Systémová léčba lokálně pokročilého či metastazujícího hormonálně pozitivního (hormone receptor-positive, HR+) karcinomu prsu s negativitou receptoru 2 pro lidský epidermální růstový faktor (human epidermal growth factor receptor 2 negative, HER2-) je v současné době většinou založena na kombinaci hormonální léčby s inhibitory cyklin dependentní kinázy 4/6 (cyclin-dependent kinase 4/6, CDK4/6). Při výběru konkrétní molekuly je třeba zohlednit jednak vlastní nádorové onemocnění, jednak potencionální nežádoucí účinky léčby. Nežádoucí účinky mohou zásadním způsobem ovlivnit compliance pacientek s onkologickou léčbou, a to do té míry, že pokračování v léčbě může být pro pacientky dokonce nepřijatelné, jak demonstrujeme v níže uvedeném textu. Přímé srovnání nežádoucích účinků jednotlivých molekul (ribociclib, abemaciclib, palbociclib) / kvality života neexistuje, nicméně existuje nepřímé porovnání ribociclibu s abemaciclibem, které jednoznačně preferuje terapii ribociclibem. Většina nežádoucích účinků / obtíží pacientek v průběhu paliativní léčby inhibitory CDK4/6 je zvládnutelná, je třeba na ně myslet a v optimálním případě jim předcházet.

Systemic treatment of the Locally advanced or metastatic hormone receptor-positive (HR+) human epidermal growth factor receptor 2 negative (HER2-) breast cancer is based on the combination of the hormonal treatment with cyclin-dependent kinase 4/6 (CDK4/6) inhibitors. In the appropriate selection of the molecule the tumor characteristics as well as potential side effects of the treatment play a role. The treatment side effects can significantly influence the patient compliance with the treatment and potentially cause the treatment refusal as demonstrated in the paper bellow. Direct comparison for the side effects or quality of life of the potential molecule (ribociclib, abemaciclib, palbociclib) is unfortunatelly not available, however there is indirect comparison of the ribociclib with abemaciclib regarding quality of life which clearly prefers the treatment with ribociclib. Majority of the side effects/patients’ difficulties during the palliative treatment with CDK4/6 inhibitors are manageable however it is necessary to manage them on time optimally prevent them.
