou 808 pacientů (15,1 %) a zbývajících 17 pacientů (0,3 %) s použitím „off-label“ léčby. Ve skupině léčených pro psoriázu bylo 62,9 % mužů, průměrný věk pacientů ke konci roku 2022 byl 52,1 roku, u psoriázy byl průměrný věk v době diagnózy 25,2 roku a při nasazení první biologické/cílené léčby 46,1 roku. Průměrná doba od diagnózy do zahájení první cílené léčby byla 20,9 roku. Ke konci roku 2022 byli pacienti léčeni cílenou léčbou v průměru 5,2 roku. Přidružené onemocnění se vyskytovalo u 70,4 % pacientů, nejčastější byla metabolická/endokrinní onemocnění (38,6 %) a kardiovaskulární onemocnění (37,3 %). Ze sledovaných komorbidit byla nejčetnější hypertenze (35,1 %), dyslipidemie (26,6 %) a diabetes mellitus prvního nebo druhého typu (12,1 %). Celkem 76,1 % pacientů trpělo nadváhou či obezitou a v registru bylo 32,2 % kuřáků. Souběžnou psoriatickou artritidu mělo 32,2 % pacientů. Při zahájení léčby a při poslední návštěvě bylo průměrné PASI 18,7 resp. 1,8, BSA bylo v průměru 29,4 %, resp. 2,4 % a průměrné DLQI 16,1, resp. 1,5. Po 12 měsících léčby dosáhlo PASI50 celkem 91,8 % pacientů, PASI75 82,3 % pacientů, PASI90 63,8 % pacientů a PASI100 40,1 % pacientů. V roce 2022 nově zahájilo léčbu celkem 514 pacientů, na konci sledovaného roku 2022 bylo aktivně léčeno celkem 3 480 pacientů a nejčastěji používaným preparátem byla Humira (17,2 %), Skyrizi (13,6 %) a Cosentyx (13,2 %). S diagnózou hidradenitis suppurativa bylo v registru na konci sledovaného období celkem 443 pacientů, z toho 54,4 % mužů, průměrný věk byl 44,6 let. V době diagnózy bylo pacientům v průměru 33,0 let a v době nasazení v průměru 42,2 let. Průměrná doba od diagnózy do zahájení první biologické léčby byla 9,3 let a pacienti byli léčeni v průměru 2,4 let. V roce 2022 svou první biologickou léčbu zahájilo celkem 94 pacientů. Na konci sledovaného roku bylo léčeno celkem 367 pacientů, průměrná délka léčby jedním léčivým přípravkem byla 2,3 let a nejčastěji použitým preparátem byla Humira (84,7 %). S atopickou dermatitidou bylo v registru BIOREP sledováno k 31. 12. 2022 celkem 808 pacientů, z toho 53,5 % mužů. V roce 2022 byl průměrný věk pacientů 36,2 let. V době diagnózy onemocnění byl průměrný věk pacientů 5,0 roků a při nasazení první cílené léčby 35,2 roku. Průměrná doba od diagnózy do zahájení první cílené léčby byla 30,2 roku. Ke konci roku 2022 byli pacienti léčeni v průměru 1,5 roku. Souběžnou alergickou rýmu mělo 64,1 % pacientů, potravinovou alergii 57,7 % a alergické astma 44,3 %. Oční postižení se vyskytlo u 28,7 % pacientů, nejčastější byla atopická konjunktivitida (15,3 %). Průměrné EASI při zahájení léčby a na poslední návštěvě bylo 31,6 resp. 4,4, BSA bylo v průměru 56,4 %, resp. 8,4 % a průměrné DLQI 18,4, resp. 3,8. Po 12 měsících léčby dosáhlo EASI50 celkem 98,3 % pacientů, EASI75 91,6 % pacientů, EASI90 57,5 % pacientů a EASI100 10,7 % pacientů. V roce 2022 zahájilo cílenou léčbu celkem 306 pacientů. Na konci sledovaného roku bylo léčeno celkem 757 pacientů, nejvíce pacientů bylo léčeno přípravkem Dupixent (91,8 %) a průměrná délka léčby jedním léčivým přípravkem byla 1,4 roku. Závěr: BIOREP je prvním registrem pacientů biologické léčby psoriázy v zemích střední a východní Evropy. Analýza potvrzuje vysokou aktivitu sledovaných onemocnění podle objektivního hodnocení a značné negativní ovlivnění kvality života pacientů při zahájení cílené léčby s poklesem hodnocených parametrů při léčbě. Současně prokazuje významnou prevalenci souběžných onemocnění a rizikových faktorů a dlouhé období nedostatečné léčby před nasazením cílené léčby.

Background and objectives: Evaluation of patients included in the registry of biological/targeted therapy BIOREP in the Czech Republic. Methods: A retrospective evaluation of patients to date 31 December 2022 enrolled in the BIOREP registry was performed. The aim of our study was to evaluate patients on biological/targeted treatment in the given period in individual categories and to analyze the population of patients with psoriasis, hidradenitis suppurativa and atopic dermatitis. Results: As of 31 December 2022, a total of 5,335 patients were registered in the BIOREP registry: 4,067 (76.2%) patients with psoriasis, 443 (8.3%) with hidradenitis, 808 (15.1%) with atopic dermatitis and 17 (0.3%) were treated off label with targeted treatment. Of the psoriasis group, 62.9% of patients were men. The average patient age at the end of 2022 was 52.1 years, the mean age at the time of diagnosis was 25.2 years, and the age at first biological/targeted treatment was 46.1 years. In 2022, the average time from diagnosis to the introduction of targeted treatment was 20.9 years, the average duration of biological/targeted treatment was 5.2 years. The comorbidities occurred in 70.4% of patients. The most common disorder were metabolic/endocrine diseases (38.6%) and cardiovascular diseases (37.3%). From the individual comorbidities, hypertension (35.1%), dyslipidemia (26.6%) and diabetes mellitus (12.1%) were the most common. More than 76% of patients were overweight or obese and 32.2% of patients were smokers. Psoriatic arthritis was observed in 32.2% of patients. At the start of treatment and at the last visit, the mean PASI was 18.7 and 1.8, respectively, the mean BSA 29.4% and 2.4%, respectively and DLQI 16.1 and 1.56, respectively. After 12 months of treatment, a total of 91.8%; 82.3.%; 63.8% and 40.1% of patients achieved PASI50, PASI75, PASI90 and PASI100, respectively. In 2022, a total of 514 patients newly started treatment and at the end of year 2022, a total of 3,480 patients were treated, the most patients used Humira (17.2%), Skyrizi (13.6%) and Cosentyx (13.2%). As of 31 December 2022, there were a total of 443 patients with hidradenitis suppurativa in the registry of mean age 44.6 years and 56.13% were men. The mean age at the time of diagnosis was 33.0 years and the age at first biological treatment was 42.2 years. The average time from diagnosis to the start of the biological treatment was 9.3 years and patients were treated for an average of 2.4 years. In 2022, a total of 94 patients started the biological treatment. At the end of 2022, a total of 367 patients were treated and the average duration of treatment was 2.3 years, the most common medication was Humira (84.7%). At the end of 2022, there were a total of 808 patients with atopic dermatitis in the registry of mean age 36.2 years, 53.5% were men. The mean age at the time of diagnosis was 5.0 years and the age at first targeted treatment was 35.2 years. The average time from diagnosis to the start of the targeted treatment was 30.2 years and patients were treated for an average of 1.5 years. A total of 64.1% of patients had concomitant allergic rhinitis, 57.7% food allergy and 44.4% allergic asthma. Ocular comorbidities occurred in 28.7% of patients, the most common were atopic conjunctivitis (15.3%). At the start of treatment and at the last visit, the mean EASI was 31.6 and 4.4, respectively, mean BSA was 56.4% and 8.4%, respectively, and mean DLQI 18.4 and 3.8 respectively. In 2022, a total of 306 patients started the biological treatment. At the end of 2022, a total of 757 patients were treated and the average duration of treatment was 1.4 years, the most common medication was Dupixent (91.8%). Conclusion: BIOREP is the first registry of patients with psoriasis treated with biologics in Central and Eastern Europe. The analysis confirmed the high activity of diseases according to objective evaluation and the significant negative impact on the quality of life before initiation of targeted treatment, with a decrease in the evaluated scores during treatment. At the same time, it demonstrates a high prevalence of comorbidities and risk factors and a long diseases duration before the introduction of targeted treatment.
